National Health Mission vs Ayushman Bharat PM-JAY: Which Should You Use?

Use the National Health Mission (NHM) for everyday public health care: free drugs, free diagnostics, immunisation, antenatal and delivery care, the 108 ambulance, at any government facility, with no card and no enrolment; use Ayushman Bharat PM-JAY when you need hospitalisation and want the bill covered at an empanelled public or private hospital. They are not rivals, the NHM guide itself names PM-JAY as the separate, complementary scheme. This article compares the two so you know which door to knock on, and confirms you can rely on both. The one-line reminder: NHM pays no cash to you and does not reimburse private treatment; it funds the government health system you walk into.

NHM: the system you access directly

NHM is the money and management behind the facility nearest you. There is no application form and no beneficiary list — you access it by going to a sub-centre, Ayushman Arogya Mandir, primary or community health centre, or district hospital. It funds the Free Drugs Service Initiative, the Free Diagnostics Service Initiative, JSSK free delivery care, the Universal Immunization Programme, and the 108/102 ambulance services. Your ASHA is the correct first contact if you do not know where to start. NHM's strength is breadth of routine care for everyone; its limit is that it does not pay for treatment at a private hospital.

PM-JAY: the cover for a hospital bill

PM-JAY is a separate centrally sponsored scheme run by the National Health Authority. It pays for hospitalisation at empanelled hospitals, including private ones, cashless to the eligible beneficiary. Where NHM strengthens the public system that delivers care, PM-JAY pays the hospital bill for eligible families, which is exactly the gap NHM does not fill. If your problem is affording a surgery or an inpatient admission, PM-JAY (if you are eligible) is the instrument.

Which should you use?

  • Routine and preventive care: antenatal check-ups, immunisation, free medicines, notified lab tests, a normal or JSSK-covered delivery, an emergency ambulance: use NHM at a government facility. No card needed.
  • A planned or emergency hospital admission, especially where you may go to a private empanelled hospital and cannot pay: use your PM-JAY entitlement if your family is eligible.
  • You want a cash payment for yourself: neither is a personal cash scheme. NHM's JSY pays a small institutional-delivery amount only to eligible categories; PM-JAY pays the hospital, not you.

Can you rely on both?

Yes. The two are designed to work alongside each other. NHM keeps the public facility staffed, stocked and reachable; PM-JAY covers the hospitalisation bill for eligible families at empanelled hospitals. A pregnant woman, for instance, can receive free antenatal care and a free JSSK delivery through NHM at a government facility, and a PM-JAY-eligible family can additionally fall back on PM-JAY cover for a hospitalisation that needs empanelled inpatient care. There is no need to "choose" one permanently, you use whichever fits the care you need at the time.

Which comes first?

There is no sequencing to worry about, because NHM needs no enrolment. Practically:

  1. For anything routine, go to your government facility / ASHA, that is NHM, and it is immediate.
  2. If a hospital admission is looming and your family is PM-JAY eligible, get your Ayushman card sorted in advance so the admission is cashless.
  3. Keep both in mind: they cover different stages of care, not the same one.
