ECHS Card Status: How to Track Your Application and Smart Card

Track an ECHS application by logging back into the online card application portal on echs.gov.in with the application reference number you noted at submission, and, once you have completed biometric enrolment, by asking your nominated polyclinic whether the printed 64 KB smart card has arrived. The two halves of ECHS enrolment, online submission and in-person biometrics, are tracked in different places, which is why applicants often think their file has vanished when it has simply moved on. For the full application walkthrough, contribution slabs and eligibility, see the Ex-Servicemen Contributory Health Scheme (ECHS) guide.

What you need before checking

  • The application reference number generated when you submitted the online form.
  • Your service number, PPO number and date of birth, the same three identifiers used at registration.
  • The MRO for the contribution, if a query is raised about payment.

The stages your application passes through

Knowing which stage you are at tells you where to look and whom to ask.

  1. Online application submitted. You have a reference number. The record now sits with ECHS for scrutiny of the uploaded PPO, discharge documents, MRO, Aadhaar and dependency proofs.
  2. Documents verified. Any deficiency, a missing MRO, an unreadable dependency proof, an Aadhaar that does not match a listed dependent, is raised at this stage. Track this through your portal login and the email or mobile number you registered.
  3. Biometric enrolment pending. The application is cleared to the polyclinic stage, and you must attend in person with every dependent. Until this is done, nothing further happens, no card is printed for an applicant who has not enrolled biometrically.
  4. Temporary Slip issued. At the polyclinic you receive a Temporary Slip valid for three months or until the smart card arrives. Holding this slip is itself confirmation that your enrolment is through and the card is in the printing pipeline.
  5. Card printed and available. The 64 KB smart card is delivered to your nominated polyclinic for collection.
  6. Card activated. You must activate the card at the polyclinic counter. A card that has been collected but not activated will not work at empanelled hospitals, and this is a distinct step from collection, not an automatic one.

Where to check at each stage

Online stages (1 to 2): log in to the ECHS online card application portal reached from echs.gov.in using the credentials you created at registration, and open your application by reference number.

Polyclinic stages (3 to 6): your nominated ECHS polyclinic is the authoritative source on biometric enrolment, Temporary Slip and card arrival. Polyclinics work 0800 to 1600 hours, Monday to Saturday, excluding gazetted holidays. Call or visit within those hours rather than expecting an online notification for card delivery.

What each situation means, and what to do

  1. No reference number after submitting. The submission did not complete. Log back in and check whether a draft application exists before starting a fresh one; duplicate applications for the same service number complicate verification.
  2. Reference number exists, no movement for weeks. Most often a document deficiency. Re-open the application, confirm the MRO, PPO, discharge documents, Aadhaar of every listed person and the dependency and income proofs are all uploaded and legible, then follow up on the smart card email address below.
  3. Application shows verified, but you have not been called for biometrics. Do not wait for a call. Go to your nominated polyclinic with all dependents; the in-person capture is the applicant's step to initiate.
  4. Temporary Slip is about to expire and no card has come. The slip runs three months or until the card arrives. Approach the polyclinic before it lapses, and use the smart card query email so the printing status can be traced against your reference number.
  5. A dependent is missing from the issued card. Usually the dependency or income proof was not accepted, the Rs 9,000 a month excluding DA income limit applies to parents, sisters, brothers and daughters, or the dependent did not attend biometric enrolment. Both are fixable, but both require going back to the polyclinic with the dependent present.
  6. Card collected but not working at a hospital. Check first that it has been activated at the polyclinic counter, and that it is a 64 KB card; 16 KB and 32 KB cards are no longer valid and must be replaced at Rs 129.38.
  7. You have moved. Dependency can be transferred from one polyclinic to another once in three months, done online through the ECHS application. Do this before chasing card status at your old polyclinic.

Keeping an issued card active

An issued card is not permanently self-maintaining. Dependent beneficiaries must complete annual validation to keep their names active on the card. If a dependent's treatment is suddenly refused despite a valid card, an overdue annual validation is the first thing to check.

Who to contact about status

Use only the official ECHS channels; no agent is needed at any stage, and the only legitimate payments are the rank-based contribution and the published printing charges.

FAQ

Can I get treatment while my card is still being printed? Yes. The Temporary Slip issued at the polyclinic after biometric enrolment is valid for three months or until the card arrives, and covers treatment in the meantime.

Does ECHS notify me by SMS when the card is ready? Cards are delivered to the nominated polyclinic for collection, so treat the polyclinic, not an SMS, as the source of truth on card arrival.

I was admitted in an emergency while my status was pending, what do I do? Inform your parent or nearest ECHS polyclinic within 48 hours of admission. For a non-empanelled hospital, original bills go to the parent polyclinic for reimbursement, and the onus of proving a genuine emergency rests with the patient.
