What is Mukhyamantri Vishesh Swasthya Sahayata Yojana?

Mukhyamantri Vishesh Swasthya Sahayata Yojana is a special health assistance scheme of the Department of Health and Family Welfare, Government of Chhattisgarh, run by the same State Nodal Agency that administers Ayushman Bharat and Dr Khubchand Baghel Swasthya Sahayata Yojana (DKBSSY) in the state. Readers searching in Hindi will find it as Mukhyamantri Vishesh Swasthya Sahayata Yojana, and it is commonly abbreviated MVSSY.

The scheme exists because ordinary health insurance limits do not survive a rare disease. A family covered under DKBSSY has a fixed annual limit; a liver transplant, a bone marrow transplant, or long-course therapy for a rare metabolic disorder can cost several times that. Mukhyamantri Vishesh Swasthya Sahayata Yojana provides a separate, committee-approved corpus for exactly those cases.

Its own portal sits at dkbssy.cg.nic.in/mvssy, alongside the DKBSSY system, where the state publishes case reports and the list of empanelled hospitals.

Main features

  • Cover reported up to Rs 25 lakh for notified rare diseases.
  • Eligibility anchored to the Antyodaya or priority ration card.
  • Mandatory referral from a government medical college.
  • Approval by a state-level committee, not automatic at the hospital counter.
  • Treatment at empanelled hospitals inside and outside Chhattisgarh.

Because this is a committee-approved scheme rather than a package-driven insurance product, the ceiling and the sanctioned amount for a specific patient are decided case by case. Secondary reporting circulates several different figures for sub-categories of beneficiaries. Get the sanctioned amount for your own case in writing from the State Nodal Agency before treatment begins, and do not rely on a figure read in a news report.

Who is eligible for Mukhyamantri Vishesh Swasthya Sahayata Yojana?

You can be covered if:

  • Your family holds an Antyodaya Anna Yojana or priority ration card listed in the updated Government of Chhattisgarh food department database.
  • You are a resident of Chhattisgarh.
  • The illness falls in the notified list of rare or serious diseases for which the scheme is meant: reported categories include organ transplants such as kidney, liver and heart, cancer treatment, cardiac surgery, neurological procedures and rare disorders not covered by other state programmes.
  • A government medical college has issued a referral for the treatment.
  • Treatment is planned at an empanelled hospital.

Reporting also indicates that acid attack survivors and road accident casualties are considered under the scheme's ambit.

You are not eligible if:

  • Your household does not hold an Antyodaya or priority ration card. A general or APL ration card household cannot apply under this scheme; the route for such families is the ordinary DKBSSY cover.
  • You are not a resident of Chhattisgarh.
  • The condition is a routine illness already covered within the normal DKBSSY or Ayushman Bharat package limits. The special scheme is not a way to jump the queue for standard care.
  • You have no government medical college referral. This is the most common disqualifier and it is not waived for urgency at a private hospital.
  • The hospital chosen is not empanelled with the State Nodal Agency.

What documents are required?

Document Mandatory Notes
Antyodaya or priority ration card Yes Must be in the updated state food department list
Aadhaar card of the patient Yes Matched against the ration card and hospital record
Government medical college referral Yes The gating document for the whole case
Diagnosis and costed treatment estimate Yes On the treating hospital's letterhead
Chhattisgarh residence proof Yes Domicile certificate, voter ID or equivalent
Bank account details Where applicable Only for reimbursement-mode settlements

How to apply for Mukhyamantri Vishesh Swasthya Sahayata Yojana

  1. Get the diagnosis confirmed at a government hospital or a government medical college. Carry all earlier reports and prescriptions.
  2. Obtain the referral from the government medical college, naming the condition and the hospital where the treatment is proposed. This step cannot be skipped.
  3. Download the application form from dkbssy.cg.nic.in/mvssy, or collect it from the district health office or the Ayushman Mitra desk at the hospital.
  4. Fill the form and attach the ration card, Aadhaar, residence proof, the referral and the treating hospital's diagnosis and costed estimate.
  5. Submit the file to the District Health Office (Chief Medical and Health Officer) or directly to the State Nodal Agency, and take an acknowledgement with the case number.
  6. Wait for the state committee decision. The committee scrutinises the diagnosis, the estimate and the eligibility, and issues a sanction letter with the approved amount and hospital.
  7. Begin treatment at the empanelled hospital with the sanction letter. For an approved case, settlement is between the hospital and the agency.

If the treatment is urgent

Speak to the Ayushman Mitra at the hospital and to the district nodal officer on the same day. Emergency cases still need the medical college referral, but districts can move a file faster when the hospital raises it through the transaction management system rather than by post. Do not start an expensive private course of treatment on the assumption that it will be reimbursed later; approval is prior, not retrospective.

