What is a Cancer Day Care Centre in Uttarakhand?

A Cancer Day Care Centre in Uttarakhand is a chemotherapy and pain-management unit set up inside a district hospital by the Department of Medical Health and Family Welfare, under the National Health Mission. According to the National Health Mission Uttarakhand, these units were created to provide chemotherapy and pain management services to cancer patients close to home.

The problem the units solve is distance. A patient living in Uttarkashi or Pithoragarh on a standard chemotherapy protocol may need a cycle every two or three weeks. Each cycle is a few hours of infusion. Without a district unit, that few hours becomes a two-day journey to Dehradun, Haldwani or Rishikesh, with transport and lodging costs that families often cannot bear repeatedly, which is why treatment gets abandoned halfway. A day care unit lets the patient arrive in the morning, take the cycle under supervision, and be home the same evening.

The units sit within the state's wider non-communicable disease programme, the National Programme for Prevention and Control of Cancer, Diabetes, Cardiovascular Diseases and Stroke, which the National Health Mission notes was launched in 2010 to prevent and control the major non-communicable diseases through infrastructure, trained manpower, early diagnosis and timely referral.

Which districts have Cancer Day Care Units?

The National Health Mission Uttarakhand lists Cancer Day Care Units in 7 districts:

District Region
Almora Kumaon
Champawat Kumaon
Pithoragarh Kumaon
Chamoli Garhwal
Pauri Garhwal
Tehri Garhwal
Uttarkashi Garhwal

Alongside them, District NCD Clinics operate in the District Hospitals of all 13 districts for opportunistic screening for hypertension, diabetes and common cancers. Two other district-level facilities under the same programme are worth knowing about: Cardiac Care Units at District Hospital Rudraprayag and District Hospital Uttarkashi, and the Pradhan Mantri National Dialysis Programme, which provides free dialysis to Below Poverty Line patients and Atal Ayushman Uttarakhand cardholders.

Medical officers and staff nurses who run the cancer units are trained at AIIMS, Rishikesh, according to the National Health Mission.

Who can use the Cancer Day Care Centre?

You can be treated here if:

  • You are a diagnosed cancer patient with a histopathology or biopsy report.
  • An oncologist has prescribed a chemotherapy protocol that can be given as a day-care infusion.
  • You are medically fit for the cycle on the day; blood counts and general condition are checked before every infusion.
  • You are living in or able to reach one of the 7 districts with a unit. There is no residence restriction that turns away a patient from a neighbouring district.

You cannot be treated here: or are not eligible: if:

  • You have not been diagnosed. Someone with symptoms is screened at the District NCD Clinic and referred for diagnosis; the day care unit does not diagnose.
  • Your treatment plan needs surgery or radiotherapy. Neither is available at a day care unit.
  • Your protocol needs inpatient admission, intensive monitoring or complex high-risk regimens, which are referred to a medical college or tertiary centre.
  • Your blood counts are too low on the day. The cycle is deferred, not refused, and you are called back after recovery.

Screening at the District NCD Clinics is open far more widely: universal screening of everyone above the age of 30 for hypertension, diabetes and common cancers, whether or not the person has symptoms.

How to apply for treatment at a Cancer Day Care Centre

  1. Get screened or diagnosed. If you have symptoms, go to the District NCD Clinic at your District Hospital, or to any government health facility, and ask for cancer screening. Screening is free.
  2. Get a confirmed diagnosis and a protocol. Referral goes to a medical college or a tertiary cancer centre, where biopsy, staging and the chemotherapy protocol are decided.
  3. Carry the protocol to your District Hospital and ask for the Cancer Day Care Unit. Take the biopsy report, the oncologist's prescription, the referral slip and your previous records.
  4. Register at the hospital counter and get an OPD card. The unit schedules your cycles against the protocol.
  5. Come on the scheduled day with your records and previous blood counts. The unit checks counts, gives the infusion, observes you, and sends you home the same day.
  6. Book your next cycle before leaving and keep the card and count reports together, they are asked for at every visit.

What does it cost?

Treatment at a government facility is charged at government rates, and several state and central schemes reduce what a family actually pays:

  • Atal Ayushman Uttarakhand and Ayushman Bharat PM-JAY give cashless treatment up to Rs 5 lakh per family per year at empanelled hospitals, which includes cancer packages.
  • The Free Diagnostics Scheme, launched in Uttarakhand on 12 August 2021, provides 208 pathological tests free of cost at 254 public health institutions — 110 in Kumaon and 144 in Garhwal; at District Hospitals, Sub-District Hospitals, Community Health Centres and Primary Health Centres. Much of the pre-chemotherapy blood work falls in this list.
  • The Free Essential Drug Scheme covers a state essential drug list of 871 drugs, 111 surgical items and 94 sutures distributed across all 13 districts.

Because coverage depends on the card you hold and the exact package, ask the hospital's Ayushman Mitra or the counter clerk to confirm what is covered before the first cycle rather than after it.

Common problems and how to fix them

  • No referral slip. The unit does not know which regimen to give. Get the oncologist to write the protocol, drug names, doses and cycle interval on the referral.
  • Records left at home. Previous counts are needed before every infusion. Keep one folder that never leaves the patient's bag.
  • Drug not in stock at the district unit. Ask the unit in advance which drugs they hold. Some regimens are stocked only at the medical college level.
  • Cycle deferred for low counts. This is normal and not a rejection. Follow the supportive-care advice and return on the new date.
  • Ayushman card not linked. Get the card verified at the hospital's Ayushman desk before treatment starts, not at discharge.

Is there a registration form for this scheme?

No. Cancer Day Care Centre is a service delivery arrangement, not a benefit you register for. There is no application form, no scheme portal, no beneficiary list and no sanction letter. Individuals cannot "apply" for it in the sense of a cash benefit, they simply use the facility. The only thing that determines access is a confirmed diagnosis and a prescribed protocol.

Help and grievance redressal

  • 104 Health Helpline Uttarakhand, toll-free medical advice, 24 hours a day, 7 days a week, run by the National Health Mission.
  • 108 Emergency ambulance: free and available 24x7 across all districts of Uttarakhand.
  • Chief Medical Officer (CMO) of your district for complaints about the district hospital, drug availability or refusal of service.
  • National Health Mission Uttarakhand at nhm.uk.gov.in for programme-level queries.
