Death Certificate in Uttarakhand
Uttarakhand state government scheme
Death Certificate in Uttarakhand is registered and issued by the Registrar of Births and Deaths of the local body where the death occurred, the Gram Panchayat in rural areas and the Nagar Nigam or Nagar Palika in towns. Uttarakhand citizens apply online through the state's Apuni Sarkar e-Services and e-District portals or the national CRS portal, and every death must be reported within 21 days under the Registration of Births and Deaths Act 1969.
What is a death certificate in Uttarakhand?
A death certificate in Uttarakhand is the certified extract of the death register maintained by the Registrar of Births and Deaths of the local body in whose area the death occurred. Uttarakhand issues it under the Registration of Births and Deaths Act 1969, a central law the state administers through its Gram Panchayats and urban local bodies.
Two registrar networks operate side by side in the state. In rural Uttarakhand, which is most of Chamoli, Pithoragarh, Uttarkashi, Rudraprayag, Bageshwar, Champawat, Almora and Tehri Garhwal, the Gram Panchayat holds the register. In the towns and cities, the Nagar Panchayat, Nagar Palika Parishad or Nagar Nigam does: Dehradun, Haridwar, Haldwani, Rudrapur, Kashipur, Roorkee, Rishikesh, Kotdwar and Pithoragarh town each register deaths through their own municipal registrar.
Uttarakhand delivers its online citizen services through two related state platforms, the Apuni Sarkar e-Services portal at eservices.uk.gov.in and the e-District portal at edistrict.uk.gov.in, run for each district by the District e-Governance Society with the state's IT Development Agency. Certificate services in districts such as Chamoli are explicitly routed through the District e-Governance Society and the e-District portal, and Common Service Centres act as the filing counter for households far from a block headquarters.
Since 1 October 2023, the Registration of Births and Deaths (Amendment) Act 2023 has applied in Uttarakhand. It makes the Medical Certificate of Cause of Death compulsory from every medical institution, with a copy to the nearest relative, and it opens the national Civil Registration System portal at dc.crsorgi.gov.in for reporting a death, with the certificate delivered electronically to the informant's registered email and mobile number.
Who can and who cannot apply?
You can apply for an Uttarakhand death certificate if the death occurred inside Uttarakhand and you are one of the persons the Registration of Births and Deaths Act 1969 treats as an informant or an interested party:
- The nearest relative, spouse, son, daughter, father, mother, brother or sister of the deceased.
- The head of the household in which the death occurred.
- The medical officer in charge of the hospital, base hospital, CHC or nursing home where the death took place, who reports it directly.
- The jail superintendent for a death in prison, and the officer in charge for a death in a hostel, ashram, dharamshala or public institution, a live category in Uttarakhand, given the number of pilgrims and trekkers in the state.
- The village headman or the officer in charge, for an unclaimed or abandoned body.
- Any person who later needs a certified copy for a bank, insurer, pension office, property or succession matter, on payment of the per-copy fee.
You cannot apply in Uttarakhand for a death that occurred outside the state. The register is territorial, so an Uttarakhand family whose relative died in Delhi or Uttar Pradesh must obtain the certificate from the local body there. Conversely, and this catches many families, a pilgrim or tourist from another state who dies in Uttarakhand is registered here, by the Gram Panchayat or municipality of the place of death, and the home state cannot issue that certificate.
You are also not eligible for a fresh registration where the death is already entered in the register; the correct request there is a certified copy or a correction. A registrar will hold the file where the death is under police investigation and the post-mortem cause of death has not been released, and where the applicant can show no relationship to or interest in the deceased.
Uttarakhand applies no income limit, no caste condition and no domicile test to a death certificate.
What documents are required?
- Report of Death in Form 2, the statutory report signed by the informant.
- Medical Certificate of Cause of Death from the hospital or attending doctor, now compulsory from medical institutions under the 2023 amendment.
- Cremation or burial receipt, for a death at home with no hospital record, the ordinary situation in hill villages.
- Identity proof of the deceased; Aadhaar, PAN, voter ID, ration card or passport.
- Identity proof of the applicant, with proof of the relationship where the applicant is not the household head.
- Proof of the deceased's address at the time of death — ration card, electricity bill or house tax receipt; to fix which panchayat or ward holds jurisdiction.
- Affidavit stating the date, time and place of death, for a home death with no doctor and for any delayed registration.
- Order under Section 13(3), only where the death is more than one year old.
How to apply for a death certificate in Uttarakhand online
- Confirm the death has been reported within 21 days. A hospital reports it directly and gives the family the Medical Certificate of Cause of Death; for a death at home, the household head must report it to the Gram Panchayat or municipal registrar.
- Register on the state portal. Open the Apuni Sarkar e-Services portal at eservices.uk.gov.in or the e-District portal at edistrict.uk.gov.in and create a citizen login with your mobile number, verifying the OTP.
- Open the death certificate service and select the district, block or municipality where the death occurred. Jurisdiction follows the place of death, not the family's home address.
- Enter the death details; full name of the deceased, date and place of death, age at death, father's or husband's name, permanent address, and the name and relationship of the informant. Spellings must match the hospital record, because the registrar compares them with the register entry.
- Upload the supporting papers, the Medical Certificate of Cause of Death or the cremation receipt, identity proof of the deceased and your own identity proof: each within the file size limit the upload screen states.
- Pay the fee shown on the payment screen and save the application number printed on the acknowledgement. Order every copy you will need in the same application.
How to apply offline at the panchayat or municipal office
- Go to the office that holds the register, the Gram Panchayat office for a village death, or the Nagar Panchayat, Nagar Palika Parishad or Nagar Nigam office for a town death.
