Chief Minister-gi Hakshelgi Tengbang (CMHT)
Manipur state government scheme
Chief Minister-gi Hakshelgi Tengbang (CMHT) is Manipur's health assurance scheme giving cashless secondary and tertiary hospital treatment worth up to Rs 5,00,000 per family per year at empanelled hospitals in Manipur, Guwahati and Kolkata. Over 4.02 lakh beneficiaries are enrolled. Enrol free at a district hospital kiosk with Aadhaar and an eligibility proof.
What is Chief Minister-gi Hakshelgi Tengbang (CMHT)?
Chief Minister-gi Hakshelgi Tengbang, usually shortened to CMHT and often written locally as Chief Minister-gi Hakshelgi Tenbang, is a health assurance scheme of the Government of Manipur launched by the Chief Minister on 21 January 2018. The CMHT portal describes it as a scheme that "will provide Cashless treatment to the poor at Empanelled Network of Hospitals in Manipur and Guwahati and give health cover worth up to Rs. 5 Lakh per eligible family per year".
CMHT covers secondary and tertiary care on hospitalisation, it is not an outpatient or medicine-reimbursement scheme. A beneficiary shows the CMHT-Manipur Health card at an empanelled hospital and receives treatment without paying the hospital anything.
The scheme is run by a State Nodal Cell (SNC) set up by the Government of Manipur. Day-to-day execution is contracted to an Implementation Support Agency (ISA), Medi Assist Insurance TPA Pvt. Ltd., selected through open tender, which handles enrolment, hospital empanelment, claims processing, health coordinators and the 24x7 call centre.
As per the dashboard published on cmhtmanipur.gov.in as on 30 November 2025, 4,02,439 beneficiaries had been enrolled and 2,74,908 beneficiaries had been treated under CMHT.
Who is eligible for CMHT in Manipur?
You can enrol if you are a bona fide citizen of Manipur and fall in any of these categories:
- AAY (Antyodaya Anna Yojana) ration card holders
- Widows
- Persons with disabilities
- ASHA workers
- Anganwadi workers and Anganwadi helpers
- Ima Keithel (Ima Market) women vendors
- Local media persons and newspaper hawkers
- Internally displaced persons (IDPs)
- Newly added families in the state SECC list, identified through the Deputy Commissioner verification process
- Other poor families approved as eligible by the Government of Manipur
A bona fide citizen of Manipur who claims to be poor and needy but does not fall in any listed category may still apply for eligibility through the DC verification process rather than being turned away.
You are not eligible if:
- You are employed in government service. Every applicant must sign a declaration of non-employment in government service at the enrolment counter before the card is made, so a government employee cannot be enrolled.
- You are not a bona fide citizen of Manipur.
- You cannot produce any eligibility proof and the Deputy Commissioner does not certify your family's economic status.
- You want outpatient consultation or medicine reimbursement only. CMHT benefits apply to hospitalisation and certain day-care procedures, not to ordinary OPD visits.
How much does CMHT cover?
| Benefit | Amount |
|---|---|
| Cashless hospitalisation cover | Up to Rs 5,00,000 per family per year, floater |
| Transport allowance within Manipur | Rs 300 per visit, annual ceiling Rs 3,000 |
| Travel outside Manipur | Cheapest economy airfare for patient plus one attendant, ceiling Rs 30,000 a year |
| Daily allowance for out-of-state treatment | Rs 1,000 a day, ceiling Rs 10,000 |
Two points decide most disputes. First, the cover is a floater โ the Rs 5 lakh is shared by the whole enrolled family in a year, not given per member. Second, transport and daily allowances are inside the Rs 5 lakh ceiling, not on top of it. The scheme documents also note a ceiling of Rs 50,000 for primary and secondary care within the state in the context of the transport allowance provision.
To receive a transport allowance you must give bank account details at the time of hospitalisation, because that amount is reimbursed into the account rather than settled with the hospital.
The annual limit is calculated from the date of enrolment, not from 1 April.
Which hospitals accept the CMHT card?
Inside Manipur, treatment is available at:
- RIMS and JNIMS, the two medical college hospitals
- The district hospitals
- Moreh Hospital
- Community Health Centres at Kangpokpi, Wangoi, Haoreibi, Mao, Moirang, Nambol, Saikul, Kakching, Sekmai, Parbung, Heirok, Kamjong, Chakpikarong, Sagolmang, Nungba and Yairipok
- PHC Maram and UHC Singjamei
- Other empanelled private hospitals
Outside Manipur, treatment is available at Narayana Superspeciality Hospital, Midland Hospital and Research Centre Pvt. Ltd. and Agile Hospital Pvt. Ltd. in Guwahati (Assam), and Ruby General Hospital Private Limited in Kolkata (West Bengal).
