What is Shaheed Veer Narayan Singh Ayushman Swasthya Yojana?
Shaheed Veer Narayan Singh Ayushman Swasthya Yojana is the flagship health assurance scheme of the Government of Chhattisgarh, run by the Department of Public Health and Family Welfare through the State Nodal Agency in the Directorate of Health Services. The scheme is named after Shaheed Veer Narayan Singh, the zamindar of Sonakhan who was hanged in Raipur in 1857 and is remembered as Chhattisgarh's first freedom fighter.
Shaheed Veer Narayan Singh Ayushman Swasthya Yojana is the renamed version of Dr Khubchand Baghel Swasthya Sahayata Yojana (DKBSSY), which the state government renamed in 2024. Readers searching for "Shaheed Veer Narayan Singh Swasthya Yojana" or the older DKBSSY name are looking at the same entitlement: the hospitals, packages and cards did not change with the name.
According to the State Nodal Agency portal, the scheme gives cashless in-hospital treatment at empanelled government and private hospitals, sitting on top of the central Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) and extending cover to Chhattisgarh families that the central SECC list leaves out.
Scale of the scheme as published on the portal in August 2026
- 1,678 empanelled hospitals across the state.
- Around 49.98 lakh total beneficiaries recorded.
- Around 47.30 lakh families benefited.
Why Chhattisgarh runs its own scheme on top of PM-JAY
The central PM-JAY list is built from the 2011 Socio-Economic and Caste Census. Chhattisgarh's own ration card database under the state Food Security Act covers a far larger share of households. Rather than leave those families uncovered, the state funds the difference itself, which is why a family that fails the PM-JAY eligibility check can still get treatment here.
How much cover does the scheme give?
| Ration card category | Annual cover per family |
|---|---|
| SECC-listed (deprived), Antyodaya, Priority | Rs 5 lakh |
| Other eligible ration card categories | Rs 50,000 |
The cover is a family floater. A family of six shares one Rs 5 lakh limit for the year; it is not Rs 5 lakh each. The limit resets each policy year, and an unused balance does not carry forward.
Payment goes directly from the State Nodal Agency to the hospital under a defined package rate list. The patient pays nothing at the counter for a covered package, including the bed, surgery, medicines, diagnostics and follow-up defined inside that package.
Who is eligible for Shaheed Veer Narayan Singh Ayushman Swasthya Yojana?
Your family is covered if:
- You hold a valid Chhattisgarh ration card issued under the state Food Security Act, in any of the eligible categories.
- The family appears in the beneficiary database, or can be added through the Beneficiary Identification System (BIS) at an empanelled hospital.
- Each member seeking treatment can complete Aadhaar-based e-KYC, by fingerprint or by OTP.
- Treatment is taken at an empanelled government or private hospital.
Your family is not eligible or cannot claim if:
- You do not hold a Chhattisgarh ration card. The ration card is the entry document; there is no alternative income certificate route.
- The treatment is taken at a hospital that is not empanelled. Bills from a non-empanelled hospital cannot be reimbursed afterwards.
- The procedure falls in the excluded list: cosmetic surgery, IVF and fertility treatment, AYUSH and traditional medicine systems.
- The visit is an outpatient consultation. The scheme covers in-hospital treatment packages, not walk-in OPD or over-the-counter pharmacy purchases.
- The family's annual cover for the year is already exhausted.
Notably, there is no age limit, no family size cap and no pre-existing disease waiting period. A newly diagnosed patient can be admitted the same day the card is made.
What documents are required?
| Document | Mandatory | Notes |

| Chhattisgarh ration card | Yes | Decides Rs 5 lakh or Rs 50,000 cover |
| Aadhaar card of the patient | Yes | For e-KYC through the BIS |
| Ayushman card | Yes | Free; can be made at admission |
| Registered mobile number | No | Needed for OTP e-KYC if fingerprints fail |
| Doctor's admission advice | No | Helps raise correct pre-authorisation |
| Government photo ID | No | Sometimes asked alongside Aadhaar |
How to apply for and get your Ayushman card
There is no application form and no fee. What you need is a card, and the card is generated free.
- Check whether your family is already in the database using the Beneficiary Identification System link on govthealth.cg.gov.in/sna, or ask at any empanelled hospital's Ayushman Mitra desk.
- Take your ration card and Aadhaar card to an empanelled hospital, an Ayushman kiosk, a Choice centre or a Common Service Centre.
- Complete e-KYC at the counter — fingerprint biometric first, and OTP to your Aadhaar-linked mobile if the fingerprint scan fails.
- Have the operator add every family member named on the ration card, not only the person who is unwell. Cards for children and elderly members are the ones most often missed.
