Birth Certificate in Gujarat (Janma Nu Pramanpatra / e-Olakh)
Gujarat state government scheme
Birth Certificate in Gujarat is registered and issued through e-Olakh, the online birth and death registration system of the Health and Family Welfare Department at eolakh.gujarat.gov.in. Births are registered only at the place of occurrence. The Gram Panchayat, Taluka Panchayat, Nagarpalika or Municipal Corporation office, and every birth must be reported within 21 days under the Registration of Births and Deaths Act 1969.
What is a birth certificate in Gujarat?
A birth certificate in Gujarat is the certified extract of the birth register kept by the Registrar of Births and Deaths of the local body in whose area the child was born, issued under the Registration of Births and Deaths Act 1969. Gujaratis call it janma nu pramanpatra, and the system that produces it online is e-Olakh. An application developed by the National Informatics Centre, Gujarat State, for the Health and Family Welfare Department, Government of Gujarat, running at eolakh.gujarat.gov.in.
That departmental placement is the first thing most families in Gujarat get wrong. The birth register is not a Revenue Department product. The Mamlatdar who signs an income, caste or domicile certificate has nothing to do with it, and neither does Digital Gujarat's general certificate counter. The record sits with the local body registrar and is surfaced through e-Olakh under Health and Family Welfare.
District administrations across Gujarat all point to the same route. The Patan district page states that registration of birth is the right of the child and the first step towards establishing their identity, that births and deaths are registered only at the place of their occurrence, and that registration happens at all Gram Panchayat, Taluka Panchayat and Nagarpalika offices, with e-Olakh as the online portal. The Kachchh district page repeats the same structure; Gram Panchayat offices for villages, Municipality offices for towns, and the respective offices for notified industrial towns, and calls reporting a birth to the Registrar compulsory under the Act.
Gujarat therefore has three parallel registrar networks:
- Rural areas, the Gram Panchayat office, with the Taluka Panchayat above it.
- Nagarpalika towns, the municipality office registers births in its area.
- Municipal Corporations: Ahmedabad, Surat, Vadodara, Rajkot, Bhavnagar, Jamnagar, Junagadh and Gandhinagar register births through the corporation's health department, and several of them run their own online birth modules alongside e-Olakh.
- Notified industrial townships; GIDC notified areas register births with their respective notified area offices, which is why a birth in an industrial estate is sometimes not found in the nearby nagarpalika register.
Since 1 October 2023 the Registration of Births and Deaths (Amendment) Act 2023 has applied across Gujarat. It makes the birth certificate the single document to prove date and place of birth for admissions, driving licences, passports, Aadhaar, marriage registration and government appointments, and it brings the Aadhaar numbers of the parents and the informant into the birth reporting information.
Who can and who cannot apply?
You can apply for a Gujarat birth certificate if the birth took place inside Gujarat and you are a person the Registration of Births and Deaths Act 1969 treats as an informant or interested party:
- The father or mother of the child.
- The head of the household in which a home birth occurred.
- The medical officer in charge of the hospital, maternity home, PHC, CHC or private nursing home where the delivery happened, who reports it directly.
- The jail superintendent for a birth in prison, and the officer in charge for a birth in a hostel, dharamshala or public institution.
- The village headman or officer in charge for an abandoned newborn.
- Any adult born in Gujarat who needs a certified copy of their own entry, on payment of the per-copy fee.
You cannot apply in Gujarat for a birth that took place outside the state. The register is territorial, and Gujarat states the rule explicitly: births are registered only at the place of their occurrence. A Gujarati family whose child was born in Mumbai, Jaipur or Indore must get the certificate from the local body there, and no Gujarat Registrar can convert or reissue that entry. Equally, a child born to visiting parents in a Surat or Ahmedabad hospital is registered in Gujarat, and the family's home state cannot issue that certificate.
You are also not eligible for a fresh registration where the birth is already registered, the correct request is a certified copy or a correction under Section 15, not a new entry. And a Gujarat Registrar cannot register a birth more than one year old without a written order of the Executive Magistrate under Section 13(3).
There is no income limit, no caste condition and no domicile test for a birth certificate in Gujarat. Residence matters only in fixing which registrar holds the record.
What documents are required?
