Death Certificate in Andhra Pradesh
Andhra Pradesh state government scheme
Death Certificate in Andhra Pradesh is issued by the Registrar of Births and Deaths of the Municipality or Gram Panchayat where the death happened. A death registered within one year is handled directly at the Municipality or Panchayat office, while a death older than one year goes through MeeSeva as a late registration with a service charge of ₹30 and a service level of 21 days.
What is a death certificate in Andhra Pradesh and who issues it?
A death certificate in Andhra Pradesh is the certified extract of the entry made in the register of deaths under the Registration of Births and Deaths Act 1969. The registrar is the local body of the place where the death occurred, the Municipal Corporation, Municipality or Nagar Panchayat in urban Andhra Pradesh, working under the Commissioner and Director of Municipal Administration, and the Gram Panchayat in rural areas under Panchayat Raj.
Andhra Pradesh is unusual in how a citizen reaches that registrar. The state runs the Grama Sachivalayam and Ward Sachivalayam network, through which several hundred government services including birth and death registration are delivered at the village and ward level, on both the Civil Registration System and manual routes. Alongside it, MeeSeva at ap.meeseva.gov.in is the one-stop citizen services portal where birth and death certificate services are available. District administrations also point citizens to the Urban Bodies MIS at ubd.ap.gov.in for municipal records.
Andhra Pradesh splits the service into two clearly different things, and knowing which one you need saves weeks. A death certificate for a current registration covers a death registered within one year of the event and is applied for at the Municipality or Panchayat office. A late registration of death covers anything older than one year and is applied for through MeeSeva. The documents, the fee and the processing route are different for each.
Who can and who cannot apply?
You can report a death and obtain the certificate in Andhra Pradesh if the death took place inside the state. Under the Act the informant is the head of the household where the death occurred; for a hospital death the institution reports it; for a death in custody the jail in-charge; and where a body is found unclaimed, the police or the village headman.
Any close relative of the deceased: spouse, son, daughter, parent, sibling or a legal heir — can apply for a certified copy of a registered death, or for a correction to the name, age or address in the entry.
You cannot apply in Andhra Pradesh for a death that occurred in another state or union territory, including Telangana. Registration follows the place of death, so a death in Hyderabad is registered by the Telangana local body even where the family is entirely resident in Andhra Pradesh. You are also not eligible for a current registration once more than a year has passed, that application must go down the late registration route, and the counter will refuse a current-registration form for it.
What documents are required?
For a current registration (death within one year), Andhra Pradesh district administrations list:
- Report of Death in Form 2, signed by the informant.
- The doctor's certificate of the cause of death from the treating hospital or attending practitioner.
- A panchanama from the recognised authority, particularly where the death was unnatural, sudden or unattended.
For a late registration (death older than one year), the listed documents are:
- The physical document or record evidencing the death.
- A non-availability certificate from the Gram Panchayat or the Municipal Administration, confirming that no entry exists in the register.
- A copy of the ration card of the household.
- A self-affidavit setting out the date, place and circumstances of the death and the reason for the delay.
Carry the identity proof of the deceased and of the applicant in both cases. The name, age and address printed on the certificate come from what is declared here, and a mismatch with Aadhaar or the ration card is the most common reason a bank later rejects the certificate.
How to apply for a death certificate in Andhra Pradesh
- Decide which service you need. If the death is less than a year old, it is a current registration. If it is more than a year old, it is a late registration of death, and only MeeSeva handles it.
- For a current registration, go to the local registrar. Submit Form 2 at the Municipality, Municipal Corporation or Gram Panchayat office of the place of death, or at your Grama or Ward Sachivalayam, with the doctor's certificate and the panchanama.
- For a late registration, get the non-availability certificate first. Ask the Gram Panchayat or the Municipal Administration to certify in writing that no death record exists for the deceased. Nothing moves without it.
- File the late registration at a MeeSeva centre. Andhra Pradesh district pages state that citizens can apply for this service at the nearest MeeSeva centre without needing to approach government offices directly. Attach the non-availability certificate, the ration card copy and the self-affidavit.
- Pay the ₹30 service charge listed for the late registration of death service and keep the MeeSeva acknowledgement receipt with its application number.
- Wait out the 21-day service level and collect the certificate. The service is listed as Category B and is converted to Category A once the application has been received.
