Death Certificate in Gujarat (e-Olakh)
Gujarat state government scheme
Death Certificate in Gujarat is registered and issued by the Registrar of Births and Deaths of the local body where the death occurred, and Gujarat runs its own state portal, e-Olakh at eolakh.gujarat.gov.in, for online death registration and certificate download. Every death must be reported within 21 days under the Registration of Births and Deaths Act 1969, and large city corporations such as Surat also issue copies from their own online modules.
What is a death certificate in Gujarat?
A death certificate in Gujarat is the certified extract of the death register kept by the Registrar of Births and Deaths of the local body in whose area the death occurred. Gujarat issues it under the Registration of Births and Deaths Act 1969, and maintains its own state civil registration portal, e-Olakh at eolakh.gujarat.gov.in, through which citizens apply for and download birth and death certificates.
The certificate is a local-body document, not a Revenue Department one. In Gujarat this matters because the Mamlatdar's Jan Seva Kendra and the Digital Gujarat portal handle income, caste and domicile certificates, while the death register sits with an entirely different office. The Talati-cum-Mantri in a village, the Chief Officer's registrar in a Nagarpalika, or the health branch of a Municipal Corporation in Ahmedabad, Surat, Vadodara, Rajkot, Bhavnagar, Jamnagar, Junagadh or Gandhinagar.
Gujarat's larger city corporations run their own online civic-service modules alongside e-Olakh. Surat Municipal Corporation, for example, accepts a death certificate enrolment directly on suratmunicipal.gov.in and issues the copy from the corporation's own register. Where the death happened inside a corporation area, that corporation's module is usually the fastest route.
Since 1 October 2023 the Registration of Births and Deaths (Amendment) Act 2023 has applied in Gujarat. It makes the Medical Certificate of Cause of Death compulsory from every medical institution, with a copy to the nearest relative, and it opens the national Civil Registration System portal at dc.crsorgi.gov.in for reporting a death and receiving the certificate electronically on the informant's registered email and mobile number.
Who can and who cannot apply?
You can apply for a Gujarat death certificate if the death occurred inside Gujarat and you fall within the class of informants and interested persons the Registration of Births and Deaths Act 1969 recognises:
- The nearest relative; spouse, son, daughter, father, mother, brother or sister of the deceased.
- The head of the household in which the death occurred.
- The medical officer in charge of the hospital or nursing home where the death took place, who reports it directly to the registrar.
- The jail superintendent for a death in prison, and the officer in charge for a death in a hostel, ashram or public institution.
- The village headman or the police officer in charge, for an unclaimed or abandoned body.
- Any person who later needs a certified copy for a bank, insurer, pension office, property or succession matter, on payment of the per-copy fee.
You cannot apply in Gujarat for a death that took place outside the state, because the register is territorial, a Gujarati family whose relative died in Mumbai or Jaipur must obtain the certificate from the local body there, and no Gujarat registrar can re-issue or convert that entry. You are also not eligible to have a second registration made where an entry already exists; in that case the correct request is a certified copy or a correction, not a fresh registration. A registrar will hold the file where the death is under police investigation and the post-mortem cause of death has not been released, and where the applicant can show no relationship to or interest in the deceased.
Gujarat applies no income limit, no caste condition and no domicile test to a death certificate. Residence only decides which registrar holds the record.
What documents are required?
The e-Olakh death application asks for the applicant's mobile number for OTP verification, the deceased's name, gender, date of death, place of death, district and taluka, and an upload of the hospital death certificate or doctor's proof: or, for a death at home, the cremation or burial slip. Registrars in Gujarat normally build the file around:
- Report of Death in Form 2, signed by the informant.
- Medical Certificate of Cause of Death from the hospital or attending doctor.
- Cremation or burial slip, where the death happened at home.
- Identity proof of the deceased, Aadhaar, PAN, election card, ration card or passport.
- Identity proof of the applicant, with proof of relationship where the applicant is not the household head.
- Affidavit stating the date, time and place of death, for a home death with no doctor and for any delayed registration.
- Order under Section 13(3), only where the death is more than one year old.
How to apply for a death certificate in Gujarat online
- Check that the death has been reported. A hospital in Gujarat reports the death to the registrar directly and gives the family the Medical Certificate of Cause of Death. For a death at home, the household head must report it within 21 days.
- Open the e-Olakh portal at eolakh.gujarat.gov.in and select the death registration section, then "Application for Death Certificate".
- Verify yourself with an OTP. Enter the applicant's name and mobile number and confirm the one-time password the portal sends.
- Enter the deceased's details — full name, gender, date of death, place of death, district and taluka. These must match the hospital record exactly, because the registrar compares them with the register entry before approving.
- Upload the proof of death, the hospital death certificate or the doctor's certificate, or the cremation or burial slip for a death at home, together with the identity proof of the deceased.
- Pay the fee shown on the payment screen and save the acknowledgement number. Order every copy you need in the same application; each additional certified copy carries the per-copy fee.
- Download the certificate from the e-Olakh search-and-download option once the registrar has approved and registered the entry.
How to apply offline at the panchayat, nagarpalika or ward office
- Go to the office that holds the register for the place of death, the Gram Panchayat office where the Talati-cum-Mantri keeps the register, the Nagarpalika office, or the ward office of the Municipal Corporation.
