What is the Uttarakhand Hemoglobinopathy scheme?

Hemoglobinopathy is Uttarakhand's health-service response for people living with inherited blood disorders: chiefly Thalassemia and Sickle Cell Disease. It is delivered by the Department of Medical, Health and Family Welfare through the state National Health Mission (NHM), and its aim, as summarised on myScheme, is to provide comprehensive care, support and treatment to affected patients. Rather than a single cash payout, it is a bundle of services: screening and early diagnosis, safe blood transfusion, and financial protection for hospital treatment.

The programme sits on top of the national framework. The Ministry of Health began structured work on hemoglobinopathies in 2016 with national guidelines on prevention and management, and the National Sickle Cell Anaemia Elimination Mission, launched in July 2023, now drives screening and management across states. That mission targets universal population-based screening from 0 to 40 years of age in high-prevalence and tribal areas, and Uttarakhand is listed as a state with partial prevalence of sickle cell disease. Uttarakhand delivers these services through its own government health facilities and links patients to state health-cover schemes, chiefly Atal Ayushman Uttarakhand Yojana. The name "hemoglobinopathy" simply groups the inherited disorders of haemoglobin, thalassemia and sickle cell disease being the two the programme concentrates on.

What does the scheme provide?

Uttarakhand's care package for hemoglobinopathy patients draws on several linked services:

  • Screening and diagnosis, free solubility testing as a screening test and free HPLC or hemoglobin electrophoresis for confirmation, including through school and community health activities, so that cases are caught early.
  • Prophylactic and symptomatic drugs: sickle cell patients receive Hydroxyurea and Folic Acid through the government health system as part of ongoing management, alongside routine follow-up.
  • Free blood transfusion: under the Free Blood Initiative of the Uttarakhand NHM, patients admitted to government health facilities receive blood without charge. This matters most for transfusion-dependent thalassemia major patients, who need regular transfusions.
  • Cashless hospital treatment: through Atal Ayushman Uttarakhand Yojana, enrolled families get cashless in-patient treatment of up to Rs 5 lakh per family per year at empanelled hospitals, which can cover admissions, investigations and management of complications. According to the state Medical, Health and Family Welfare Department, the cover extends to more than 1,350 treatment packages across roughly 100 government and 70 private hospitals in the state, and includes 3 days of pre-admission and up to 15 days of post-discharge care.
  • Bone marrow transplant support, thalassemia bone marrow transplant is funded nationally under the Thalassemia Bal Sewa Yojana at up to Rs 10 lakh per eligible patient at empanelled centres; patients are referred through the treating hospital.

Who is eligible for the Hemoglobinopathy scheme?

You can access the scheme if:

  • You have been diagnosed with Thalassemia or Sickle Cell Disease by hemoglobin electrophoresis, HPLC or a genetic test.
  • You are a resident of Uttarakhand.
  • For cashless hospital cover, your family is enrolled or eligible under Atal Ayushman Uttarakhand Yojana (Golden Card).

Who cannot apply / is not eligible:

  • People without a confirmed hemoglobinopathy diagnosis cannot claim this specific care package, though general anaemia and other blood conditions are treated through routine hospital services.
  • For the Atal Ayushman cashless cover specifically, families already covered by the Central Government Health Scheme (CGHS), the Ex-Servicemen Contributory Health Scheme (ECHS), Employees' State Insurance (ESI) or a state government health scheme are not eligible for the state-funded Golden Card, because they draw hospital cover through their own scheme instead.
  • There is no automatic cash pension attached to this scheme, the support is service-based (screening, blood, drugs, treatment cover). Where a patient also has a certified disability, separate disability benefits of the Social Welfare Department must be claimed under those schemes, not this one.

Note that Atal Ayushman enrolment itself is open to every permanent resident family of Uttarakhand regardless of income (BPL or APL), provided the family holds a valid ration card or family register entry, so most patients can obtain the Golden Card free.

What documents do I need?

  • Diagnosis report, the electrophoresis, HPLC or genetic test confirming Thalassemia or Sickle Cell Disease. This is the single most important document.
  • Aadhaar card, for identity and health records.
  • Uttarakhand residence proof — domicile certificate or ration card.
  • Uttarakhand Golden Card; to claim cashless treatment up to Rs 5 lakh a year under Atal Ayushman; it can be made free at Ayushman kiosks and CSCs.

How to apply for care under the Hemoglobinopathy scheme

  1. Get a confirmed diagnosis at a government hospital, medical college or recognised laboratory through hemoglobin electrophoresis, HPLC or genetic testing. Keep the report, it is the single document that opens every part of the scheme.
  2. Register for care at a government district hospital or medical college, which becomes your point for regular transfusions and follow-up. Blood transfusion in government facilities is free under the state Free Blood Initiative.
  3. Enrol for the Uttarakhand Golden Card under Atal Ayushman Uttarakhand Yojana at an Ayushman kiosk, an empanelled hospital or a Common Service Centre, carrying your ration card and Aadhaar, so that hospital admissions are cashless up to Rs 5 lakh a year.
  4. Collect Hydroxyurea and Folic Acid and other prescribed drugs from the government facility as part of routine management, and attend the follow-up schedule the treating doctor sets.
  5. Ask about referral for advanced care such as bone marrow transplant, which is routed to empanelled centres and supported under the national Thalassemia Bal Sewa Yojana.

How do I check my status and what if a claim is refused?

Because this is a service-based scheme rather than a single application with a tracking number, "status" means two different things. For the Golden Card, check enrolment at the same Ayushman kiosk, CSC or empanelled hospital where you applied, or ask the hospital's Ayushman Mitra (help desk officer). For a cashless admission, the empanelled hospital raises the pre-authorisation with the State Health Agency, so the hospital's Ayushman desk is the place to confirm whether a package has been approved.

Common reasons a cashless claim is refused or delayed, and how to fix them:

  • No valid Golden Card; enrol before or at the time of admission; the card is free.
  • Treatment taken at a non-empanelled hospital. The Rs 5 lakh cover applies only at empanelled government and private hospitals; check the empanelment list before a planned admission.
  • Already covered by CGHS, ECHS or ESI; such families are not eligible for the state Golden Card and must use their own scheme.
  • Package not covered: confirm with the hospital's Ayushman desk that the treatment maps to one of the listed packages before proceeding.

How is screening organised?

Uttarakhand carries out hemoglobinopathy screening through its health system and school-linked health activities, in line with the national mission's push to find cases early. Early detection helps in two ways: it lets a diagnosed patient start regular, safe transfusion and management, and it identifies carriers (trait) so that counselling can prevent the birth of severely affected children, the core prevention message of the national hemoglobinopathy guidelines.

Help and where to raise a problem

  • National Health Mission Uttarakhand; nhm.uk.gov.in is the state portal for health-mission services.
  • Your treating government hospital: first point for transfusion, referral and any service issue.
  • Atal Ayushman / Golden Card helpdesk, for enrolment and cashless treatment queries at empanelled hospitals.

Note on completeness: Uttarakhand's hemoglobinopathy support is delivered as a set of linked services rather than a single notified cash scheme, and the state publishes limited standalone detail on it. The amounts quoted here (free blood in government facilities, Rs 5 lakh Atal Ayushman cover, Rs 10 lakh national transplant support) come from the linked schemes; patients should confirm current empanelment and eligibility at their treating hospital.
