Birth Certificate in Assam
Assam state government scheme
Birth Certificate in Assam is registered by the local registrar. The municipal board, town committee, gaon panchayat or notified hospital where the birth occurred: under the Registration of Births and Deaths Act 1969. Report the birth within 21 days to avoid a late fee, apply through Sewa Setu or the national Civil Registration System portal, and note that a birth over one year old needs a District Magistrate's order.
What is a birth certificate in Assam?
A birth certificate in Assam is the extract of the birth register maintained under the Registration of Births and Deaths Act 1969 by the registrar of the local body in whose area the birth occurred. In Assam that registrar sits in a municipal board, a town committee, a gaon panchayat, or in a hospital notified as a registration unit, all reporting to the Chief Registrar of Births and Deaths for the state.
Assam runs two relevant online systems and it helps to know which does what. Sewa Setu, at sewasetu.assam.gov.in, is the state's Right to Public Services portal. The Government of Assam describes it as carrying over 800 government services across citizen, business, utility and scheme categories, with delivery time limits stipulated under the Assam Right to Public Services (ARTPS) Act. eDistrict Assam, at edistrict.assam.gov.in, is managed by the Department of Information Technology and carries an Application Status lookup against an application number. Underneath both, the birth register itself is a civil registration record, and since 1 October 2023 it is maintained on the national Civil Registration System that every state feeds.
That 2023 date matters. Under the Registration of Births and Deaths (Amendment) Act 2023, the digitally issued birth certificate is the single accepted proof of date of birth for admission to an educational institution, a driving licence, a passport, voter registration, Aadhaar and government employment for anyone born on or after 1 October 2023.
Who can and who cannot apply?
You can apply for a birth registration in Assam if the birth occurred inside Assam. The informant is normally the child's father or mother; for an institutional birth the medical officer in charge reports the event directly, which is why most Assam hospital births are registered without the family filing anything separately. For a birth in a jail the jailor reports it, and in a hostel or rest house the person in charge does. An adult born in Assam whose birth was never entered in the register can apply personally through the delayed route.
You cannot apply to an Assam registrar for a birth that occurred outside Assam; registration follows the place of the event, so a child born in Meghalaya, Nagaland or West Bengal must be registered there even if the family's home is in Assam. You are also not eligible for a second registration of a birth already recorded; that case is a correction or a name inclusion, handled by the same unit. A birth abroad to Indian parents is registered through the Indian mission under separate law.
There is no income limit, no caste condition and no land-holding condition attached to a birth certificate. It records an event.
What documents are required?
For a routine institutional birth, the hospital supplies the report and the parents supply identity details. Keep ready:
- The hospital or nursing home birth report, or the discharge summary carrying date, time and place of birth.
- Aadhaar or another photo identity of both the mother and the father.
- Proof of the parents' address for the ward, town or village where you are filing: Aadhaar, voter ID, ration card, an electricity bill or a municipal or panchayat tax receipt.
- The parents' marriage certificate, where the registration unit asks for it.
- For a delayed case, an affidavit setting out the date, time and place of the birth and why it went unregistered, with secondary evidence such as a school admission register extract, an immunisation card, an anganwadi record or a hospital record.
How to apply for a birth certificate in Assam
- Check whether the hospital has already reported the birth. In Assam a notified hospital sends the birth report to the registration unit itself, so for an institutional birth the first step is a phone call to the hospital records desk, not a form.
- Identify the correct registration unit, the municipal board or town committee ward office for an urban birth, the gaon panchayat for a rural one, or the hospital where it is itself a notified unit. Filing against the family's home local body instead of the one covering the hospital is the most frequent Assam error.
- Apply online. Use Sewa Setu at sewasetu.assam.gov.in, where notified services carry an ARTPS time limit, or file the birth report on the national Civil Registration System portal as a general public user against the Assam registration unit. A Common Service Centre operator can do the data entry if you cannot scan documents yourself.
- Fill the birth details and upload the documents, the child's date, time and place of birth and sex, the mother's and father's names, Aadhaar numbers and address, plus the hospital report and identity and address proof.
- Pay any applicable late fee or copy fee on the payment screen and save the acknowledgement.
- Track the application with the application number and download or collect the certificate once the registrar approves the entry.
How to apply offline at the registration unit
- Visit the municipal board, town committee or gaon panchayat office covering the place of birth with the hospital report and the parents' identity and address proof.
- Submit the statutory birth reporting form. The registrar keys the same details into the civil registration system, so the paper route ends in the same register as the online one.
- Pay the late fee where the report is beyond 21 days and take the receipt.
- Collect the signed certificate against the acknowledgement, and check the spelling of every name and the date before you leave the counter.
What are the rules for a late birth registration in Assam?
