Birth Certificate in Uttarakhand (Janm Praman Patra)
Uttarakhand state government scheme
Birth Certificate in Uttarakhand is issued by the Registrar of Births and Deaths of the local body where the birth took place. The Nagar Nigam or Nagar Palika Parishad in towns and the Gram Panchayat in villages, and the state's online route is the Apuni Sarkar e-Services portal at eservices.uk.gov.in. Every birth must be reported within 21 days under the Registration of Births and Deaths Act 1969.
What is a birth certificate in Uttarakhand?
A birth certificate in Uttarakhand is the certified extract of the birth register kept by the Registrar of Births and Deaths of the local body in whose area the child was born, issued under the Registration of Births and Deaths Act 1969. Uttarakhand calls it janm praman patra, and it is the single document that proves date of birth and place of birth for the rest of a person's dealings with the state.
Uttarakhand administers the register through its local bodies, not through the Revenue Department. The Haridwar district administration names the issuing office as Nagar Nigam and Nagar Palika Parishad and directs applicants to https://eservices.uk.gov.in/. The Apuni Sarkar e-Services portal built by the Information Technology Development Agency (ITDA), Government of Uttarakhand. Older district pages, such as Almora, still list the tehsil office as the counter and the earlier edistrict.uk.gov.in address, which is a genuine source of confusion in the state: the platform was renamed and merged into Apuni Sarkar, and different districts updated their pages at different times. If a district page sends you to edistrict.uk.gov.in and it does not respond, use eservices.uk.gov.in instead.
Uttarakhand has four registrar tiers, and which one holds your record depends only on where the birth happened:
- Nagar Nigams; Dehradun, Haridwar, Haldwani, Rudrapur, Kashipur, Roorkee, Rishikesh, Kotdwar, Pithoragarh and Srinagar register births through the corporation's birth and death section.
- Nagar Palika Parishads, the municipal boards of the state's mid-sized towns, including hill towns such as Mussoorie, Nainital, Almora, Pauri, Tehri and Champawat.
- Nagar Panchayats, the smallest urban bodies.
- Gram Panchayats. The registrar for births in the state's villages, which is where most rural Garhwal and Kumaon births are recorded.
Since 1 October 2023, the Registration of Births and Deaths (Amendment) Act 2023 has applied across Uttarakhand. It makes the birth certificate the single accepted proof of date and place of birth for admissions, driving licences, passports, Aadhaar, marriage registration and government appointments, brings the Aadhaar numbers of the parents and the informant into the birth reporting information, and supports electronic delivery of the certificate to the informant's registered mobile number and email.
The Apuni Sarkar portal also carries a download-certificates section for citizens and a separate certificate verification page that any office, school or employer can use to confirm a certificate is genuine, which matters in Uttarakhand, where forged date-of-birth documents have long been a problem in recruitment.
Who can and who cannot apply?
You can apply for an Uttarakhand birth certificate if the birth took place inside Uttarakhand and you are a person the Registration of Births and Deaths Act 1969 treats as an informant or interested party:
- The father or mother of the child.
- The head of the household in which a home birth occurred: still common in the interior hill blocks.
- The medical officer in charge of the hospital, CHC, PHC, maternity home or private nursing home where the delivery took place, who reports it directly.
- The jail superintendent for a birth in prison, and the officer in charge for a birth in a hostel, dharamshala or public institution.
- The village headman or officer in charge for an abandoned newborn.
- Any adult born in Uttarakhand who needs a certified copy of their own entry, on payment of the per-copy fee.
You cannot apply in Uttarakhand for a birth that took place outside the state. The register is territorial. An Uttarakhandi family whose child was born in Delhi, Noida, Bareilly or Chandigarh must get the certificate from the local body there, and no Uttarakhand Registrar can convert or reissue that entry. Equally, a child born to visiting parents at a Dehradun or Haldwani hospital is registered in Uttarakhand, and the family's home state cannot issue it.
There is a version of this that catches hill families in particular. Because much of the state's institutional delivery load is concentrated in the plains towns and the larger hill hospitals, a family from Pithoragarh, Bageshwar or Uttarkashi whose baby is delivered in Haldwani or Rishikesh must register with that Nagar Nigam. The home Gram Panchayat cannot register a birth that did not occur in its area, however much simpler that would be.
You are also not eligible for a fresh registration where the birth is already registered, the correct request is a certified copy or a correction under Section 15, not a new entry. And an Uttarakhand Registrar cannot register a birth more than one year old without a written order of the Executive Magistrate under Section 13(3).
