Mukh Mantri Sehat Yojana, Punjab (formerly Ayushman Bharat – Mukh Mantri Sehat Bima Yojana)
Punjab state government scheme
Mukh Mantri Sehat Yojana is Punjab's health assurance scheme, launched on 22 January 2026, giving cashless treatment of up to Rs 10 lakh per family per year to all 3 crore residents of the state with no income cap. It replaces the earlier Rs 5 lakh Ayushman Bharat – Mukh Mantri Sehat Bima Yojana. Enrolment is free at a Common Service Centre.
What is Mukh Mantri Sehat Yojana in Punjab?
Mukh Mantri Sehat Yojana (MMSY) is Punjab's health assurance scheme, run by the State Health Agency under the Department of Health and Family Welfare, Government of Punjab. The State Health Agency describes it as a historic step toward Universal Health Coverage for the state, aiming to provide cashless treatment of up to Rs 10 lakh per family per year to all 3 crore residents of Punjab.
MMSY is the successor to Ayushman Bharat – Mukh Mantri Sehat Bima Yojana (AB-MMSBY), the converged scheme under which Punjab delivered Ayushman Bharat PM-JAY along with a state extension from 2019 onwards. The State Health Agency notes that MMSY "significantly expands health protection from the earlier Rs 5 lakh coverage which was limited to specific categories". The MMSY scheme is recorded on the agency's site as dated Thursday, 22 January 2026.
Two features define the new scheme:
- No income cap and no exclusion criteria. MMSY includes all bona fide residents of Punjab, and the State Health Agency specifically names government employees and pensioners as included.
- Health Benefit Package 2.2, the latest national package list, under which more than 2,000 treatment packages have been selected for the scheme, with higher treatment rates than the older package set.
The scale is already substantial. The State Health Agency's dashboard puts the target at more than 65 lakh families, with approximately 39.50 lakh families enrolled, approximately 12.58 lakh treatments taken since 2019, around Rs 1,299 crore of treatment amount, and 900-plus empanelled hospitals.
Who is eligible for the Punjab Sehat Card?
You can enrol if:
- You are a bona fide resident of Punjab. Residence is the single substantive test.
- You can produce Aadhaar and Voter ID at the Common Service Centre, which are the documents the State Health Agency names for enrolment.
- You are enrolling as part of a family, since the Rs 10 lakh limit is a family limit for the year. The agency publishes both a family declaration format and a single-member declaration form for households that do not fit the standard family record.
Government employees and pensioners are explicitly inside the scheme. So are households that were never covered under the earlier AB-MMSBY category list, which is the practical change most Punjab families will notice.
You are not eligible, and cannot apply, if:
- You are not a bona fide resident of Punjab. A non-resident cannot enrol, even with Punjabi family ties.
- You cannot produce identity documents for the members being enrolled.
There is deliberately no income test, no BPL condition, no occupational category and no exclusion list. Where a scheme is universal, the usual eligibility questions collapse into a residence question.

How to apply for the Punjab Sehat Card
- Wait for the Sehat Mitra visit. A government representative called a Sehat Mitra comes to your home and gives an appointment slip for registration, carrying the date on which you should go to the Common Service Centre.
- Visit your nearest Common Service Centre on the date mentioned in the appointment slip, with the necessary documents.
- Carry documents for all family members. The beneficiary, the members and the family should be residents of Punjab and must carry valid identity documents; Aadhaar and Voter ID are the ones the agency names.
- Complete the declaration using the MMSY family declaration format, or the single-member declaration form where only one member is being enrolled. Both are downloadable from sha.punjab.gov.in under Resources.
- Collect the SMS confirmation. The agency states that a confirmation message of registration and benefits arrives immediately.
- Collect the Sehat Card within 2-3 weeks at your nearest Common Service Centre, or download it online through the "Download Health Card" link on sha.punjab.gov.in.
Families that do not receive a Sehat Mitra visit can still approach a Common Service Centre directly; the agency's Citizen Corner publishes a location finder for ecard generation centres.
How do I use the Punjab Sehat Card at a hospital?
- Visit any empanelled government or private hospital in Punjab. The list of empanelled hospitals is published on sha.punjab.gov.in and on the national hospital empanelment portal at hem.nha.gov.in.
- Present your Mukh Mantri Sehat Yojana Sehat Card at the hospital's Ayushman or Arogya Mitra desk.
- Let the hospital verify your details digitally. The verification runs against the beneficiary database rather than on paper.
- Take cashless treatment up to your eligible limit. No money changes hands for a covered package; the hospital raises the claim with the State Health Agency.
The cover includes pre- and post-hospitalisation expenses and hospitalisation costs, and extends to secondary and tertiary care in both public and private empanelled hospitals.
How do I check my Mukh Mantri Sehat Yojana eligibility and card status?
The State Health Agency's Citizen Corner carries four self-service checks:
- Check for eligibility in scheme: confirms whether a person or family is on the beneficiary list.
- Check active or inactive status of ecard, the most useful check before a planned hospitalisation.
- Check for disabled AB-SSBY ecard — for older cards issued under the previous scheme that have been disabled.
- Location for ecard generation (CSC), finds the nearest centre that can generate or reissue a card.
A card showing as inactive is the commonest reason a family is asked to pay at the counter. Fixing it at a CSC before admission avoids an argument at the hospital desk.
Why do Punjab Sehat Card claims get refused?
- Card inactive or not generated. Enrolment is not the same as a live card; check active status before admission.
- Hospital not empanelled. Cashless treatment works only at empanelled government and private hospitals. Verify on the agency site or hem.nha.gov.in before choosing a hospital.
- Treatment outside the package list. MMSY runs on Health Benefit Package 2.2 with more than 2,000 selected packages; a procedure outside those packages is not payable.
- Identity mismatch at the hospital's digital verification step, usually where the name or Aadhaar detail on the card differs from the document produced.
- Family member not enrolled, where only some members were registered at the CSC. The Rs 10 lakh limit belongs to the family record, and an unlisted member cannot draw against it.
- Old AB-SSBY card being used instead of the MMSY Sehat Card. The disabled ecard check on the Citizen Corner exists precisely for these cases.
Where do I complain about Mukh Mantri Sehat Yojana?
- Helpline 104. The State Health Agency publishes 104 as the grievance support and helpline number for the scheme, and the Department of Health and Family Welfare separately publishes information on the medical helpline 104.
- Grievance portal of the National Health Authority, linked from sha.punjab.gov.in as "Grievance – GoI", for claim and hospital-level grievances.
- State Health Agency, Punjab, 4th Floor, BSNL Circle Office, Sector-34A, Chandigarh, the scheme's administrative office.
- Arogya Mitra desk at the empanelled hospital, for refusal of cashless treatment at the point of admission. Raising it there, before paying, is far more effective than seeking reimbursement later.
Enrolment, the Sehat Card and treatment under a covered package are all free. No Common Service Centre operator or hospital may charge a fee for generating a Sehat Card.