Assam applies the windows in the Registration of Births and Deaths Act 1969:
- Within 21 days — reported to the registrar with no late fee.
- After 21 days and within 30 days: registered on payment of the prescribed late fee.
- After 30 days and within one year; registered with the written permission of the prescribed authority, on the prescribed late fee and an affidavit.
- After one year: registered only on the order of the District Magistrate or an officer authorised by the District Magistrate, under section 13(3) as amended in 2023.
The over-one-year route is what most adults in Assam seeking a first birth certificate actually need. It is an enquiry, not a form-filling exercise: the District Magistrate's office weighs the affidavit against whatever school, hospital, anganwadi or immunisation record you can produce. Collect that evidence before opening the file.
How do you check status and correct mistakes?
Both eDistrict Assam and Sewa Setu carry an application status lookup that works on the application number printed on your acknowledgement. Use it first, then follow up in person with the registration unit holding the file: in practice a stalled birth application is sitting with a registrar rather than in the software. The Sewa Setu helpline is 1800 345 3574, and queries can be sent to sewasetu.queries@gmail.com.
For a wrong spelling, a wrong date or a wrong parent's name, apply to the same registration unit for a correction with documentary proof of the correct entry. Do not file a fresh application: a duplicate entry for one birth takes far longer to unwind than the original mistake.
Why are Assam birth applications rejected?
- The birth is more than a year old and no District Magistrate order has been produced.
- The application was filed against the residential local body instead of the unit covering the hospital.
- The hospital never sent the birth report, so there is nothing for the registrar to approve.
- Names differ across the hospital record, the parents' Aadhaar and the application form.
- Scans of the hospital report are unreadable, or a discharge summary without a date of birth has been attached.
- The delayed-registration affidavit does not say why the birth went unregistered.
- A registration already exists for the same child.
What does it cost, and what about agents?
Registering a birth within 21 days carries no registration fee under the Registration of Births and Deaths Act 1969. After that you pay only the late fee under section 13 and the notified copy fee for a certified extract, the amounts the registration unit publishes at its counter or the portal shows on its payment screen. Where you use a Common Service Centre, the operator's notified service charge is added and nothing else.
No clerk, ward officer, hospital staff member or CSC operator in Assam is entitled to a rupee beyond that. Agents who promise a birth certificate in a few days for a few thousand rupees cannot shorten a District Magistrate's enquiry in an old case, which is the only genuine bottleneck. If money is demanded or the file simply stops, take it up with the Executive Officer of the municipal board or the Block Development Officer, then with the District Registrar of Births and Deaths, and use the Sewa Setu helpline on 1800 345 3574 for a service notified under the ARTPS Act.

Frequently asked questions
Which portal do I use for a birth certificate in Assam?
Assam delivers notified citizen services through Sewa Setu at sewasetu.assam.gov.in, the state's Right to Public Services portal, which carries over 800 government services with time limits set under the Assam Right to Public Services Act. Birth registration itself is a civil registration function of the local registrar and also runs on the national Civil Registration System portal.
Who registers a birth in Assam?
The registrar of the local body where the birth occurred registers it — the municipal board, town committee or gaon panchayat, and in many cases the notified hospital itself. All of them work under the Chief Registrar of Births and Deaths for Assam.
How many days do I get to register a birth in Assam?
You have 21 days from the date of birth to report it without a late fee, under section 8 of the Registration of Births and Deaths Act 1969. After 21 days the birth can still be registered, but the authority who must approve it changes at 30 days and again at one year.
What happens if the birth is more than one year old?
A birth over one year old can be registered in Assam only on the written order of the District Magistrate or an officer authorised by the District Magistrate, under section 13(3) of the Registration of Births and Deaths Act 1969 as amended in 2023. The order follows an enquiry into the secondary evidence you produce, so allow far more time than for a routine registration.
What does an Assam birth certificate cost?
Registration within 21 days carries no registration fee under the Act. Beyond that you pay the late fee under section 13 and the notified copy fee for a certified extract — the amounts shown at the registration unit counter or on the portal's payment screen, and nothing more.
How do I check my Assam application status?
Use the Application Status option with your application number on the portal you applied through — the Assam eDistrict portal and Sewa Setu both carry one — and follow up with the registration unit that holds the file. The Sewa Setu helpline is 1800 345 3574 and queries can be emailed to sewasetu.queries@gmail.com.
How do I add my child's name to an Assam birth certificate?
Apply to the same registration unit that registered the birth for a name inclusion. Most births in Assam are registered before the parents settle on a name, so the registrar treats this as a normal second step rather than a correction.
I was born in Assam but live in another state — where do I apply?
Apply to the Assam registration unit covering the place of birth, because a birth is always registered where it happened. If the birth is over a year old, the District Magistrate of that Assam district is the authority who must permit the registration.