- Collect the Ayushman card, which is generated free of cost. Ask for a printed copy and also save the digital version.
- Optionally create an ABHA number under the Ayushman Bharat Digital Mission at the same counter, which links your health records.
How to take cashless treatment at a hospital
- Find an empanelled hospital using the hospital search on the State Nodal Agency portal, or call 104 and ask for the nearest one for your condition.
- Report to the Ayushman Mitra desk in the hospital before admission, not after, and hand over the Ayushman card, ration card and Aadhaar.
- Let the desk complete biometric verification of the patient and raise the pre-authorisation request to the State Nodal Agency for the treatment package.
- Wait for pre-authorisation approval. Emergency admissions proceed immediately and the approval is processed alongside.
- Take treatment without paying for anything inside the approved package, and check the discharge summary before signing it.
How to check your eligibility and claim status
- Open govthealth.cg.gov.in/sna and use the Beneficiary Identification System to check whether your family and each member appear in the database.
- Use the hospital search on the same portal to confirm that a hospital is currently empanelled before you travel to it. Empanelment can lapse.
- Ask the Ayushman Mitra desk for the pre-authorisation number at admission, and quote it in any later query about the claim.
- Call 104 or 0771-4095198 to check a stuck pre-authorisation or a disputed bill.
Why claims get refused, and how to fix each one
- Name mismatch between ration card and Aadhaar. Correct the spelling at the food department or the Aadhaar centre; the BIS matches both records.
- Member missing from the ration card. A newborn or a newly married spouse not yet added to the ration card will not be covered. Add them at the food department first.
- e-KYC failure. Worn fingerprints are common among manual labourers and elderly patients. Use the OTP route with an Aadhaar-linked mobile number.
- Hospital not empanelled. Verify on the portal before admission. There is no reimbursement route for treatment taken at a non-empanelled hospital.
- Procedure outside the package list, for example cosmetic surgery or IVF.
- Cover already exhausted for the policy year across the family.
- Pre-authorisation not raised before discharge. Once the patient has paid and left, converting the bill to a cashless claim is extremely difficult, which is why the Ayushman Mitra desk must be approached before admission.

Frequently asked questions
How much treatment cover does Shaheed Veer Narayan Singh Ayushman Swasthya Yojana give?
Antyodaya, Priority and SECC-listed families get cashless treatment of up to Rs 5 lakh per family per year, and families holding other categories of ration card get up to Rs 50,000 per family per year. The cover is shared across every member named on the card, not given per person.
Is this the same scheme as Dr Khubchand Baghel Swasthya Sahayata Yojana?
Yes. Shaheed Veer Narayan Singh Ayushman Swasthya Yojana is the renamed version of Dr Khubchand Baghel Swasthya Sahayata Yojana (DKBSSY), renamed by the Chhattisgarh government in 2024. The entitlement, hospitals and existing cards remain valid, so a card issued under the old name still works.
Who is eligible for Shaheed Veer Narayan Singh Ayushman Swasthya Yojana?
Every Chhattisgarh family holding a valid ration card issued under the state Food Security Act is eligible, including families that do not appear in the central SECC list used for Ayushman Bharat PM-JAY. There is no separate income test beyond the ration card category.
Does the scheme cost anything?
No. Treatment at an empanelled hospital is cashless within the cover limit, and the Ayushman card is generated free of charge. A hospital that demands cash from a covered patient for a covered package is violating the scheme and should be reported on 104.
How many hospitals are empanelled?
As shown on the State Nodal Agency portal in August 2026, 1,678 hospitals were empanelled across Chhattisgarh, covering both government and private facilities. Use the hospital search on govthealth.cg.gov.in to find the nearest one before an admission.
What is not covered by the scheme?
Cosmetic surgery, routine outpatient consultations, IVF and fertility treatment, and AYUSH or traditional medicine systems are outside the scheme. Cover applies to in-hospital treatment under a defined package list, not to walk-in OPD visits or pharmacy purchases.
Can I use the card outside Chhattisgarh?
The scheme is integrated with Ayushman Bharat PM-JAY, so a beneficiary who appears in the PM-JAY database can generally use the card at PM-JAY hospitals in other states under portability. The purely state-funded component, including the Rs 50,000 cover for other ration card categories, is meant for use within Chhattisgarh — confirm with the hospital first.
What do I do if a hospital refuses cashless treatment?
Call the Chhattisgarh health helpline on 104 or the national Ayushman helpline on 14555 immediately, from the hospital itself if possible. Every empanelled hospital must have an Ayushman Mitra desk, and refusal of a covered package is a grievance the State Nodal Agency acts on.