How to check the status of a case

  1. Open dkbssy.cg.nic.in/mvssy and use the report or case-status page.
  2. Enter the case or application number printed on your acknowledgement.
  3. If the number is lost, ask the Ayushman Mitra at the treating hospital or the District Health Office to look up the case by ration card number.
  4. Call the state health helpline 104 for guidance on where a stuck file currently sits.

The same portal also publishes the list of empanelled hospitals. Check that list before choosing where to be treated β€” a hospital that is excellent but not empanelled will leave you paying the whole bill.

Why applications get rejected, and what to do

  • No government medical college referral. Return to the medical college and get the referral before resubmitting; nothing else in the file substitutes.
  • Ration card not in the updated priority or Antyodaya list. Correct the ration card status at the food department first. A card that has lapsed or been reclassified as general blocks the claim.
  • Name or Aadhaar mismatch between the ration card, Aadhaar and hospital records. Fix the mismatch at the ration card office or through Aadhaar correction.
  • Condition outside the notified list. The committee cannot sanction a disease the scheme does not cover; ask whether the routine DKBSSY package covers it instead.
  • Non-empanelled hospital. Shift the proposed treatment to an empanelled hospital and get the referral amended.
  • Incomplete cost estimate. A one-line estimate is usually returned; the committee needs an itemised projection.
  • Treatment already started without approval. Prior approval is the norm. Cases begun before sanction are the hardest to regularise.

Help and grievance redressal

  • State health helpline: 104, operating across Chhattisgarh.
  • Ayushman Mitra desk at every empanelled hospital, for case entry and status.
  • District Health Office (CMHO) of your district, for verification-stage delays.
  • State Nodal Agency, Ayushman Bharat Chhattisgarh, through dkbssy.cg.nic.in, for committee-stage escalation.

No fee is payable to any person for filing an application under Mukhyamantri Vishesh Swasthya Sahayata Yojana, and an approved case at an empanelled hospital should not generate a cash demand at the counter. Report any such demand to the district nodal officer.

Documents required

Antyodaya or priority ration card
The card must appear in the updated Government of Chhattisgarh food department list; eligibility is anchored to it.
Aadhaar card of the patient
Used for identity verification and for matching against the ration card and the hospital record.
Referral from a government medical college
The single gating document. Without a medical college referral the case does not reach the state committee.
Diagnosis and treatment estimate from the treating hospital
Detailed diagnosis, proposed line of treatment and costed estimate on hospital letterhead.
Chhattisgarh residence proof
Domicile certificate, voter ID or another accepted state residence document.
Bank account details of the patient or attendantoptional
Needed where the case is settled as reimbursement rather than cashless.

Frequently asked questions

What does Mukhyamantri Vishesh Swasthya Sahayata Yojana cover?

Mukhyamantri Vishesh Swasthya Sahayata Yojana covers treatment of notified rare and serious illnesses that exhaust or fall outside the routine packages of Ayushman Bharat and Dr Khubchand Baghel Swasthya Sahayata Yojana. Reported categories include organ transplants, cancer, cardiac surgery and neurological procedures, treated at empanelled hospitals inside and outside Chhattisgarh.

Who is eligible for the scheme?

Families of Chhattisgarh holding an Antyodaya or priority ration card in the state food department's updated list are eligible. Eligibility is family-based and follows the ration card, not individual income certificates.

How much cover does the scheme give?

Cover of up to Rs 25 lakh is reported for notified rare diseases, subject to state committee approval. Lower ceilings circulate in secondary reporting for different beneficiary categories, so treat the exact ceiling applicable to your case as something to be confirmed by the State Nodal Agency in writing before treatment starts.

Is a government medical college referral compulsory?

Yes, a referral from a government medical college is compulsory. The referral is what converts a private diagnosis into a case the state committee can consider, and files without it are returned regardless of how serious the illness is.

Can I be treated outside Chhattisgarh under this scheme?

Yes, treatment at empanelled hospitals outside Chhattisgarh is permitted for notified conditions where the required care is unavailable in the state. The referral must specifically record that.

How is this different from Dr Khubchand Baghel Swasthya Sahayata Yojana?

Dr Khubchand Baghel Swasthya Sahayata Yojana (DKBSSY) is the state's general health insurance cover with a standard annual limit per family. Mukhyamantri Vishesh Swasthya Sahayata Yojana sits above it as a special corpus for rare and very high-cost illnesses that the general cover cannot absorb.

Do I pay anything at the hospital?

No payment is intended at an empanelled hospital for an approved case, because approved treatment is settled directly between the hospital and the State Nodal Agency. Any demand for cash for an approved package should be reported to the district nodal officer or the 104 health helpline.

How do I check the status of my case?

Use the report and case-status pages on dkbssy.cg.nic.in/mvssy with the application or case number issued when the file was uploaded, or ask the Ayushman Mitra at the treating hospital, who can see the case in the transaction management system.
