Financial Assistance for Medical Expenses on Major Ailments (OB&OCWW Board), Odisha
Odisha state government scheme
Financial Assistance for Medical Expenses under the Odisha Building and Other Construction Workers' Welfare Board covers up to Rs 3,00,000 for treatment of brain, heart, kidney, cancer, insulin-dependent diabetes and thalassemia or sickle cell disease, for workers registered at least one year and their dependants. Claims go in Form XXXIV to the District Labour Officer.
What is the major ailments medical assistance scheme in Odisha?
Financial Assistance for Medical Expenses is the health benefit of the Odisha Building and Other Construction Workers' Welfare Board, which sits under the Labour & ESI Department, Government of Odisha. Under Rule 276 of the Odisha Building and Other Construction Workers' (RE&CS) Rules, 2002, the Board may sanction up to Rs 3,00,000 towards the treatment of listed major ailments for a registered construction worker and his or her dependent family members.
Rule 276 was substituted by Gazette Notification No. 933 of 30 May 2019 (S.R.O. No. 187/2019). Before that, the Labour & ESI Department Resolution No. 6096 of 11 August 2016 had already raised the assistance to Rs 3 lakh for beneficiaries suffering from diseases of the nervous system (brain and spinal), the cardiovascular system, kidney and cancer, expressly "in lines of Odisha State Treatment Fund". The current rule adds insulin-dependent diabetes and thalassemia-cum-sickle cell disease to that list.
The design is cashless. The rule states that the beneficiary shall avail cashless medical treatment and that the cost of treatment shall be reimbursed to the Odisha State Treatment Fund as per government guidelines. In practice districts also process reimbursement claims in Form XXXIV where a worker has already paid, which is the route described in the Board's operating circulars.

- You have been registered for not less than one year.
- Your Rs 50 annual contribution is paid up to date and you hold the money receipt.
- The illness is one of the listed major ailments, brain, heart, kidney, cancer, insulin-dependent diabetes, or thalassemia-cum-sickle cell.

- Treatment was taken at a Government hospital, or elsewhere with a referral or a non-availability certificate from the nearby Government hospital.
You cannot apply, or will not be eligible, if:

How to apply for major ailments assistance in Odisha
- Get the essentiality certificate filled in completely and signed by a Government doctor not below the rank of Assistant Surgeon while the patient is still under treatment: obtaining it later is much harder.
- Collect the original bills, the discharge certificate or OPD report, and a detailed list of medicines, laboratory tests and investigations, and have each bill verified, signed and sealed by the treating doctor.
- Fill in Form No. XXXIV, downloadable from the Board's Downloads page as the Major Ailments Form, and attach the attested registration certificate copy, the annual contribution receipt, the referral form, identity proof and the first page of your bank passbook.
- Submit the complete file within 3 months of discharge to the authority under the administrative control of the District Labour Officer of the district where you reside, and obtain a dated acknowledgement.
- Attend the verification by the district committee headed by the Chief District Medical Officer, taking all original documents with you.
- Track the sanction with the DLO, and check that the amount reaches the bank account shown in the passbook copy you filed.
The Board's beneficiary portal at nirmanshramik.odisha.gov.in is used alongside this route. Log in through Construction Worker Login to confirm your registration is active, to Pay Annual Contribution if it has lapsed, and to follow Application Tracking once a claim has been entered.
How the district committee assesses the claim
Letter No. 142(32) dated 28 January 2017, issued by the Labour Commissioner-cum-Member Secretary, sets out the standing procedure. The District Collector constitutes a committee headed by the Chief District Medical Officer with one Surgery Specialist, one Medicine Specialist and the District Labour Officer. The committee compares the application and appended documents against the originals held by the applicant, which is why the applicant must appear with originals rather than send photocopies alone.
The same circular sets out practical requirements that decide most claims: every signature must be supported by seal and designation, all photocopies must be attested by the DLO, ALO or RLI of the district, an abstract of medicine bills must be attached, and the enquiring officer's report and a checklist must accompany the file. Where original documents have been lost, photocopies signed by the medical authority are entertained only against an affidavit.
How much is paid, and how does it interact with other schemes?
The ceiling is Rs 3,00,000 per case. The amount is not a flat grant, it is assistance towards actual, documented medical expenditure, so the sanction can be lower than the ceiling depending on the verified bills.
Rule 276 links the scheme to the Odisha State Treatment Fund, under which the worker takes cashless treatment and the cost is reimbursed to that fund under government guidelines. Workers already protected by Biju Swasthya Kalyan Yojana or a similar scheme are excluded, so that the same treatment is not paid for twice. Historically the Board also extended medical cover through RSBY and BKKY, which are still listed among the Board's benefits on its portal.
Accident cases follow a parallel track. Letter No. 138(32) of 28 January 2017 prescribes the same Form XXXIV, the same 3-month window from discharge and the same CDMO committee for accidental benefit cases, with additional papers such as the police FIR where applicable. Permanent disablement is compensated separately under Rule 270 — Rs 80,000 for permanent total disablement from injuries other than those specifically scheduled, and a percentage of Rs 1,00,000 for scheduled partial disablements such as loss of hearing in both ears at 50%.
Why major ailment claims get rejected
- Essentiality certificate missing or incomplete. The commonest single defect.
- Bills not verified by the treating doctor, or without seal on the face of the bill.
- Claim filed after 3 months from the date of discharge for indoor treatment.
- No referral or non-availability certificate from the nearby Government hospital.
- Contribution in arrears, or membership already ceased.
- Disease not on the Rule 276 list.
- Already covered under Biju Swasthya Kalyan Yojana or a similar scheme.
- Tampered documents, which the Board's circular says will lead to rejection outright.

