What is a death certificate in Lakshadweep and who issues it?

A death certificate in Lakshadweep is the legal record of a death, made under the Registration of Births and Deaths Act 1969 by the Registrar appointed for the island on which the death occurred. Lakshadweep's registration machinery is built around its health network rather than around a revenue office: the Health Inspector in the Dweep Panchayat islands is the designated Registrar, and on the smallest inhabited islands the first aid centre staff carry out the function. The Department of Health Services of the UT Administration, at IG Hospital Road, Kavaratti 682555, is the department behind that network and is contactable on 04896-262316 between 10 am and 6 pm.

That structure follows from the geography. Lakshadweep is ten inhabited islands scattered across roughly 400 kilometres of the Arabian Sea, and no single district office could serve them all. The Department of Health Services runs one 50-bed district hospital, two sub-district hospitals of 50 and 20 beds, three community health centres, three primary health centres, 14 sub-centres and two first aid centres. That facility list is also the list of places a Medical Certificate of Cause of Death can realistically come from, which is why the health department and the registration function sit together in the islands.

Who can apply for a death certificate in Lakshadweep?

The informant is the person legally obliged to report the death. For a death in a house, the head of the household reports it. For a death in a hospital, sub- district hospital, community health centre or primary health centre, the head of that institution reports it. For a death in a public place or in custody, the officer in charge of that place reports it. On the islands this in practice means either the family walking to the island Registrar, or the health facility filing the report on the family's behalf.

For a copy of a death already registered, any family member of the deceased can apply, spouse, son, daughter, parent: or a legal heir who needs the certificate for a bank claim, an insurance claim, family pension, deletion of the deceased's name from the ration card, or a succession or property matter. Applicants cannot apply in Lakshadweep for a death that occurred outside the Union Territory; jurisdiction follows the place of death, so a family whose relative died at a referral hospital in Kochi or Kozhikode has to obtain the certificate from the Kerala local body concerned. That is a real and frequent situation in Lakshadweep, because serious cases are routinely referred to the mainland. A person with no relationship to the deceased and no legal interest in the record is also not eligible to obtain a copy. There is no income, caste or Scheduled Tribe test for this document; unlike almost every other Lakshadweep certificate, where ST status and island nativity are central, a death certificate is a civil registration record open to the family of any person who died in the UT.

What documents are needed?

The core set is the report of death in Form 2, the Medical Certificate of Cause of Death in Form 4 or Form 4A from the treating doctor or facility, identity proof of the deceased, proof of residence on the island, and identity proof of the family member reporting. Lakshadweep issues a Resident ID Card as a separate Revenue Department service, and that card or the ration card is the usual residence proof. For a death at home rather than in a health facility, the Registrar commonly asks for the burial record from the island burial ground or masjid committee. An unnatural death. A drowning, a fishing or boat accident, a road accident: adds the police report and the post-mortem report, because the cause of death entry then comes from the mortuary record and not from a treating physician.

How to get a death certificate in Lakshadweep, the step-by-step process

  1. Obtain the Medical Certificate of Cause of Death from the health facility that attended the deceased, the district hospital at Kavaratti, a sub-district hospital, a community health centre or a primary health centre. For a death at home with no medical attendance, collect the burial record instead.
  2. Report the death in Form 2 to the island Registrar within 21 days of the date of death. In a Dweep Panchayat island that is the Health Inspector; on the smallest islands it is the first aid centre. If the death happened in a health facility, confirm with that facility that it has already filed the report rather than filing a second one.
  3. Attach the supporting papers — identity proof of the deceased, residence proof, and your own photo ID with a statement of your relationship to the deceased.
  4. Let the Registrar make the entry on the Civil Registration System at dc.crsorgi.gov.in, the national system on which every birth and death in India has been registered since the Registration of Births and Deaths (Amendment) Act 2023 came into force on 1 October 2023.
  5. Collect the certificate and take extra copies at the same visit. The bank, the insurer, the pension office, the ration card office and any succession file each want a copy, and inter-island travel to repeat the trip depends on ship and helicopter schedules.

How to apply when the death is more than a year old

Lakshadweep has a named authority for this. The Additional District Magistrate, Lakshadweep has been appointed to issue orders registering events reported more than one year after they occurred. The route is: assemble the evidence of the death: hospital or health facility records, the burial record, the identity documents of the deceased and a sworn statement by the informant, apply to the Additional District Magistrate for an order under Section 13(3) of the Registration of Births and Deaths Act 1969, and then take that order to the island Registrar, who makes the entry and issues the certificate. Between 21 days and one year, no magisterial order is needed but the prescribed late fee applies and a written explanation of the delay is asked for.

What does it cost, and should I pay an agent?

