Procedure for Making Medical Certificate (for Certifying Disability), Uttarakhand
Uttarakhand state government scheme
The Procedure for Making Medical Certificate in Uttarakhand is how a person with a disability gets a disability certificate from the district medical board headed by the Chief Medical Officer. A person assessed at 40 per cent or more disability qualifies for scheme benefits. Apply free on the national UDID portal, swavlambancard.gov.in, or at the CMO office.
What is the procedure for making a medical certificate for disability in Uttarakhand?
The Procedure for Making Medical Certificate in Uttarakhand is the official route by which a person with a disability is examined by a government medical board and issued a disability certificate stating the type of disability and the percentage of disability assessed. The certificate is issued by the Department of Medical Health and Family Welfare, Government of Uttarakhand, through the Chief Medical Officer (CMO) of each district, and it is recorded nationally as a Unique Disability ID (UDID) card on the portal swavlambancard.gov.in run by the Department of Empowerment of Persons with Disabilities.
The certificate is not itself a benefit. It is the gateway document. Almost every entitlement that Uttarakhand and the Government of India offer to persons with disabilities, the state disability pension, the maintenance grant for children with disabilities, aids and appliances, scholarships, the 4 per cent reservation in government employment, travel concessions, income tax relief, school and college admission relaxations, begins by asking for a valid disability certificate. Without it, a person with a genuine disability cannot enter any of those schemes; with it, all of them become reachable through one document.
According to district administration information published by the Government of Uttarakhand, a person with any disability of more than 40 per cent is treated as qualifying for the facilities, benefits and concessions available under the schemes. That 40 per cent line is the practical threshold most readers are trying to establish when they seek a certificate.
Who assesses
The certifying authority is a medical board at district or state level. As described by the Uttarakhand district administration, the board includes the Chief Medical Officer or a sub-divisional medical officer of the district together with another expert in the specified field, an orthopaedic surgeon for locomotor disability, an ENT specialist for hearing, an ophthalmologist for vision, a psychiatrist for mental illness, and so on. Where a person has multiple disabilities, a board of at least three doctors including a specialist in the relevant type conducts the assessment.
The legal frame
The Rights of Persons with Disabilities Act, 2016 governs certification. The Act specifies 21 disabilities, which is the list the UDID system implements. It also provides for appeal against a medical authority's assessment, so a person who believes the assessed percentage is wrong is not without a remedy.
Who can get a disability certificate in Uttarakhand?
You can get a certificate if:
- You have one or more of the 21 specified disabilities under the RPwD Act, 2016 to including blindness, low vision, deafness, hard of hearing, speech and language disability, locomotor disability, leprosy cured, dwarfism, cerebral palsy, muscular dystrophy, acid attack injury, intellectual disability, specific learning disability, autism spectrum disorder, mental illness, chronic neurological conditions such as multiple sclerosis and Parkinson's disease, and blood disorders such as haemophilia, thalassaemia and sickle cell disease.
- You reside in Uttarakhand and can be examined by the medical board of the district you live in.
- You can attend the board in person on the notified assessment day, or arrange a home or hospital assessment where the district administration allows it for a bedridden applicant.
You are not eligible / cannot get a certificate if:
- Your condition is not among the specified disabilities under the RPwD Act,
- A general illness, an ordinary injury or a temporary condition that does not fall in the scheduled list is not certifiable, however difficult it may be.
- The board assesses the disability below the threshold the individual scheme requires. A certificate may still be issued stating a lower percentage, but it will not open benefits reserved for 40 per cent and above.
- You do not appear for assessment. No certificate can be issued on paperwork alone; physical examination by the board is the whole basis of the document.
- You already hold a valid certificate for the same disability. In that case the correct step is to convert the existing paper certificate into a UDID card, not to apply afresh.
What documents are required?
| Document | Mandatory | Notes |
|---|---|---|
| Identity proof (Aadhaar) | Yes | Identifies the applicant and seeds the UDID record |
| Passport-size photograph | Yes | Printed on the certificate and the UDID card |
| Residence proof | Yes | Fixes the district medical board that will assess you |
| Existing disability certificate | No | Upload if you already hold an older paper certificate |
| Medical records of the condition | No | Specialist notes, audiometry, vision or imaging reports speed up assessment |
| Income certificate | No | Not needed here; needed later for income-linked schemes |
How to apply for a disability medical certificate
- Register on the UDID portal at swavlambancard.gov.in. Create an account with your name and mobile number and verify it with the OTP sent to that number.
- Fill the application form. Enter personal details, address, the type of disability, and the details of any existing certificate. If you have never held a disability certificate, select "No" where the form asks whether you already have one.
- Upload the documents. A recent passport-size photograph, identity proof such as Aadhaar, residence proof, and any medical records or an older certificate you hold.
- Submit the application and note the enrolment number. The application is routed electronically to the CMO office of your district.
- Attend the medical board on the date given. The CMO office schedules the assessment at the district hospital or the notified centre and informs you by SMS or through the portal. Carry the printout of your application, your identity proof and your medical records.
- Get assessed and collect the certificate. The board examines you, records the type and percentage of disability, and the CMO office issues the disability certificate; the UDID card is then generated and can be downloaded from the portal and is also despatched by post.
