Track a Unique Disability ID (UDID) / Swavlamban Card application at swavlambancard.gov.in. Open Track Your Application Status, enter your enrolment number, UDID number or request number with your date of birth and the captcha, and submit to see the current stage. The UDID card is issued free by the Department of Empowerment of Persons with Disabilities, so there is no "payment" to track. There is no payment status, and why

The UDID or Swavlamban Card carries no cash benefit and no fee. Registration, medical assessment and card issue are all free of cost. So unlike a scholarship or pension, there is nothing to disburse and no PFMS credit to check. "Status" here means where your application sits in the certification-and-issue pipeline. Anyone asking for a payment to release your card is not authorised.

How to check application status step by step

1. Open swavlambancard.gov.in on a browser, or the UDID app from the Play Store.
2. Click Track Your Application Status on the homepage (on the app, choose PwD Login).
3. Enter your enrolment number, UDID number or request number.
4. Enter your date of birth and the captcha shown.
5. Submit to see the current stage of your application.

What each status means and what to do next

• Not Verified / Pending at District. The District Disability Rehabilitation Officer has not yet verified your form. If it lingers, contact the District Disability Rehabilitation Centre (DDRC) or District Social Welfare Officer.
• Verified / Forwarded to CMO: your details are confirmed and the file has moved to the Chief Medical Officer or designated medical authority for assessment. If you did not upload an existing disability certificate, this is where the physical medical examination is scheduled.
• Pending Medical Assessment. The single most common stall; a designated doctor or medical board must examine you and record the percentage. Attend when called; the file cannot advance without it.
• Card Generated — the disability certificate and UDID card have been issued. Log in and download the e-UDID card and e-disability certificate from your dashboard.
• Card Dispatched. The physical card has been posted; the e-UDID is already valid in the meantime.

How to download the e-UDID once generated

1. Log in at swavlambancard.gov.in with your registered UDID or enrolment number.
2. Open the applicant dashboard and confirm the status shows the card is generated.
3. Download the e-UDID card and the e-disability certificate, both are valid documents you can use before the physical card arrives by post.

Why an application stalls

Per the Department of Empowerment of Persons with Disabilities, the most common cause of a long pending status is the medical assessment stage, where a designated doctor or board must physically examine the applicant. Other frequent causes are mismatched Aadhaar details, an unclear uploaded photograph or certificate, and pending verification by the District Disability Rehabilitation Officer. The first is resolved by attending the assessment; the others through the update option or by following up with the district authority.

FAQ

Can I track without an enrolment number? The tracker also accepts your UDID number or request number with your date of birth; if you have lost all three, contact the district office that filed your form.

My status has said "pending medical assessment" for weeks, what do I do? Follow up with the DDRC or CMO office in your district to confirm your assessment date; the card cannot be generated until the percentage is recorded.

Is the e-UDID enough, or do I need the physical card? The downloaded e-UDID and e-disability certificate are valid documents and can be used before the physical card is delivered.