Registration reported inside the 21-day window is free; the only charge is the prescribed fee for a certificate copy, and Lakshadweep issues the birth certificate free of charge immediately after registration at the same counters. The UT Administration does not publish a separate death certificate copy fee schedule on lakshadweep.gov.in, so ask the island Registrar what the listed charge is and take a receipt for anything you pay. Pay only the government's listed fee at the Registrar's own counter. There is no express or tatkal service, and on islands with a few thousand residents each, anyone offering to "arrange" a certificate for a fee is a middleman adding cost to a duty the Registrar already owes you.

How do I check the status of the application?

Go back to the same island Registrar with the date of death, the name of the deceased and the Form 2 acknowledgement, and ask whether the entry has been made and what registration number it carries. Where a hospital filed the report, the records section of that facility holds the reference. Do not file a second Form 2 for the same death, a duplicate entry in the register is considerably harder to correct than a delayed one. Escalation from the island Registrar runs to the Department of Health Services at Kavaratti on 04896-262316, and to the Collector and District Magistrate's office for the Union Territory.

Does a death certificate expire or need renewal?

No. A death certificate has no validity period and is never renewed. What sometimes needs correction is a misspelt name, a wrong age or a wrong island entered at the time of registration. Corrections are made by the same Registrar on a written application supported by documentary proof of the correct particular, the deceased's Aadhaar, ration card or school record, after which a corrected certificate is reissued against the same registration number.

Why do death certificate applications get rejected in Lakshadweep?

Most refusals are procedural. A report filed after 21 days without an explanation of the delay is held up, and one filed after a year without the Additional District Magistrate's order cannot be registered at all. A name on the medical certificate that does not match the deceased's Aadhaar or ration card stalls verification until the difference is explained in writing. A death that actually occurred at a mainland referral hospital cannot be registered by a Lakshadweep Registrar, however long the family has lived on the island. An unnatural death filed without the police and post-mortem reports is incomplete. And a copy asked for by someone who cannot show a relationship to the deceased or a legal interest in the record is refused.

Where do I complain if the certificate is delayed?

Start with the island Registrar holding your Form 2, then take it to the Department of Health Services, IG Hospital Road, Kavaratti 682555, on 04896-262316 during office hours of 10 am to 6 pm. The Deputy Collector of your sub-division, for example the Deputy Collector Office, Agatti Island, is the local revenue authority to approach when a service has stalled, and the Collector and District Magistrate, Lakshadweep at Kavaratti is the final administrative escalation within the Union Territory.

Frequently asked questions

Within how many days must a death be registered in Lakshadweep?

Twenty-one days from the date of death, the period fixed by the Registration of Births and Deaths Act 1969 and applied across every State and Union Territory including Lakshadweep. Reporting inside 21 days is the free and straightforward route; delay adds a late fee, and delay beyond a year requires a magisterial order.

Who registers a death in Lakshadweep?

The Registrar appointed for the island where the death occurred, working under the Union Territory's Health Services machinery. In the Dweep Panchayat islands the Health Inspector is the designated Registrar, and on the smallest islands the first aid centre staff perform the function.

What if the death was never registered and it is now more than a year old?

The Additional District Magistrate, Lakshadweep is the authority appointed to order registration of an event reported more than one year after it occurred. Apply to that office with the evidence of the death — hospital records, the burial record, and the identity documents of the deceased — and take the resulting order to the island Registrar.

Which portal is the death registered on?

The national Civil Registration System at dc.crsorgi.gov.in, which has carried all birth and death registration in India since the Registration of Births and Deaths (Amendment) Act 2023 took effect on 1 October 2023. Lakshadweep has no separate UT-specific death registration portal of its own.

What does a Lakshadweep death certificate cost?

The UT Administration does not publish a death certificate copy fee on lakshadweep.gov.in, so ask the island Registrar for the listed charge and take a receipt. Lakshadweep issues the birth certificate free of charge immediately after registration, and the same counter handles death records.

Do I need a Medical Certificate of Cause of Death?

Yes for any death in a hospital or attended by a doctor, and the treating facility issues it — the district hospital at Kavaratti, a sub-district hospital, a community health centre or a primary health centre. For a death at home with no medical attendance, the family reports in Form 2 and the Registrar records the death as unattended.

Can I get a Lakshadweep death certificate for a relative who died on the mainland?

No. Jurisdiction under the Registration of Births and Deaths Act 1969 follows the place of death, not the residence of the family. A Lakshadweep family whose relative died in a Kochi or Kozhikode hospital must obtain the certificate from that local registration authority in Kerala.

Should I pay an agent to get the certificate faster?

No. Registration is a statutory duty of the island Registrar, there is no tatkal route in Lakshadweep, and the same application filed directly at the Registrar's counter costs only the government's listed charge. Pay nothing without a receipt.