There is no income limit, no caste condition and no domicile test for a birth certificate in Uttarakhand. Residence matters only in fixing which registrar holds the record.
What documents are required?
Where the hospital has already reported the birth and you only need a copy, the download route needs the registration details and OTP verification. Where the entry has to be created or matched, Uttarakhand Registrars ask for:
- Report of Birth in Form 1, signed by the informant.
- Hospital discharge summary or delivery record showing date, time, sex of the child and the mother's name.
- Aadhaar of both parents and of the informant, now part of the birth reporting information under the 2023 amendment.
- Proof of the family's address at the time of birth, ration card, electricity bill or house tax receipt.
- Marriage certificate or a joint affidavit where the parents' surnames differ or the father's name is being entered later.
- Affidavit explaining the delay, where the birth was not reported within 21 days.
- Executive Magistrate's order under Section 13(3), only where the birth is more than one year old.
How to apply for a birth certificate in Uttarakhand online
- Confirm the birth was reported within 21 days. Ask the hospital for the Form 1 acknowledgement or the registration number, or check with the Nagar Nigam birth and death section or the Gram Panchayat. If nothing was reported, registration comes before any certificate.
- Open eservices.uk.gov.in, the Apuni Sarkar e-Services portal, and register as a citizen with your mobile number. Verify the OTP and set a password.
- Select the birth certificate service and choose the local body of the place of birth; not your home address. Getting this wrong is the single most common cause of a rejected file in Uttarakhand.
- Enter the birth details — child's name if already given, date and time of birth, sex, place of birth with the hospital name, both parents' names and Aadhaar, and the address at the time of birth. Spellings must match the hospital record, because the Registrar compares them against the discharge summary.
- Upload the supporting papers; discharge summary, parents' identity proof and address proof, each within the file size the upload screen states.
- Pay the fee shown on the payment screen and save the acknowledgement or application number. Order all the copies you need in one application, since each extra copy is charged separately.
- Download the digitally signed certificate from the download-certificates section of the portal once the Registrar approves the entry, and keep the PDF, you can reprint it later without reapplying.
How to apply offline at the local body office
- Go to the office that holds the register for the place of birth, the Nagar Nigam birth and death section for a city birth, the Nagar Palika Parishad or Nagar Panchayat office for a town, or the Gram Panchayat for a village birth. Some districts, such as Almora, still route applications through the tehsil office counter.
- Submit the Report of Birth in Form 1 with the hospital discharge record, the parents' Aadhaar and the address proof.
- Pay the counter fee and keep the receipt. Reporting inside 21 days carries no penalty at all; beyond that the Registrar quotes the late fee under the state rules before accepting the file.
- Collect the signed certificate from the same counter, or from a Common Service Centre or Jan Seva Kendra where the office issues digitally.
What if the birth was never registered, or is over 1 year old?
Uttarakhand follows Section 13 of the Registration of Births and Deaths Act 1969 in three stages:
- Within 30 days of the birth. The Registrar registers it on payment of the prescribed late fee.
- After 30 days but within one year; registration needs the written permission of the prescribed authority and an affidavit stating the reason for the delay, plus the late fee.
- After one year: registration is possible only on the written order of an Executive Magistrate or a First Class Magistrate under Section 13(3), supported by hospital, school or panchayat evidence of the date and place of birth.
This third category is large in Uttarakhand, because home deliveries in the interior blocks of Chamoli, Pithoragarh, Bageshwar, Uttarkashi and Rudraprayag went unregistered for years. Adults from these districts applying for a passport or a government post routinely discover there is no entry at all. The practical order is: get written confirmation from the Gram Panchayat or Nagar Palika that no entry exists, gather the school leaving certificate and any hospital or ANM record, then move the magistrate's application. The magistrate stage is the slowest part, so start it well before any deadline.
How do you check the application status?
Log back into eservices.uk.gov.in with the same account and use the application tracking screen with the acknowledgement number from your receipt. Once the certificate is issued, the download-certificates section of the portal releases the digitally signed PDF. The portal's separate certificate verification page lets a school, employer or passport office confirm the certificate against the state's records: useful if an office questions a digitally signed copy.
For a file that has stalled, follow up with the same Nagar Nigam, Nagar Palika Parishad or Gram Panchayat that accepted the Form 1, and escalate to the District Magistrate's grievance cell if the office does not act.
Why are birth certificate applications rejected in Uttarakhand?
- The birth was never reported in Form 1, so there is no register entry to certify.
- The application was filed with the home local body instead of the local body of the place of birth, the commonest hill-district error.
- The child's date of birth, sex or the mother's name does not match the hospital discharge record.
