Niramaya Health Insurance Scheme for Persons with Disabilities (Madhya Pradesh)
Madhya Pradesh state government scheme
Niramaya Health Insurance Scheme gives persons with disabilities in Madhya Pradesh health cover of up to Rs 1,00,000 a year with a single flat premium for every age and every covered disability. Enrolment costs Rs 250 for BPL families and Rs 500 for others, and is free where a legal guardian other than a parent has been appointed.
What is the Niramaya Health Insurance Scheme in Madhya Pradesh?
Niramaya Health Insurance Scheme is a health cover for persons with disabilities, run by the National Trust under the Ministry of Social Justice and Empowerment and delivered in Madhya Pradesh through the Social Justice and Empowerment of Persons with Disabilities Department and its SPARSH portal (sparsh.mp.gov.in).
According to the Madhya Pradesh SPARSH portal, Niramaya provides insurance coverage of up to Rs 1.0 lakh per beneficiary per year, with only one premium payable across all ages and the same coverage for all types of disabilities covered under the National Trust Act, 1999.
That single design decision; one flat premium, no age loading, no medical test: is what makes Niramaya different from ordinary health insurance. A family raising a child with cerebral palsy cannot normally buy affordable health cover for that child on the open market at any price. Niramaya prices it at Rs 250 or Rs 500 a year.
What does Niramaya cover?
- Up to Rs 1,00,000 a year of health expenditure per enrolled person with disability.
- Hospitalisation, including surgery, for the covered beneficiary.
- Outpatient medical treatment, medicines and consultations.
- Therapies; physiotherapy, occupational therapy, speech therapy and behaviour therapy, which are the recurring costs that matter most to a family raising a person with autism, cerebral palsy or intellectual disability.
- Corrective surgeries for existing disability conditions.
- Pre-existing conditions from the first day. There is no waiting period and no exclusion for the disability the person already has.
- Transportation costs to the treating facility, within the limits the National Trust prescribes.
The Rs 1,00,000 is a single annual ceiling covering all of the above together, with internal sub-limits set by the National Trust for each head. Families should confirm the current sub-limits with the registered organisation at the time of enrolment, since the National Trust revises them from time to time.
Who is eligible for Niramaya in Madhya Pradesh?
A person can be enrolled if:
- The person has at least one disability defined in the National Trust Act, 1999, that is autism, cerebral palsy, intellectual disability or multiple disabilities. The SPARSH portal states this test directly.
- The person holds a valid disability certificate issued by the District Medical Board or district hospital.
- The person is a resident of Madhya Pradesh, verified through an address proof.
- The person's name is registered and verified on the Madhya Pradesh SPARSH portal and the Samagra portal.
- The annual enrolment fee is paid, unless the person is exempt.
There is no age limit. A newborn diagnosed with cerebral palsy and a 45-year-old with intellectual disability pay exactly the same premium.
Who cannot apply or is not eligible:
- Persons with disabilities outside the National Trust Act's four categories cannot apply. A person with only a locomotor disability, blindness, deafness or leprosy-cured status is not covered by Niramaya, though other Madhya Pradesh disability schemes on the SPARSH portal — pension, aids and appliances, corrective surgery, education assistance, remain open to them.
- A person without a disability certificate is not eligible. The certificate from the District Medical Board is the gateway document and cannot be substituted by a doctor's letter.
- Residents of other states are not eligible through the Madhya Pradesh route; they enrol through a registered organisation in their own state. The National Trust operates Niramaya nationwide except Jammu and Kashmir.
- A person already enrolled for the current year is not eligible to enrol again. One enrolment per beneficiary per year.
What does enrolment cost?
| Category | Annual enrolment fee |
|---|---|
| BPL family | Rs 250 |
| Non-BPL family | Rs 500 |
| Person with a legal guardian other than a parent | Free |
The SPARSH portal lists these three slabs. The exemption for a person with a legal guardian appointed under the National Trust Act recognises that such a person usually has no surviving parent to pay for them.

How to apply for Niramaya in Madhya Pradesh
- Get the disability certificate first. Apply at your district hospital's medical board through the SPARSH portal or the district Deputy Director, Panchayat and Social Justice office. Without this certificate nothing else proceeds.
- Register the person on the SPARSH portal at sparsh.mp.gov.in and make sure the name also appears correctly in the family's Samagra ID. Madhya Pradesh verifies every disability benefit against both databases, and a mismatch between them is the commonest cause of delay.
- Find a registered organisation of the National Trust working in your district. Niramaya applications are submitted through such organisations; parents' associations, special schools and disability NGOs registered with the Trust: not by the family directly at a government counter. The district Deputy Director, Panchayat and Social Justice, can tell you which organisations are registered locally.