- Submit the Report of Death in Form 2 with the Medical Certificate of Cause of Death or the cremation slip, plus identity proof of the deceased and of yourself.
- Pay the counter fee and take the receipt. A report inside the 21-day window carries no penalty; beyond it, the registrar quotes the late fee under the state's rules before accepting the file.
- Collect the certified copies from the same counter, or download them later from e-Olakh once the entry is live in the state database.
How do you check the application status?
Track the application on e-Olakh with the acknowledgement number and mobile number you used at the OTP step, or ask the ward or panchayat office that holds the register. Where the death was reported through a Municipal Corporation module such as Surat's, that corporation's own online services page carries the status and the download link. Where the report went through dc.crsorgi.gov.in, the national portal emails the certificate to the informant's registered address once the registrar approves the entry.
Why are death certificate applications rejected in Gujarat?
- The death was never reported in Form 2, so no register entry exists to certify.
- The application names the wrong district or taluka, jurisdiction follows the place of death, not the family's home town.
- The name, age, gender or father's or husband's name on the form does not match the hospital record or the identity proof of the deceased.
- No hospital certificate and no cremation or burial slip was uploaded for a death at home.
- The death is more than one year old and no order under Section 13(3) was attached.
- The uploaded scan is unreadable, or a document has been attached in the wrong slot.
- The death is under police investigation and the cause of death is not yet finalised.
What does it cost, and what about agents?
Gujarat issues the death certificate at the fee printed on the e-Olakh payment screen or quoted at the counter of the panchayat, nagarpalika or corporation ward office. Reporting a death inside 21 days carries no penalty at all, and additional certified copies are charged at the same published per-copy rate.
No clerk, ward officer, Talati or agent is entitled to anything beyond that. Touts outside crematoria and hospital gates in Ahmedabad and Surat offer "urgent" death certificates for a few thousand rupees; they are filing the same free-to-file application on e-Olakh, and the file still waits for the same registrar's approval. Keep the online payment receipt and report any demand for money to the office of the Municipal Commissioner or the District Development Officer.
What do you need the death certificate for in Gujarat?
A Gujarat death certificate is the first document every downstream office asks for: closing bank and post office accounts, claiming life insurance and EPFO or state pension dues, transferring the gas connection and the electricity meter, mutating agricultural land in the 7/12 and 8-A records and city property in the city survey record, removing the name from the ration card and the electoral roll, and applying for a legal heir certificate from the Mamlatdar or a succession certificate from the civil court. Each of those offices keeps a copy permanently, which is why five or six certified copies ordered together saves months of repeat visits.

Frequently asked questions
Who issues a death certificate in Gujarat?
The Registrar of Births and Deaths of the local body where the death took place issues the death certificate in Gujarat — the Talati-cum-Mantri of the Gram Panchayat in a village, the Chief Officer's registrar in a Nagarpalika, and the health branch of the Municipal Corporation in Ahmedabad, Surat, Vadodara, Rajkot, Bhavnagar, Jamnagar, Junagadh and Gandhinagar. The Revenue Department and the Mamlatdar do not issue it.
What is e-Olakh in Gujarat?
e-Olakh is the Gujarat government's own birth and death registration portal at eolakh.gujarat.gov.in, used to apply for and download birth and death certificates from the state's civil registration database. Its death section takes the applicant's mobile number and OTP, the deceased's name, gender, date and place of death, district and taluka, and the hospital certificate or cremation slip as an upload.
Within how many days must a death be registered in Gujarat?
A death must be reported to the registrar within 21 days of its occurrence in Gujarat, as everywhere in India, under the Registration of Births and Deaths Act 1969. Registration after 21 days is still possible under the delayed registration provisions of Section 13, with a late fee, and a registration more than one year old needs an order under Section 13(3).
What does a death certificate cost in Gujarat?
Gujarat charges only the fee published on the e-Olakh payment screen or at the ward or panchayat counter, and reporting the death inside the 21-day window carries no penalty. Additional certified copies are charged per copy. Nobody at the ward office, the crematorium or a cyber cafe is entitled to a payment beyond the printed fee.
Can I download a Gujarat death certificate online?
Yes, e-Olakh at eolakh.gujarat.gov.in lets you search the state's civil registration database and download a death certificate once the entry has been registered and approved. Surat Municipal Corporation and other city corporations run parallel online modules that issue a copy directly from the corporation's own register.
Is a medical certificate of cause of death compulsory in Gujarat?
Yes, a medical institution in Gujarat must issue a Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, under the Registration of Births and Deaths (Amendment) Act 2023 in force from 1 October 2023. For a death at home with no doctor in attendance, e-Olakh accepts the cremation or burial slip instead.
My relative died in a village — where do I apply in Gujarat?
Apply to the Gram Panchayat of the village where the death occurred, where the Talati-cum-Mantri holds the register, or file the same application on e-Olakh by selecting that district and taluka. Jurisdiction follows the place of death, not the place of residence, so a Gujarat resident who died in an Ahmedabad hospital is registered in Ahmedabad even if the family lives in Kutch.
How many copies of the death certificate should I order in Gujarat?
Order at least five certified copies in one application, because the bank, the insurer, the EPFO or pension office, the electricity and gas connection transfer, and the mutation of land or property each retain a copy permanently. Returning to the registrar for one copy at a time costs the same per-copy fee and adds another trip.