How do you check the status of an Andhra Pradesh application?
Use the MeeSeva application number printed on the acknowledgement receipt. The MeeSeva portal at ap.meeseva.gov.in carries a status check against that number, and the same number is what the MeeSeva operator uses if you go back to the centre. For a current registration made at a Municipality or Gram Panchayat, follow up with the registrar's office directly, and for a municipal record check whether the entry has reached the Urban Bodies MIS at ubd.ap.gov.in. Where the case is running through a Grama or Ward Sachivalayam, the Digital Assistant at that Sachivalayam can pull up the application.
Why do Andhra Pradesh death certificate applications get rejected?
- The application was filed as a current registration when the death is more than a year old, or as a late registration when it is not.
- No non-availability certificate was attached to a late registration.
- The doctor's certificate of cause of death is missing for a current registration.
- The death occurred outside Andhra Pradesh, so no state local body holds the register entry.
- The name, age or address on the report does not match the ration card or the Aadhaar of the deceased.
- The self-affidavit does not explain why the death went unregistered for more than a year.
- The panchanama is missing in an unnatural or unattended death case.
What does it cost, and should you use an agent?
Andhra Pradesh publishes ₹30 as the service charge for the late registration of death, and a current registration made in time at the Municipality or Gram Panchayat carries only the statutory registration fee at the registrar's counter. Those published amounts, paid at a MeeSeva counter or a government office against a printed receipt, are the whole legitimate cost.
Touts outside MeeSeva centres and municipal offices offer to "arrange" a death certificate for several thousand rupees. They cannot shorten the 21-day service level, they cannot make a registrar accept a case without a non-availability certificate, and a death entry created on false particulars is a criminal matter for the family that uses it as much as for the agent. File it yourself at the Sachivalayam or MeeSeva counter, insist on the printed acknowledgement, and refuse to pay anything the published fee does not cover.

Frequently asked questions
Who issues a death certificate in Andhra Pradesh?
The Registrar of Births and Deaths of the local body where the death took place issues it — the Municipal Corporation, Municipality or Nagar Panchayat in urban Andhra Pradesh, and the Gram Panchayat in rural areas. The Grama and Ward Sachivalayam network and MeeSeva centres act as the counters citizens actually walk into.
What is the difference between a current and a late death registration in Andhra Pradesh?
A death registered within one year of the event is a current registration, applied for directly at the Municipality or Panchayat office with the doctor's certificate and panchanama. A death older than one year is a late registration, applied for through a MeeSeva centre with a non-availability certificate, ration card copy and self-affidavit.
What does a late death registration cost in Andhra Pradesh?
Andhra Pradesh district administration pages list a service charge of ₹30 for the late registration of death service, with a service level agreement period of 21 days. The service is classified as Category B and is converted to Category A once the application is received.
How long does a death certificate take in Andhra Pradesh?
The published service level for the late registration of death service in Andhra Pradesh is 21 days. A current registration made at the Municipality or Gram Panchayat within the statutory window is generally issued far faster, because the registrar only has to verify the doctor's certificate and make the entry.
Do I have to visit a government office to apply in Andhra Pradesh?
No — Andhra Pradesh district pages state that citizens can apply for the late registration of death through the nearest MeeSeva centre without approaching government offices directly. A current registration is filed at the Municipality, Gram Panchayat or Grama/Ward Sachivalayam counter.
Can a death still be registered in Andhra Pradesh after many years?
Yes. Andhra Pradesh district administrations state explicitly that a death can be registered even after one year, through the late registration route at MeeSeva backed by a non-availability certificate and a self-affidavit. A very old event will need the order of the competent authority before the registrar makes the entry.
Where do urban local bodies in Andhra Pradesh keep the death records?
Urban local bodies in Andhra Pradesh work under the Commissioner and Director of Municipal Administration, and district pages point citizens to the Urban Bodies MIS at ubd.ap.gov.in for municipal birth and death records. Records for a village death sit with the Gram Panchayat register instead.
Does an Andhra Pradesh death certificate expire?
A death certificate never expires, because the registered fact of a death does not change. Banks and insurers sometimes ask for a recently issued copy, which you obtain as a fresh certified extract of the same registration entry rather than as a new registration.
