Ayushman Bharat PM-JAY: Why Applications Get Rejected and How to Fix It

Why does an Ayushman card application get rejected?
Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY) is entitlement-based, not an open enrolment, so most "rejections" fall into two buckets: your household is not in the identified database, or your e-KYC failed. A card can also sit stuck at "under process" for days, which people mistake for rejection when it is only pending approval. This article separates each cause and gives the fix. Household not found in the database
If a search on beneficiary.nha.gov.in returns no match, your household is not in the SECC 2011 identified list or your state's approved additional database. This is the hardest case because there is no route to add a household on request under the national criteria.
- Fix: Check whether your state has extended PM-JAY to ration-card-linked or state-scheme families beyond the SECC list, and search under that state scheme. Raise inclusion disputes with your State Health Agency, which owns the database, not the hospital.
- Being under 70 with a currently low income does not make you eligible if your household was ranked out of the SECC deprivation criteria.
e-KYC not completed or photo mismatch
The most common technical rejection. The card is only issued after e-KYC succeeds and the live photograph matches Aadhaar.
- Live photo failing to match: retake the photo in good light, facing the camera squarely.
- Fingerprint failing: switch to face authentication or Aadhaar OTP in the Ayushman App — face auth is the route when your mobile is not linked to Aadhaar.
- Mobile not linked to Aadhaar: complete assisted e-KYC at an Ayushman Mitra desk or a Common Service Centre, where biometric modes are available.
Name or date-of-birth mismatch
If the name, gender or date of birth in Aadhaar does not match the beneficiary record, verification fails. Use the update option in the Ayushman App to correct name, gender or date of birth against the record, then reattempt e-KYC.
Card stuck at "under process"
This is the single most common cause of a card that never appears, and it is not a rejection. After you submit, the request waits for the state-level (L2) approver. Approval is usually completed within a few days.
- Fix: Wait a few working days; if it persists, call the helpline with your reference number so it can be escalated to the state approver.
Duplicate application
If your household already holds a card under a different family identifier, a fresh application is blocked as a duplicate. Search for the existing card by Aadhaar, ration card, family ID or name before reapplying, and download the card already issued rather than creating a second record.
State has not implemented PM-JAY
Residents of a state or union territory that has not implemented PM-JAY cannot obtain a card there. West Bengal, for example, has not implemented the scheme. There is no workaround within that state, though a cardholder from an implementing state retains portability at empanelled hospitals elsewhere.
How to raise a grievance
- Call the national PM-JAY helpline on 14555 or 1800-111-565, operated 24x7.
- Use the Centralised Grievance Redressal Management System on the National Health Authority website.
- For database inclusion disputes, approach your State Health Agency.
- At a hospital, use the Pradhan Mantri Arogya Mitra desk.
PM-JAY is free at every stage. No hospital, agent or intermediary can charge you to make an Ayushman card, and if a hospital demands payment for a covered procedure, report it on 14555 immediately.
