Before you start: decide your route and gather files

You do not need an existing disability certificate to apply. Online application, screen by screen

1. Open the portal. Go to swavlambancard.gov.in and select Apply for Disability Certificate & UDID Card.

Offline application through your district

1. Visit the District Disability Rehabilitation Centre (DDRC), the Chief Medical Officer's office, or the designated government hospital in your district.