The scheme deliberately steers patients to hospitals within Manipur first: the beneficiary "will be offered the option to choose hospitals within the State in the first instance, provided there is the availability of capacity and the treatment procedure". Out-of-state referral is the fallback, not the default.
At every empanelled public hospital the ISA posts a Health Coordinator whose job is to guide the beneficiary "from enrollment to admission until discharge", receiving the patient, verifying the card, arranging consultation and admission, counselling on treatment, making sure pre-authorisation is sent and approved, and helping with discharge.
What documents do I need to enrol in CMHT?
| Document | Mandatory | Notes |
|---|---|---|
| Aadhaar card | Yes | At least one family member; copy plus consent note |
| Eligibility proof | Yes | AAY card, disability certificate, husband's death certificate, DC certificate of economic status, or a membership letter |
| ID proof | Yes | Ration card, EPIC, PAN, driving licence, passport or government ID |
| Aadhaar of family members | Yes | For the names printed on the card |
| Non-employment declaration | Yes | Filled at the counter before the card is made |
| IDP certificate | For IDPs | Issued by the district Deputy Commissioner |
If a person does not yet have an Aadhaar card but wants to enrol, the scheme allows enrolment on the strength of the Aadhaar application number plus a consent form, provided the person gets enrolled into Aadhaar.
For local media persons and newspaper hawkers, an authorised agency's ID card is enough as eligibility proof.
How to apply for CMHT and enrol your family
- Check that your family falls in one of the CMHT categories, or be ready to ask for Deputy Commissioner verification if it does not.
- Collect the originals, Aadhaar card, an eligibility proof, an ID proof and the Aadhaar cards of your family members.
- Visit the nearest CMHT enrolment kiosk. Kiosks are set up at all district hospitals, RIMS, JNIMS and Wangoi CHC, and are run by the Implementation Support Agency alongside a claims help desk.
- Fill the declaration form for non-employment in government service at the counter.
- Let the Verifying Authority check and authenticate your documents. PHC Medical Officers nominate the Verifying Authorities for their areas, and the Medical Superintendents nominate them for RIMS and JNIMS.
- Collect the CMHT-Manipur Health card with its unique identification number, carrying up to five family names.
- If you are not in the SECC database, the Health Coordinator registers your claim, uploads your documents to the Deputy Commissioner for verification, and calls you back to enrol at the nearest district hospital once the DC clears it.
To use the card for treatment: go to an empanelled hospital, show the CMHT card to the Health Coordinator at the help desk, let them verify the card and your documents, and let the hospital raise the pre-authorisation request. Once pre-authorisation is approved, treatment is cashless, you pay the hospital nothing.
How do I add or remove a family member on the CMHT card?
Names are added and deleted on the recommendation of the head of the family. Where a member has died or has separated from the family, that name is removed from the scheme enrolment list on the head of the family's recommendation. Take the card and the supporting document to a district hospital enrolment kiosk.
Because only five names are printed, a family of eight will see the fifth name followed by "3 more". That is normal and does not mean the other members are uncovered.
Why do CMHT enrolments and claims get refused?
- No acceptable eligibility proof. The commonest reason. An ordinary (non-AAY) ration card is not by itself an eligibility proof; you need one of the listed documents or a DC certificate.
- Government employment. The non-employment declaration is a hard bar.
- Aadhaar missing and no application number given. At least one family member's Aadhaar credentials are essential.
- Treatment taken at a non-empanelled hospital. CMHT is cashless only inside the empanelled network; a bill from an outside hospital is not reimbursed.
- Pre-authorisation not raised or not approved before the procedure.
- Annual Rs 5 lakh floater limit already exhausted by earlier claims in the same year by any family member, including transport and daily allowances.
- Transport allowance claimed without bank details given at the time of hospitalisation.
Where do I complain about CMHT?
- Call centre: the ISA runs a 24x7 call centre and the registered toll-free number for CMHT-Manipur is 1800 103 2015. The call centre is the designated first channel for timely grievance redressal, working with the State Nodal Cell.
- Grievance link on cmhtmanipur.gov.in.
- Health Coordinator at the empanelled hospital, for admission, pre-authorisation and discharge problems.
- District Nodal Officer, in the rank of District Community Mobiliser, who coordinates the scheme in each district under the Deputy Commissioner.
- Implementation Support Agency office: Medi Assist Insurance TPA Pvt. Ltd.,
#127 Directorate of Health Services, Opposite RIMS Gate, Lamphelpat, RIMS Road, Imphal, Manipur 795 004.
Enrolment in CMHT is free. No fee is payable for the CMHT-Manipur Health card, and no agent is authorised to charge for enrolling a family.
How to check your CMHT application status
CMHT does not run a single online tracking dashboard where an applicant types in a reference number and sees a status screen. Because enrolment and treatment approval both happen through people, in this scheme "checking status" means going back to whichever step you are stuck at.
