To open a Jan Aushadhi Kendra under the Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP), you must upload a fixed set of documents with your online application on janaushadhi.gov.in, and an application from an individual or proprietor without an Aadhaar card is rejected outright under the PMBI guidelines. This checklist tells you exactly what to gather, what each paper proves, and the mistakes that stall applications. The complete document checklist
Copy this list before you start the online form:
- Aadhaar card: identity of the individual applicant or proprietor. Mandatory; missing it means outright rejection.
- PAN card; required for every applicant category: individual, firm, company, trust, society, NGO or nominated agency.
- Pharmacist registration certificate. A D.Pharma or B.Pharma holder registered with the State Pharmacy Council must be the applicant or be employed by the applicant.
- Income Tax Returns for the last two years, for individual, institutional and private operating-agency applicants.
- Bank statement for the last six months; establishes financial capacity to run the Kendra.
- Proof of space; ownership papers, lease deed or allotment letter for a minimum of 120 sq ft of own or hired space. PMBI does not arrange premises.
- Distance-policy undertaking, a self-declaration that no existing Kendra operates within a 1 km radius of the proposed site.
- GST registration declaration. An undertaking to register for GST once the turnover threshold is crossed.
- Three cheques plus one cancelled cheque — drawn on an Indian nationalised bank in favour of PMBI, submitted after in-principle approval.
Category-specific extras
- SC/ST or Divyangjan (PwD) certificate, only if you claim the Rs 2 lakh special incentive and the Rs 5,000 fee waiver.
- Darpan ID, only for NGO applicants.
- Women entrepreneur / ex-serviceman / aspirational-district proof; to claim the fee exemption and special-category status, which cannot be changed after submission.
What each document proves
The Aadhaar and PAN establish who you are and anchor the KYC. The pharmacist registration certificate is the licence-critical one: a Kendra legally dispenses medicines, so PMBI must see a real registered pharmacist behind it. The two years of ITRs and the six-month bank statement together prove you can fund the 50% advance PMBI requires against each proforma invoice. The space proof and distance undertaking protect the network to 120 sq ft used solely for the Kendra, and no cannibalising of an existing Kendra within 1 km (the 500-metre exemption applies only near district government hospitals, private hospitals with 100+ beds, and medical-college hospitals).
Format and common mistakes
- Upload self-attested scanned copies, clear and legible; blurred pharmacist certificates are a frequent hold-up.
- The name on Aadhaar, PAN and bank documents should match. A mismatch triggers verification queries.
- The lease agreement must be valid and in the applicant's name, an expired or third-party lease fails the space check.
- Choose your special-category status at registration if you qualify; the guidelines say this choice cannot be changed later, so a woman entrepreneur who forgets to select it loses the fee waiver and the Rs 2 lakh incentive route.
- Keep the Rs 5,000 application fee ready online unless you have valid category proof for exemption. The fee is non-refundable.
- Do not treat the drug licence as an upfront document: it is obtained in the Kendra's name after in-principle approval, not at application.

Before you hit submit, confirm you have: Aadhaar, PAN, pharmacist registration, two years of ITRs, six months of bank statements, 120 sq ft space proof, the distance undertaking, the GST declaration, and; if claiming the special incentive; your category certificate. With those in hand, the online application on janaushadhi.gov.in moves to viability assessment without avoidable rejections. No agent can arrange a Kendra for you; the portal is the only route and the Rs 5,000 fee is the only charge.
