Ayushman Bharat Digital Mission: Why ABHA Creation Fails and How to Fix It

Why does ABHA creation fail?

Ayushman Bharat Digital Mission (ABDM) does not "reject" applications the way a benefit scheme does, because creating an Ayushman Bharat Health Account (ABHA) is a free, self-service registration open to every resident. What people call a rejection is almost always a failed verification step at abha.abdm.gov.in, an OTP that never arrives, an identity that will not authenticate, or a duplicate account blocking a new one. This article lists each failure point under ABDM and the fix, so you are not left staring at an error screen. Mobile number not linked to Aadhaar

This is the single most common reason the Aadhaar route stalls. The ABHA portal sends the identity OTP to the mobile number registered against your Aadhaar at UIDAI. If your Aadhaar has no linked mobile, no OTP arrives and creation cannot complete.

  • Fix 1: Choose the "alternative mobile number" option when prompted, which sends a second OTP to a number you can actually receive on.
  • Fix 2: Use assisted creation at a government hospital, health and wellness centre or ABDM-participating facility, where staff complete biometric authentication instead of OTP.

Identity verification fails

The Aadhaar or driving-licence check can fail even when the number is correct.

  • Aadhaar authentication down: UIDAI services are occasionally unavailable; wait and retry rather than re-entering repeatedly.
  • Driving-licence route provisional: the driving-licence option creates an account in a provisional state until an additional verification step is completed, so it is not a failure: finish the verification to fully activate the ABHA.
  • Consent box not ticked: the National Health Authority cannot verify identity unless you tick the consent declaration; an unticked box blocks the whole flow.

"ABHA address already taken"

The ABHA address (the yourname@abdm handle) is unique across the country. If your preferred handle is taken, the portal will not create it. Fix it by adding a numeral or choosing a different handle — this is not a rejection of your identity, only of that one string.

Duplicate account blocking a new one

ABDM does not permit two ABHAs for one person. If you created an account earlier and forgot, the portal surfaces the existing account instead of making a second one. Use the "Login with Aadhaar" option to recover the old ABHA rather than trying to force a new registration.

Records will not link to your ABHA

A created ABHA is separate from linking old hospital records to it. Records do not auto-match and are never pulled automatically.

  • Name mismatch: if the name in Aadhaar differs from your hospital file, records will not auto-match; link them manually through the facility search in the ABHA app or phr.abdm.gov.in.
  • Facility not on ABDM: only ABDM-linked hospitals and labs appear in the search; a facility that has not joined cannot share records digitally.

How to raise a grievance

If a genuine technical fault persists after these fixes, use the official ABDM channels rather than any third party:

  1. Call the ABDM helpline on 1800-11-4477 or the short code 14477.
  2. Use the "Raise a query" section on the ABDM portal at abdm.gov.in.
  3. Escalate to the National Health Authority through nha.gov.in.

Remember that ABHA creation is always free. No agent, cyber cafe or hospital staff member should charge you, and no genuine ABDM process asks you to share your OTP with another person. A request to do so is a fraud attempt, not a verification step.