If you have applied for a CMHT-Manipur Health card and are waiting for it:
- Call the 24x7 toll-free number 1800 103 2015. The Implementation Support Agency's call centre can look up your enrolment record and tell you whether the card has been generated, is pending Deputy Commissioner verification, or needs a missing document.
- If your family was not found in the SECC database at enrolment, your case goes through DC verification rather than instant card issue. In that situation, the Health Coordinator who registered your claim is expected to call you back once the Deputy Commissioner's office clears it, so keep the enrolment kiosk's acknowledgment slip or your registered mobile number handy in case they try to reach you.
- Visit the same enrolment kiosk (district hospital, RIMS, JNIMS or Wangoi CHC) where you first applied and ask the Health Coordinator or Verifying Authority posted there for an update, since kiosk staff can see the case they logged.
- For a case still awaiting DC-level clearance, you can also approach the District Nodal Officer (District Community Mobiliser) under the Deputy Commissioner's office, since verification of non-SECC and "other poor family" cases is routed through that office.
- Use the grievance link on cmhtmanipur.gov.in if the call centre and kiosk visit do not resolve the delay.
If you already have a CMHT card and are asking about a hospitalisation claim or pre-authorisation:
- Ask the Health Coordinator stationed at the empanelled hospital where you were or will be admitted. Pre-authorisation status, approval and any query raised by the Implementation Support Agency on your claim are tracked at this desk, since the coordinator's role explicitly covers following the case "until discharge."
- Call 1800 103 2015 and quote your CMHT card number and the hospital name; the call centre is the designated first channel for claim-related grievances and can confirm whether pre-authorisation is approved, pending or rejected.
- Check how much of your family's annual Rs 5,00,000 floater limit remains before assuming a claim was wrongly refused, since a claim can stall simply because the yearly ceiling is already used up by an earlier admission in the same family.
- For a transport allowance that has not been credited, confirm with the Health Coordinator or call centre that your bank account details were correctly recorded at the time of hospitalisation, as that is the single most common reason this specific payment gets held up.
Keep your CMHT card number, Aadhaar number and the hospital or kiosk name ready before calling; the call centre and coordinators work off these identifiers rather than any separate online application ID.

Frequently asked questions
How much health cover does CMHT give in Manipur?
CMHT gives cashless cover of up to Rs 5,00,000 per enrolled family per year on a floater basis, for secondary and tertiary hospitalisation at empanelled hospitals. The Rs 5 lakh ceiling includes transport and daily allowances, so it is the total annual limit for the family, not per person.
Who is eligible for Chief Minister-gi Hakshelgi Tengbang?
Bona fide citizens of Manipur from poor families are eligible, specifically AAY card holders, widows, persons with disabilities, ASHA workers, Anganwadi workers and helpers, Ima Keithel women vendors, local media persons, newspaper hawkers and internally displaced persons. Other poor families not in these categories can be added after verification by the Deputy Commissioner.
What is the CMHT helpline number?
The toll-free number for CMHT Manipur is 1800 103 2015, run as a 24x7 call centre by the Implementation Support Agency, Medi Assist Insurance TPA Pvt. Ltd. It handles scheme queries and grievances.
Where can I use the CMHT card?
Inside Manipur the card works at the district hospitals, RIMS, JNIMS, Moreh Hospital, a list of Community Health Centres including Kangpokpi, Wangoi, Mao, Moirang, Nambol, Saikul, Kakching and Sekmai, PHC Maram, UHC Singjamei and empanelled private hospitals. Outside the state it works at Narayana Superspeciality, Midland and Agile hospitals in Guwahati and Ruby General Hospital in Kolkata.
How many names go on a CMHT card?
Up to five family member names are printed on the CMHT-Manipur Health card, each card carrying a unique identification number. If the family has more than five members, the extra count is written next to the fifth name, for example "2 more".
Does CMHT pay travel costs?
Yes. Within Manipur CMHT pays a transport allowance of Rs 300 per visit subject to an annual ceiling of Rs 3,000. For treatment outside the state at empanelled hospitals, the patient and one attendant get the cheapest economy airfare on that route up to Rs 30,000 a year, plus a daily allowance of Rs 1,000 up to Rs 10,000. All of it counts inside the Rs 5 lakh family limit.
What if my family is not in the SECC list?
You can still register a claim at a district hospital enrolment counter. The Health Coordinator records your documents and uploads the case to the Deputy Commissioner of your district for verification. Once the DC verifies, you are told to visit the nearest district hospital to enrol.
Is CMHT the same as Ayushman Bharat PM-JAY?
No. CMHT is a state scheme of the Government of Manipur with its own CMHT-Manipur Health card, its own empanelled hospital network and its own call centre, though the CMHT portal also carries a PM-JAY link. The two are administered separately.
