Birth Certificate in Kerala
Kerala state government scheme
Birth Certificate in Kerala is registered by the local body Registrar under the Local Self Government Department, and Kerala district administrations point citizens to the civil registration site cr.lsgkerala.gov.in for birth certificate services. Since 4 April 2025 the citizen services that ran on the ILGMS portal have moved to K-SMART at ksmart.lsgkerala.gov.in, and Akshaya centres remain the assisted counter.
What is a birth certificate in Kerala?
A birth certificate in Kerala is the certified extract from the register of births maintained under the Registration of Births and Deaths Act 1969, and Kerala places that register with the local body. The Secretary of the Grama Panchayat, Municipality or Corporation is the Registrar of Births and Deaths for that area, working under the Local Self Government Department. This is the structural difference between Kerala and most other Indian states, where birth registration sits with the Health department: in Kerala it is a local self-government function, which is why the portals carry the lsgkerala.gov.in domain.
District administrations in Kerala direct citizens to the civil registration site at cr.lsgkerala.gov.in for birth certificate services, and name the concerned Panchayat or municipal office as the physical location. Malappuram district's service page, for example, does exactly that. Alongside this, the Local Self Government Department has been consolidating its citizen-facing systems: the department has confirmed that services on the Citizen Service Portal (ILGMS) moved to the K-SMART portal at ksmart.lsgkerala.gov.in from 4 April 2025, and that Grama Panchayat services are no longer available on ILGMS. So a Kerala applicant today deals with the local body through K-SMART, and looks up or downloads civil registration records through the civil registration site.
Kerala's registration coverage is effectively complete, and the near-universal institutional delivery rate means most births are reported by the hospital itself. That makes the common Kerala problem not "how do I register" but "how do I find, correct or add a name to a record that already exists".
Who can and who cannot apply?
You can apply for birth registration or a birth certificate in Kerala if the birth took place inside the jurisdiction of a Kerala local body; any hospital, nursing home or residence within a Grama Panchayat, Municipality or Corporation area. The informant is normally the child's father or mother; for an institutional birth the medical officer in charge of the hospital reports the event. An adult whose own birth was never registered may be the informant for their own delayed registration.
You cannot apply to a Kerala local body for a birth that occurred outside Kerala. Registration follows the place of the event, so a birth in Karnataka, Tamil Nadu or abroad has to be registered by the authority for that place, and a Kerala Panchayat will not create a fresh entry for it. A birth to Keralite parents abroad is registered with the Indian Mission concerned, not with the home Panchayat. You are also not eligible to file a second registration where a record already exists. The correct route for a wrong or missing detail is correction or name inclusion in the existing entry.
No income, caste or residence test applies. Birth registration is a statutory duty attached to the event, and it is the same for every family in the state.
What documents are required?
For an ordinary institutional birth, the Kerala hospital reports the event and the parents mainly supply identity details:
- Hospital birth record or discharge summary carrying the delivery details.
- Identity proof of both parents, most commonly Aadhaar, with the names spelled as they should appear in the register.
- Address proof establishing which Grama Panchayat, Municipality or Corporation has jurisdiction.
For a home birth, the informant reports the event directly to the local body Registrar with a certificate from the local authority for the area confirming that the birth occurred there.
For delayed registration and for corrections, add a declaration or affidavit from the parent stating the true date, place and parentage, and; where the birth is more than one year old, the order of the prescribed authority under section 13(3) of the Act.
Get the spelling right at the registration stage. Kerala names transliterate several ways between Malayalam and English, and a mismatch between the birth register, the Aadhaar record and the school record creates work at every later stage: passport, PSC application, professional registration.
How to apply for a birth certificate in Kerala
- Identify the local body that holds the register. It is the Grama Panchayat, Municipality or Corporation in whose area the birth took place, not where the family lives now. For a hospital birth, that is the local body covering the hospital's address.
- Search for the existing record online on the civil registration site at cr.lsgkerala.gov.in, which Kerala district administrations name for birth certificate services. Search with the date of birth, the local body and the parents' or child's name to confirm the entry exists.
- Apply through the local body's online service. Since 4 April 2025 the Local Self Government Department's citizen services run on K-SMART at ksmart.lsgkerala.gov.in, after the ILGMS Citizen Service Portal was retired. Register on K-SMART, select the concerned local body and apply for the birth certificate service.
- Upload the identity and supporting documents the service asks for, and pay the fee displayed on the payment screen.
- Or apply at the counter. Take the hospital record and the parents' identity documents to the Grama Panchayat, Municipality or Corporation office, or use an Akshaya centre, Kerala's assisted service counter network, whose service charge is additional to any government fee.
- Collect or download the certificate. Save the application or acknowledgement number. It is how you track the file and retrieve the certificate later.
How do you register a birth reported late in Kerala?
The 1969 Act applies in Kerala in the same graded way as everywhere in India:
- Within 21 days of the birth: report it to the local body Registrar and the entry is made with no registration fee. This is the free, unconditional window, and it is why Kerala hospitals complete the paperwork before discharge.
