Gopabandhu Jana Arogya Yojana (GJAY)
Odisha state government scheme
Gopabandhu Jana Arogya Yojana is Odisha's universal health assurance scheme. GJAY makes treatment free for every patient at all state government health facilities, and gives over 96.5 lakh economically vulnerable families cashless cover of Rs 5 lakh a year at empanelled private hospitals, with an extra Rs 5 lakh for women members.
What is Gopabandhu Jana Arogya Yojana?
Gopabandhu Jana Arogya Yojana (GJAY) is the universal health assurance scheme of the Government of Odisha, run by the Health & Family Welfare Department through the State Health Assurance Society (SHAS) in Bhubaneswar. GJAY is the scheme Odisha readers often search for as "Gopabandhu Jan Arogya Yojana" or by the name of the programme it continues, Biju Swasthya Kalyan Yojana, and many of the official document links still carry the older bsky.odisha.gov.in address.
Gopabandhu Jana Arogya Yojana is built around two distinct components, and confusing them is the single most common mistake readers make.
Component one; free care at government facilities, for everyone. According to the official description on gjay.odisha.gov.in, the State Government bears the full cost of all health services delivered to all patients irrespective of income, status or residence, in every state government health care facility from sub-centre level up to District Headquarters Hospital, Government Medical College Hospital and blood bank level. This covers drugs, diagnostics, dialysis, cancer chemotherapy, operation theatre charges, ICU, in-patient admission and blood bank services. There is no card, no income test and no annual ceiling on this component.
Component two, cashless private-hospital cover for vulnerable families. The State Government bears the cost of treatment at empanelled private hospitals for over 96.5 lakh economically vulnerable families, amounting to annual health coverage of Rs 5 lakh per family, with an additional Rs 5 lakh for the women members of the family once the initial limit is exhausted. This is the component that needs a card.
GJAY is now delivered on an Ayushman Bharat PM-JAY co-branded card, referred to on the portal as the AB PMJAY-GJAY card, alongside the older GJAY Smart Health Card. The card is portable: a holder family can take cashless treatment at any empanelled private hospital under GJAY, within Odisha or outside it.
Who is eligible for Gopabandhu Jana Arogya Yojana?
You are covered at government facilities if:
- You are a patient at any Odisha state government health facility. Coverage here does not depend on income, social status or residence, so visitors and people from outside the state are included.
Your family can get the private-hospital cover if:
- Your family is on the State Government's list of economically vulnerable families, the group of over 96.5 lakh families identified for GJAY.
- Your family record exists in the State Health Assurance Society database and an AB PMJAY-GJAY card has been generated against it.
- Your Aadhaar-based e-KYC for the individual member is complete.
You are not eligible for the private-hospital component if:
- Your family is not on the State Government's beneficiary list. Families outside that list cannot apply to be added by filling in a form of their own; identification is done by the State Government, and a family that believes it has been wrongly left out must take the matter to the State Health Assurance Society or the district office rather than to a hospital.
- You are already drawing benefits under another government-funded health scheme, run by another State Government or by the Central Government, and you are not willing to forego those benefits. In this situation your card is put on hold rather than cancelled, and it stays on hold until you submit the self-declaration described below.
- You are seeking treatment at a private hospital that is not empanelled under GJAY. Non-empanelled hospitals cannot raise a cashless claim, and bills paid there are not reimbursed.
What documents are required for GJAY?

| AB PMJAY-GJAY card or GJAY Smart Health Card | Yes | The single document that unlocks cashless private-hospital treatment |
| Aadhaar card | Yes | Used for e-KYC and member identification |

How to apply for GJAY and get your health card
Gopabandhu Jana Arogya Yojana has no citizen application form. Eligible families are identified by the State Government, so the practical task is to locate your family record and download the card.
- Open the official GJAY portal at gjay.odisha.gov.in and choose the ABPMJAY section, which carries the "Know Your ABPMJAY - GJAY Health Card Number" link.
- Follow that link to the National Health Authority beneficiary portal at beneficiary.nha.gov.in and sign in as a beneficiary using your mobile number and the OTP sent to it.