- A home delivery was filed with no affidavit and no panchayat, ANM or ASHA worker's confirmation.
- The birth is over one year old and no Section 13(3) magistrate's order is attached.
- The father's name is being inserted without a marriage certificate or joint affidavit.
- Uploaded scans are unreadable, upside down or attached to the wrong slot.
What does it cost, and what about agents?
Uttarakhand issues the birth certificate at the government fee shown on the Apuni Sarkar payment screen or quoted at the local body counter, plus the notified operator charge if you file through a Common Service Centre. Reporting a birth within 21 days carries no penalty at all. No panchayat secretary, Nagar Nigam clerk, tehsil dealing assistant or middleman is entitled to a rupee beyond the listed fee.
Agents outside hospitals in Dehradun, Haridwar and Haldwani sell a "quick janm praman patra" for several hundred or several thousand rupees. They file the same form on the same portal, and the file still queues before the same Registrar. Keep the receipt the portal generates and report any demand for extra money to the District Magistrate's grievance cell.
What do you need the Uttarakhand birth certificate for?
An Uttarakhand birth certificate is the base document for school and college admission, Aadhaar enrolment for a child, a passport, a learner's licence, PAN, the electoral roll entry at 18, caste, domicile and permanent residence certificates, marriage registration, EWS and scholarship applications, and any government appointment where date of birth must be proved. Because each of those offices retains a copy, most families in the state order three or four certified copies in a single application, and, more usefully in a state where records were long kept on paper, download and store the digitally signed PDF so the certificate survives a lost file at the panchayat.

Frequently asked questions
Who issues a birth certificate in Uttarakhand?
The Registrar of Births and Deaths of the local body where the birth took place issues a birth certificate in Uttarakhand — the Nagar Nigam or Nagar Palika Parishad in urban areas, as the Haridwar district administration states, the Nagar Panchayat in smaller towns, and the Gram Panchayat in villages. Applications are filed through the Apuni Sarkar e-Services portal or, in some districts, at the tehsil office counter.
What is the Uttarakhand portal for a birth certificate?
Uttarakhand's online route is the Apuni Sarkar e-Services portal at eservices.uk.gov.in, the citizen platform built by the Information Technology Development Agency under the Government of Uttarakhand. The Haridwar district administration lists eservices.uk.gov.in for birth certificates, while some district pages such as Almora still point to the older edistrict.uk.gov.in address and to the tehsil office counter.
Within how many days must a birth be registered in Uttarakhand?
A birth must be reported to the Registrar within 21 days of its occurrence in Uttarakhand, as everywhere in India, under the Registration of Births and Deaths Act 1969, and district administration pages in the state repeat that 21-day rule. Reporting inside 21 days carries no penalty; after that the delayed registration route under Section 13 applies.
What if the birth in Uttarakhand is more than 1 year old?
A birth more than one year old can be registered in Uttarakhand only on the written order of an Executive Magistrate or a First Class Magistrate under Section 13(3) of the Registration of Births and Deaths Act 1969, after the applicant proves the birth with hospital, school or panchayat evidence. Between 30 days and one year, the written permission of the prescribed authority plus an affidavit explaining the delay and the late fee is enough.
Where must a birth in Uttarakhand be registered?
A birth in Uttarakhand must be registered with the local body of the place where it occurred, not where the family lives. A hill family whose baby is delivered at a hospital in Haldwani, Rishikesh or Dehradun registers the birth with that city's Nagar Nigam, not with the home Gram Panchayat in Almora, Pauri or Pithoragarh.
Can I download an Uttarakhand birth certificate online?
Yes — the Apuni Sarkar e-Services portal has a download-certificates section at eservices.uk.gov.in from which a digitally signed certificate can be retrieved against the registration details after OTP verification. The same portal carries a certificate verification page that any office can use to confirm the certificate is genuine.
Can I add or correct the child's name on an Uttarakhand birth certificate?
Yes — the child's name can be entered in the birth register after registration by applying to the same Registrar who made the entry, and a correction of a spelling, date or parent's name is made under Section 15 of the Registration of Births and Deaths Act 1969 on documentary proof. The Registrar amends the original register entry and reissues the certificate; a certificate altered anywhere else is not valid.
Does an Uttarakhand birth certificate expire?
A birth certificate in Uttarakhand never expires, because it certifies a permanent entry in the register of the Nagar Nigam, Nagar Palika Parishad or Gram Panchayat. An office asking for a "recent" copy is asking for a fresh certified extract of the same entry, which the Registrar issues at the per-copy fee.