Frequently asked questions
How much medical assistance does the Odisha BOCW Board give for major ailments?
The Board may sanction financial assistance of up to Rs 3,00,000 towards medical expenses for major ailments. Rule 276 of the Odisha BOCW Rules, as substituted by the 2019 amendment rules, sets this ceiling and requires the beneficiary to have been registered for not less than one year.
Which diseases are covered as major ailments?
Rule 276 lists diseases of the brain, heart and kidney, cancer, insulin-dependent diabetes, and thalassemia-cum-sickle cell disease. The 2016 resolution described the same group as nervous system (brain and spinal), cardiovascular system, kidney and cancer, on the lines of the Odisha State Treatment Fund.
Can family members of a construction worker use this benefit?
Yes. Rule 276 extends the assistance to the registered beneficiary and to his or her dependent family members, provided the worker has completed one year of registration.
Is there a time limit for filing the claim?
Yes — for indoor admission the claim must be preferred within 3 months from the date of discharge from the hospital, as stated in Letter No. 142(32) dated 28.01.2017 of the Labour Commissioner-cum-Member Secretary.
Who verifies the medical claim?
A district committee headed by the Chief District Medical Officer verifies it. The District Collector constitutes the committee with the CDMO, one Surgery Specialist, one Medicine Specialist and the District Labour Officer, who check the application and appended documents against the originals.
Can I claim this if I am covered under Biju Swasthya Kalyan Yojana?
No. Rule 276 states that beneficiaries covered under Biju Swasthya Kalyan Yojana or a similar scheme cannot avail this benefit; the assistance is designed to be cashless through the Odisha State Treatment Fund route rather than duplicated.
What happens if I only had temporary disability from an accident?
In case of temporary disability the construction worker is eligible only for medical treatment, not for a disablement award. Permanent disablement is dealt with separately under Rule 270, which fixes Rs 80,000 for permanent total disablement from injuries other than those specifically listed.
What is the most common reason a major ailment claim is returned?
Incomplete documents. The Board's own circular of 28 January 2017 returned every pending application to the districts because applicants had not furnished the minimum pre-requisite documents, chiefly the essentiality certificate and doctor-verified original bills.