- Select Odisha as the state and the AB PMJAY-GJAY scheme, then search for your family using your family identification details, ration card number or Aadhaar number.
- Locate your name in the family list and complete Aadhaar-based e-KYC for yourself and for each family member who does not yet show as verified.
- Once the record shows as approved, download the AB PMJAY-GJAY card and note the PMJAY ID and URN number printed on it.
Offline route. If you cannot do this yourself, go to the Swasthya Mitra desk at any empanelled private hospital in Odisha. Swasthya Mitras are placed at empanelled private hospitals specifically to help GJAY beneficiaries, and the GJAY portal publishes their names, hospital, contact number and email under "Who is Who". A Common Service Centre from the CSC list published on the GJAY portal can also do the search, the e-KYC and the card download for you.
How to use GJAY at a hospital
- At a government facility, simply seek treatment. No card and no pre-approval are needed, because the State Government meets the entire cost.
- At an empanelled private hospital, present the AB PMJAY-GJAY card at the Swasthya Mitra desk before admission.
- The hospital raises a pre-authorisation request against your family's balance through the state Transaction Management System. The pre-authorisation form is published on the GJAY portal under Citizen Information.
- Once the pre-authorisation is approved, registration, consultation, medical tests, pathology, treatment, in-patient care and follow-up consultation are all cashless up to the annual coverage amount.
If a government facility cannot provide the required treatment, it issues a referral using the referral form published on the GJAY portal.
How to check your GJAY card balance and status
Gopabandhu Jana Arogya Yojana publishes a card-details view that shows, for the member ID you enter: Total Family Fund, Total Female Fund, Available Balance, Amount Blocked, Claimed Amount, Policy Start Date, Policy End Date, PMJAY ID, URN No., gender, Aadhaar number and address.
Read those fields carefully before an admission. Available Balance is what is left of the Rs 5 lakh family limit for the current policy year. Female Fund is the separate additional Rs 5 lakh available to women members after the family limit is exhausted. Amount Blocked is money already reserved against a pre-authorisation that has not been settled, so it is not spendable on a second treatment. If the screen returns "You are not a GJAY beneficiary", your family record is not on the state list.
The GJAY Citizen Dashboard at bskydashboard.odisha.gov.in and the GJAY chatbot on the main portal both answer card and hospital queries.
Why GJAY cards get rejected or put on hold
- Card on hold because of an Aadhaar clash. This is the most common problem since the AB PMJAY co-branding. Your Aadhaar has been found linked to another government-funded health scheme in another state or under a central programme. The fix is the two-page self-declaration form addressed to the State Health Assurance Society, Bhubaneswar, in which you state your full name, URN (GJAY/PMJAY) which is mandatory, Aadhaar number, mobile number and residential address, and declare that you forego benefits under the other scheme. The form warns that the choice is final and binding, and that a false declaration can lead to cancellation of eligibility.
- e-KYC not completed for the individual member, even though the family is listed.
- Name or Aadhaar mismatch between the family record and the member's Aadhaar.
- Family not on the state beneficiary list, in which case the portal returns "You are not a GJAY beneficiary".
- Hospital not empanelled, so no cashless claim can be raised.
- Balance exhausted for the policy year, or blocked against a pending pre-authorisation.
Key dates and documents published by SHAS
Gopabandhu Jana Arogya Yojana has no application deadline β the government facility component is available whenever you need treatment, and the card can be downloaded at any time. The card itself carries a Policy Start Date and Policy End Date, and the Rs 5 lakh limit resets with each policy year.
The State Health Assurance Society publishes operational documents on the GJAY portal, including the ABPMJAY_GJAY Health Benefit Package list and the CSC list (both dated 11 April 2025), the ABPMJAY empanelled hospital list (12 April 2025), and office orders naming District Coordinators, Additional District Coordinators and Nodal Officers (April 2026). Check the empanelled hospital PDF before you travel for treatment, because empanelment and the specialities each hospital is empanelled for both change.