Offline route: a person who cannot use the portal can go directly to the CMO office or the district hospital with the same documents, fill the paper application there, and have it digitised by the office into the UDID system. In Haridwar district, for example, the office listed for this purpose is the CMO Office, Roshnabad, Haridwar, PIN 249403, telephone 01334-239902. Each district publishes its own CMO office address and number on its NIC district website.
How to check the status of your application
- Log in to swavlambancard.gov.in with the mobile number you registered.
- Open Track Your Application Status and enter your enrolment or application number.
- The screen shows which stage the file is at; submitted, verified by the CMO office, assessment scheduled, assessed by the medical board, certificate generated, or card despatched.
- If the status has not moved for several weeks, take the printout of the application to the CMO office of your district and ask for the board's next sitting date.
Why applications are delayed or rejected
- Blurred or wrong-format uploads โ an unreadable photograph or identity document is the single most common reason a file is sent back at the verification stage.
- Applicant does not attend the board on the scheduled day, so the file stays open until the next sitting.
- Specialist not available for the relevant disability on the board day, so the case is deferred rather than rejected. Ask the CMO office when that specialist sits.
- Condition not in the specified list under the RPwD Act, 2016.
- Duplicate application by a person who already holds a valid certificate.
- Address mismatch, where the applicant applies to a district other than the one they live in.
Fixing these is usually a matter of re-uploading a clear document and attending on the correct day. A person whose assessed percentage seems wrong should not re-apply from scratch; the correct route is the appeal for reassessment provided under the RPwD Act, 2016, filed through the CMO office that issued the certificate.
What the certificate lets you claim afterwards
Once the certificate is in hand, a resident of Uttarakhand can use it to apply for the state's disability pension, the maintenance grant for children with disabilities from birth, aids and appliances assistance, coaching support for divyang students, the incentive for marrying a person with a disability, travel and education concessions, and the reservation available in government recruitment. The UDID card also works across states, so a certificate issued in Uttarakhand is accepted when the holder moves or travels.
Help and grievance route
- Chief Medical Officer's office of your district, for assessment dates, board composition, corrections and reassessment appeals.
- Department of Medical Health and Family Welfare, Uttarakhand, health.uk.gov.in, for escalation above the district.
- Uttarakhand health helpline 104, for directions to the correct district office and hospital.
- UDID portal grievance section at swavlambancard.gov.in, for problems with the online application, card printing or despatch.
- District Social Welfare Office, for what to do with the certificate once issued, that is, which state schemes to apply for next.
Assessment and issue of the disability certificate and UDID card are free. No government official or agent may charge for arranging a certificate, and a demand for money to "get the percentage increased" should be reported to the CMO or the District Magistrate.
Note on verification: the medical board composition, the 40 per cent threshold and the district CMO office details cited here come from Uttarakhand district administration pages and the national UDID sources. Uttarakhand does not publish a single consolidated state page setting out board sitting days, district-wise specialist availability or a dedicated disability-certificate helpline, so those details must be confirmed at your own district's CMO office.

Frequently asked questions
Who issues a disability certificate in Uttarakhand?
A district medical board issues the disability certificate, and the board is headed by the Chief Medical Officer or a sub-divisional medical officer of the district along with a specialist in the relevant field. The board functions under the Department of Medical Health and Family Welfare, Government of Uttarakhand.
What is the minimum disability percentage for benefits?
A person assessed at more than 40 per cent disability qualifies for the facilities, benefits and concessions available under disability schemes. The certificate states the specific percentage, and many individual schemes set their own higher or lower thresholds within that framework.
How do I apply for a disability certificate?
Register on the national UDID portal at swavlambancard.gov.in, fill the application with your personal, disability and address details, upload a photograph and identity proof, and submit. The application reaches the CMO office of your district, which calls you for assessment by the medical board.
Is there any fee for the certificate?
No. Assessment by the government medical board and issue of the disability certificate and UDID card are free. No fee is charged by the CMO office and no agent is authorised to charge for arranging a certificate.
How many disabilities are recognised?
Twenty-one specified disabilities are recognised under the Rights of Persons with Disabilities Act, 2016, and the UDID system currently covers those 21 categories. They include blindness and low vision, deafness and hard of hearing, locomotor disability, intellectual disability, mental illness, autism, cerebral palsy, muscular dystrophy, chronic neurological conditions, blood disorders such as thalassaemia and sickle cell disease, and multiple disabilities.
What if I have more than one disability?
A person with multiple disabilities is assessed by a board of at least three doctors, including at least one specialist in the relevant disability type, so that each condition is examined by a competent authority before a combined percentage is arrived at.
How long is the certificate valid?
A certificate for a permanent, non-progressive condition is issued as permanent for adults, while a certificate for a child is normally issued for a limited period and reassessed as the child grows. A temporary or likely-to-improve condition also carries a review date printed on the certificate.
What can I do if I disagree with the assessed percentage?
You can appeal for reassessment. The Rights of Persons with Disabilities Act, 2016 provides an appeal route against a medical authority's decision, and the appeal is made to the appellate authority notified by the state government, normally through the CMO office that issued the certificate.