- Fill the Niramaya enrolment form through that organisation, attach the documents, and pay the applicable fee of Rs 250, Rs 500 or nil.
- Wait for approval. According to the SPARSH portal, the National Trust reviews the submission within 15 days and issues the health identification card within 30 days of approval.
- Collect the health card and keep it with the disability certificate. Treatment claims are made against the card number.
- Renew every year. Niramaya cover is annual. A lapsed enrolment leaves the family uninsured until it is renewed.
How to claim treatment under Niramaya
- Take the person for treatment and keep every original bill, prescription and discharge summary.
- Submit the claim through the same registered organisation that enrolled the beneficiary, with the health card number and the original documents.
- The insurer settles reimbursable claims against the Rs 1,00,000 annual ceiling and the applicable sub-limit for that head of expense.
- Track the claim through the registered organisation; escalate to the district Deputy Director, Panchayat and Social Justice, if there is no movement.
Why Niramaya applications get rejected
- The disability is not one of the four National Trust Act categories. This is the single largest reason. Niramaya is narrower than the disability certificate itself.
- The disability certificate has expired, is temporary, or does not state the disability percentage.
- The name is not verified on the SPARSH and Samagra portals, or the two records spell the name differently.
- BPL status claimed but not proved, so the Rs 250 fee is short-paid.
- Guardianship exemption claimed without a guardianship certificate issued under the National Trust Act.
- The enrolment was not renewed for the current year.
Other Madhya Pradesh disability schemes on the same portal
The SPARSH portal, run by the Social Justice and Empowerment of Persons with Disabilities Department, carries several benefits a family should check at the same time: disability pension, aids and appliances, corrective surgery sanctioned by the District Collector, education assistance, and the marriage promotion assistance for persons with disabilities. Registering once on SPARSH makes all of them accessible from the same verified record.
Help and grievance redressal
- SPARSH portal: sparsh.mp.gov.in, for registration, scheme status and the disability certificate application.
- Deputy Director / Joint Director, Panchayat and Social Justice, at your district headquarters. The officer who handles disability schemes locally.
- CM Helpline: 181, or cmhelpline.mp.gov.in, for a tracked written complaint.
- Social Justice Department portal: socialjustice.mp.gov.in.
No registered organisation may charge a family more than the notified Niramaya enrolment fee of Rs 250 or Rs 500. Any additional demand should be reported to the district Deputy Director and on the CM Helpline.

Frequently asked questions
How much health cover does Niramaya give in Madhya Pradesh?
Up to Rs 1,00,000 a year per beneficiary. The cover is the same for every age and for every disability covered by the National Trust Act 1999, which is unusual — most health insurance charges more as age rises, and Niramaya does not.
What does the enrolment cost?
Rs 250 a year for a BPL family, Rs 500 a year for a non-BPL family, and free where the person with disability has a legal guardian other than a parent appointed under the National Trust Act. There is no medical test and no age loading.
Which disabilities are covered under Niramaya?
The four disabilities defined in the National Trust Act 1999 — autism, cerebral palsy, intellectual disability (mental retardation in the Act's wording) and multiple disabilities. A person with at least one of these, holding a valid disability certificate, is eligible.
Are pre-existing conditions covered?
Yes. Niramaya was designed specifically to cover pre-existing conditions and requires no medical test before enrolment, because every beneficiary by definition already has a disability.
How does a family in Madhya Pradesh apply?
Through a registered organisation of the National Trust, with the beneficiary's name verified on the Madhya Pradesh SPARSH portal and the Samagra portal. Applications are not filed directly by the family at a government counter; the registered organisation submits them.
How long does it take to get the health card?
The National Trust reviews an application within 15 days and issues the health identification card within 30 days of approval, according to the Madhya Pradesh SPARSH portal.
Is Niramaya the same as the construction workers' medical aid scheme?
No, though both are sometimes written as "Ayushman Bharat Niramaya" in Madhya Pradesh listings. This scheme is for persons with disabilities under the Social Justice and Empowerment of Persons with Disabilities Department and pays up to Rs 1,00,000. The construction workers' medical benefit sits with the Labour Department's welfare board and links to Ayushman Bharat Niramayam cover of up to Rs 5,00,000.
Does Niramaya cover therapy and outpatient costs, not just admission?
Yes, it is designed as a package rather than a pure hospitalisation cover. The Rs 1,00,000 limit is applied across hospitalisation, outpatient medical care and therapies such as physiotherapy and occupational therapy, with sub-limits fixed by the National Trust.
