National Tuberculosis Elimination Programme: Why Nutrition Payments Get Rejected and How to Fix It

Under the National Tuberculosis Elimination Programme (NTEP), nobody with TB is refused free diagnosis or free drugs, those are not "applications" and cannot be rejected for want of documents. What does fail is the Rs 1,000-a-month Ni-kshay Poshan Yojana payment, and it fails for a small set of well-defined reasons: a bank account not Aadhaar-seeded, a name mismatch, a follow-up examination not recorded, bank details never entered, or a minor's account. Each has a fix, and Ni-kshay displays the failure reason to the health worker so it can be re-triggered.

The National Tuberculosis Elimination Programme guide explains that treatment is free and that only the nutrition cash needs a validated bank account. This article is only about why that cash gets stuck and how to release it.

First: is this a rejection at all?

Two things are often confused. Free treatment is an entitlement for anyone diagnosed with TB, it is never rejected. The cash benefit is a DBT payment that can fail validation. If you were refused a TB test or drugs at a government facility, that is a grievance to escalate (call 1800-11-6666), not a rule. Everything below is about the payment.

Reason 1 to Bank account not Aadhaar-seeded

Ni-kshay Poshan Yojana is paid only into an Aadhaar-seeded bank account. If the account is not linked to Aadhaar, the transfer fails validation before it ever reaches the Public Financial Management System (PFMS). Fix: visit your bank branch, complete Aadhaar seeding, and ask your treatment supporter to re-enter or re-validate the account in Ni-kshay so the payment can be re-triggered.

Reason 2 to Name mismatch across records

A mismatch between the name in your Ni-kshay record, your Aadhaar and your bank account causes the validation to reject the payment. Even small differences, an initial, a spelling, a maiden name: can break it. Fix: align the three. Usually the quickest correction is to update the Ni-kshay record to match the bank/Aadhaar name exactly; ask the district TB office which field is causing the mismatch, since the system shows the reason.

Reason 3 to Follow-up examination not recorded

Subsequent instalments are released against documented follow-up. If your latest follow-up sputum examination has not been entered in Ni-kshay, the next payment simply does not generate — it looks like a stoppage but is really a missing record. Fix: confirm with your treatment supporter that each follow-up examination is entered on schedule. Attend your follow-up tests, and check that they are logged.

Reason 4 to Case notified but bank details never entered

A case can be notified (so treatment and drugs flow) while the bank details were never captured by field staff. No bank details means no DBT, the drugs arrive but the money never starts. Fix: give your bank account details and Aadhaar number to your Senior Treatment Supervisor, treatment supporter or health worker, and confirm they have been entered in your Ni-kshay record.

Reason 5, The account belongs to a minor

For a child patient, the payment cannot go into the child's own account. Fix: provide the parent's or guardian's Aadhaar-seeded bank account, which is the account Ni-kshay Poshan Yojana uses for minors.

How the payment recovers after a fix

The NTEP knowledge base notes that when validation or payment fails, the reason is displayed to the users, and on rectification of the errors the payment process can be re-triggered. So a failed instalment is not lost; once the underlying error is corrected, the district staff can push it through the two-level approval and on to PFMS again.

How to escalate

  1. Ask your treatment supporter or District TB Officer to open your Ni-kshay record and read out the displayed failure reason.
  2. Correct the specific cause; seeding, name, follow-up, missing details, or minor's account.
  3. Ask for the payment to be re-triggered after the fix.
  4. If it still does not move, call the Ni-kshay Sampark TB helpline on 1800-11-6666, toll-free, with your Ni-kshay ID.

What NTEP will not do

No health worker, chemist or agent can charge you for NTEP drugs or for getting your name on Ni-kshay. The entire programme is free. Anyone demanding a fee to "release" your nutrition payment is not part of the process; the money moves only through PFMS after validation, and correcting the recorded error is the only legitimate route.