- Submit the Report of Death in Form 2 with the Medical Certificate of Cause of Death or the cremation or burial receipt, plus identity proof of the deceased and of yourself.
- Pay the counter fee and take the receipt. A report inside 21 days carries no penalty; beyond that the registrar quotes the late fee under the state's rules before accepting the file.
- Collect the certified copies from the same counter. In the hill districts, a Common Service Centre operator can file the same application and hand over the printed copies, which saves a trip to the block headquarters.
How do you check the application status?
Track the application with the application number on the same portal you filed it on. The Apuni Sarkar e-Services portal or e-District Uttarakhand; or ask the Gram Panchayat or municipal registrar directly. Where the death was reported on dc.crsorgi.gov.in, the national portal carries the registration number and emails the certificate to the informant's registered address once the registrar approves the entry. The District e-Governance Society at the collectorate of your district is the escalation point when a file sits without movement; for Chamoli, for example, the district administration office at Gopeshwar handles e-District service queries.
Why are death certificate applications rejected in Uttarakhand?
- The death was never reported in Form 2, so there is no register entry to certify.
- The application was filed with the wrong panchayat or municipality, jurisdiction is the place of death.
- The name, age or father's name on the form does not match the hospital record or the deceased's identity proof.
- No Medical Certificate of Cause of Death and no cremation or burial receipt was attached for a death at home.
- The death is more than one year old and no order under Section 13(3) accompanies the file.
- The death is under police investigation, common for accidental deaths on trekking routes and highways, and the cause of death is not yet finalised.
- Uploaded scans are unreadable or attached to the wrong slot.
What does it cost, and what about agents?
Uttarakhand issues the death certificate at the government fee displayed on the portal payment screen or quoted at the panchayat or municipal counter, plus the notified Common Service Centre charge if you file through an operator. Reporting the death inside the 21-day window carries no penalty at all, and additional certified copies are charged at the same published per-copy rate.
No clerk, panchayat secretary, ward officer or middleman is entitled to anything beyond that. Agents around hospitals in Dehradun, Haridwar and Haldwani offer "quick" death certificates for several thousand rupees; they file the same application on the same portal, and the file still waits for the same registrar. Keep the online receipt, and take a demand for money to the District Magistrate's grievance cell or the Municipal Commissioner.
What do you need the death certificate for in Uttarakhand?
An Uttarakhand death certificate is the document every downstream office starts from: closing bank and post office accounts, claiming life insurance and EPFO or state pension dues, transferring the LPG connection and electricity meter, mutating agricultural land in the khatauni and urban property in the municipal record, removing the name from the ration card and the electoral roll, claiming disaster or accident ex-gratia where the death was caused by a landslide, flood or road accident, and applying for a legal heir certificate from the Tehsildar or a succession certificate from the civil court. Each of those offices retains a copy permanently, which is why families order five or six certified copies in a single application.

Frequently asked questions
Who issues a death certificate in Uttarakhand?
The Registrar of Births and Deaths of the local body where the death took place issues the death certificate in Uttarakhand — the Gram Panchayat registrar in a village, and the Nagar Panchayat, Nagar Palika Parishad or Nagar Nigam registrar in Dehradun, Haridwar, Haldwani, Rudrapur, Kashipur, Roorkee and other towns. The Tehsildar and the Revenue Department do not issue it.
Can I apply for an Uttarakhand death certificate online?
Yes, Uttarakhand delivers certificate services online through the Apuni Sarkar e-Services portal at eservices.uk.gov.in and the e-District portal at edistrict.uk.gov.in, both run for the districts by the state's IT Development Agency. The death itself can also be reported on the national Civil Registration System portal at dc.crsorgi.gov.in, which emails the certificate to the informant.
Within how many days must a death be registered in Uttarakhand?
A death must be reported to the registrar within 21 days of its occurrence in Uttarakhand, as everywhere in India, under the Registration of Births and Deaths Act 1969. Delayed registration is still allowed under Section 13, with a late fee, and any registration more than one year after the death needs an order under Section 13(3).
What does a death certificate cost in Uttarakhand?
Uttarakhand charges only the government fee displayed on the portal payment screen or quoted at the Nagar Nigam, Nagar Palika or Gram Panchayat counter, and reporting a death inside 21 days carries no penalty. Additional certified copies are charged per copy. Confirm the exact amount on the portal or at the counter before paying anyone.
Is a medical certificate of cause of death compulsory in Uttarakhand?
Yes, every medical institution in Uttarakhand must issue a Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, under the Registration of Births and Deaths (Amendment) Act 2023 in force from 1 October 2023. For a death at home in a village with no attending doctor, the registrar relies on the cremation or burial receipt and an affidavit instead.
My relative died in a hill village far from the block office — what do I do?
Report the death to the Gram Panchayat registrar of that village within 21 days, or file the application through the nearest Common Service Centre, which is the standard route across Uttarakhand's hill districts. Chamoli, Pithoragarh, Uttarkashi, Rudraprayag, Bageshwar and Champawat districts all route certificate services through the District e-Governance Society and the e-District portal, so a CSC operator can file the same application you would file yourself.
Someone died on a Char Dham yatra in Uttarakhand — who registers the death?
The local body where the death occurred registers it, so a pilgrim from another state who dies in Uttarakhand is registered by the Gram Panchayat, Nagar Palika or Nagar Nigam of that place, not by the home state. The family collects the certificate from that registrar or downloads it once the entry is live, and no other state can re-issue the entry.
How many copies of the death certificate should I order in Uttarakhand?
Order at least five certified copies in one application, because the bank, the insurer, the pension or EPFO office, the electricity and gas transfer, and the mutation of khatauni land records each keep a copy permanently. Returning later for one copy at a time costs the same per-copy fee plus another trip, which is expensive in the hill districts.