- After 21 days and up to 30 days, the Registrar may register on payment of the prescribed late fee.
- After 30 days and within one year — registration requires the written permission of the prescribed authority together with a declaration from the informant.
- After one year, the Registrar cannot make the entry on their own authority at all. An order under section 13(3) of the Act, from the authority prescribed for the purpose, is required first.
The one-year line is the one that catches families out. Someone discovering at 40 that their birth was never registered, because a school certificate carried them through life until a passport or pension file demanded a birth certificate, is looking at the section 13(3) route and a wait measured in months. Start it as early as you can, and gather the oldest documentary evidence of the date of birth you can find. The school admission register extract, the hospital record, the baptism or equivalent record; because that is what the prescribed authority weighs.
Why do Kerala birth certificate applications run into trouble?
- The wrong local body is selected, so the search returns no record even though the entry exists.
- The English transliteration of the name in the register differs from the Aadhaar or school record.
- The birth was registered before the child was named, so the certificate reads "male child" or "female child" and needs a name-inclusion application, not a new registration.
- The date of birth on the hospital record and on the application do not agree.
- A birth more than one year old is filed as an ordinary registration instead of going through the section 13(3) route.
- A second registration has been created at some point, leaving two conflicting entries that now have to be reconciled.
What does it cost, and what about agents?
Reporting a birth within 21 days carries no registration fee in Kerala, as everywhere in India. Beyond that, the only legitimate charges are the prescribed late fee where one is due, the local body's published fee for a certified copy, and, if you use an assisted counter, the Akshaya centre's own service charge, which is separate from and additional to the government fee. Every one of those amounts is displayed to you before payment on the portal, or published at the counter.
No agent or middleman can obtain a birth certificate faster than the Registrar's own process. Every application enters the same local body workflow, and no payment produces an entry the Registrar has not made. If someone offers to "get it done" for several thousand rupees, they are charging you for filling a form you can file yourself or have filed at an Akshaya centre for a published fee. Insist on the receipt, and take a demand for extra money to the Secretary of the local body and the District Registrar.
Where do you go if nothing moves?
Escalate through the civil registration hierarchy: the local body Registrar (the Panchayat, Municipality or Corporation Secretary), then the District Registrar of Births and Deaths, then the Chief Registrar for Kerala in the Local Self Government Department. For portal problems, a login that will not work, a record that will not display, a certificate that will not download: use the support channels published on K-SMART and the civil registration site, or ask the local body office to look up the entry directly from the register. Where the entry exists but a detail is wrong, apply for a correction or name inclusion in the same register, and never file a fresh registration, because two entries for one birth are far harder to unwind than a single wrong one.

Frequently asked questions
Which department issues birth certificates in Kerala?
The Local Self Government Department issues birth certificates in Kerala through the local body — the Secretary of the Grama Panchayat, Municipality or Corporation acts as the Registrar of Births and Deaths for that area. Kerala district administrations, such as Malappuram, list the concerned Panchayat or municipal office as the location for the service.
Which Kerala portal do I use for a birth certificate?
Kerala district websites direct citizens to cr.lsgkerala.gov.in for birth certificate services, which is the civil registration site of the Local Self Government Department. Separately, the local body citizen services that used to run on the ILGMS Citizen Service Portal moved to the K-SMART portal at ksmart.lsgkerala.gov.in from 4 April 2025.
What is K-SMART and does it affect birth certificates?
K-SMART is the Kerala Local Self Government Department's unified local body services platform at ksmart.lsgkerala.gov.in, and the department has confirmed that Citizen Service Portal (ILGMS) services moved to it after 4 April 2025. Since birth registration is a local body function, K-SMART is the platform through which Grama Panchayat services are now delivered.
How many days do I have to register a birth in Kerala?
21 days from the date of birth is the reporting window under the Registration of Births and Deaths Act 1969, and a birth reported within that window carries no registration fee. Kerala hospitals normally report institutional births to the local body Registrar themselves inside this period.
How do I register a birth in Kerala that happened years ago?
Approach the Registrar at the Grama Panchayat, Municipality or Corporation where the birth took place and start delayed registration. A birth more than one year old cannot be registered on the Registrar's own authority and requires an order under section 13(3) of the Registration of Births and Deaths Act 1969.
Can I get a Kerala birth certificate from an Akshaya centre?
Yes — Akshaya centres are Kerala's assisted service counters and can file local body applications for citizens who cannot complete the online steps themselves. Akshaya service charges are separate from and additional to any government fee.
My name is missing from my Kerala birth certificate — what do I do?
Apply to the same local body Registrar for name inclusion in the birth register, which is a standard service where the birth was registered before the parents chose a name. Do not file a fresh registration, because a second entry for the same birth causes far more difficulty later than the missing name.
Does a Kerala birth certificate expire?
No, a birth certificate never expires, because it records a fixed past event. When the paper copy is lost, ask the same local body for another certified copy from the register or download it from the civil registration site rather than starting again.
