Ex-Servicemen Contributory Health Scheme: Why Applications Get Rejected and How to Fix It
Most ECHS applications stall on eligibility and document problems, no pension from Defence Estimates, a dependent over the income limit, a missing MRO, or an unresolved Fixed Medical Allowance, rather than on the medical side. The Ex-Servicemen Contributory Health Scheme (ECHS) gives pensioner ex-servicemen and their dependents lifelong cashless, capless medical cover through a 64 KB smart card obtained online. This guide covers each rejection cause, drawn from the official ECHS FAQ, and its fix.
Reason 1: You do not draw a pension from Defence Estimates
ECHS is for ex-servicemen who draw pension from Defence Estimates and their eligible dependents. Ex-servicemen who took a lump sum in lieu of pension and receive no defence pension are not eligible.
Fix: There is no workaround. A defence pension (including disability or family pension) is the core qualifying condition. Confirm your PPO reflects a defence pension before applying.
Reason 2: You are still drawing Fixed Medical Allowance
FMA and ECHS membership cannot be held at the same time. An application from someone still drawing FMA is rejected.
Fix: Stop drawing Fixed Medical Allowance first, then apply. Note the reverse case too; if there is no ECHS polyclinic, Armed Forces hospital or upgraded MI Room in your district, you may instead claim FMA of Rs 1,000 per month (effective 1 November 2018), but not both.
Reason 3: A dependent exceeds the income limit
For parents, sisters, minor/disabled brothers and certain daughters, income from all sources must be below Rs 9,000 a month excluding DA. A dependent over that ceiling is removed from the card.
Fix: Submit accurate dependency and income proof. If a parent's or sibling's income is above the limit, they cannot be added; do not attempt to under-declare, as annual validation will catch it.
Reason 4: A dependent no longer qualifies
Sons are covered only while unemployed, unmarried and under 25; a son who starts earning, marries or turns 25 falls out. Daughters remain covered while unmarried, divorced, abandoned, separated or widowed. Disabled children are covered for life under the PwD Act, 2016.
Fix: Add or retain only dependents who currently meet the category test, and complete annual validation — dependent beneficiaries must validate each year to keep their names active on the card.
Reason 5: Missing or wrong MRO for the contribution
The Military Receivable Order (MRO) proving payment of the one-time contribution must be uploaded with the online application. A missing, illegible or wrong-slab MRO stalls the application.
Fix: Pay the contribution for your rank, Rs 30,000 (up to Havildar), Rs 67,000 (Naib Subedar to Subedar Major), or Rs 1,20,000 (officers), effective 29 December 2019, and upload a clear MRO. War widows, pre-1996 retirees and battle casualties are exempt and skip this step, but must produce the relevant casualty or pension documents.
Reason 6: You are not an eligible category
The official ECHS FAQ states that serving personnel (covered by Armed Forces Medical Services), ex-cadets and cadets boarded out on medical grounds are not eligible, because the last two do not qualify for ex-serviceman status.
Fix: This is a status bar with no workaround. Confirm your ex-serviceman status before applying.
Reason 7: Documents incomplete at biometric enrolment
Enrolment cannot be completed without in-person biometric and photograph capture at the nominated polyclinic, with Aadhaar of the member and every dependent.
Fix: Attend the polyclinic with all listed dependents and their Aadhaar cards. Until the card arrives, the Temporary Slip issued at the polyclinic (valid three months) allows treatment.
How to correct or contest a rejection
- Check the rejection reason against the application reference number you noted at submission.
- Fix the specific defect; pension proof, MRO, dependency/income proof, or annual validation.

There is no age bar and no pre-existing-disease bar for membership, and no time limit on applying after retirement, so most rejections trace back to pension status, income limits or paperwork, all of which are fixable except the hard status bars above. No agent is needed; the only valid payments are the rank-based contribution and the published smart card printing charges.