Where the hospital has already reported the birth and you only need a copy, the e-Olakh download route asks only for the application number, your mobile number and the year of birth. Where the entry has to be created or matched, Gujarat Registrars ask for:
- Report of Birth in Form 1, signed by the informant.
- Hospital discharge summary or delivery record showing date, time, sex of the child and the mother's name.
- Aadhaar of both parents and of the informant, now part of the birth reporting information under the 2023 amendment.
- Proof of the family's address at the time of birth, ration card, light bill or property tax receipt.
- Marriage certificate or a joint affidavit where the parents' surnames differ or the father's name is being entered later.
- Affidavit of date, time and place of birth, for a home delivery and for any delayed registration.
- Executive Magistrate's order under Section 13(3), only where the birth is more than one year old.
How to apply for a birth certificate in Gujarat online
- Confirm the birth was reported within 21 days. Ask the hospital for the Form 1 acknowledgement or the registration or application number, or check with the Gram Panchayat, Nagarpalika or corporation ward office. If nothing was reported, registration comes before any certificate.
- Open eolakh.gujarat.gov.in, the Health and Family Welfare Department's birth and death system. Use the birth registration section if the entry still has to be made, or Download Certificate if it already exists.
- For a certificate you already have a number for, click Download Certificate, choose Birth Certificate, and enter the application number. The portal then asks for your mobile number and the year of birth to verify you.
- For a new registration, enter the birth details — child's name if already given, date and time of birth, sex, place of birth with the hospital name, both parents' names and Aadhaar, and the address at the time of birth. Spellings must match the hospital record exactly, because the Registrar compares them against the discharge summary.
- Upload the supporting papers; discharge summary, parents' identity proof and address proof; each within the file size the upload screen states.
- Pay the fee shown on the payment screen and save the acknowledgement with the application number. That number is the only key to the download screen later, so store it somewhere you will still have it in five years.
- Download the certificate as a PDF once the Registrar approves the entry, and print the copies you need.
How to apply offline at the Registrar's office
- Go to the office that holds the register for the place of birth, the Gram Panchayat office for a village birth, the Taluka Panchayat or notified area office where applicable, the Nagarpalika office for a town, or the corporation health department for a city birth.
- Submit the Report of Birth in Form 1 with the hospital discharge record, the parents' Aadhaar and the address proof.
- Pay the counter fee and keep the receipt. Reporting inside 21 days carries no penalty; beyond that the Registrar quotes the late fee under the state rules before accepting the file.
- Collect the signed certificate from the same counter, or take the application number home and download the certificate from e-Olakh instead.
What if the birth was never registered, or is over 1 year old?
Gujarat follows Section 13 of the Registration of Births and Deaths Act 1969 in three stages:
- Within 30 days of the birth, the Registrar registers it on payment of the prescribed late fee.
- After 30 days but within one year: registration needs the written permission of the prescribed authority and an affidavit explaining the delay, plus the late fee.
- After one year: registration is possible only on the written order of an Executive Magistrate or a First Class Magistrate under Section 13(3), supported by hospital, school or panchayat evidence of the date and place of birth.
Adults in Gujarat who need a birth certificate for a passport or a government job most often fall in this third category. Begin by getting written confirmation from the relevant Gram Panchayat or Nagarpalika that no entry exists, because the magistrate will ask for it before passing an order.
How do you check the application status?
Use the e-Olakh Download Certificate screen with the application number: if the certificate is released, the entry has been approved. For a file still pending, follow up with the same Gram Panchayat, Nagarpalika or corporation office that accepted the Form 1, quoting the acknowledgement.
For a file stuck at the state level, the address published for the e-Olakh system is the Deputy Registrar (Birth-Death) and Additional Director (Statistics), Block 5/3, Dr. Jivraj Mehta Bhavan, Gandhinagar 382010. Escalation within the district goes through the District Collector's grievance cell.
Why are birth certificate applications rejected in Gujarat?
- The birth was never reported in Form 1, so there is no register entry to certify.
- The application was filed with the wrong local body, jurisdiction is the place of birth, not the family's home town or village.
- The child's date of birth, sex or the mother's name does not match the hospital discharge record.
- A home delivery was filed with no affidavit and no panchayat or ASHA worker's confirmation.
- The birth is over one year old and no Section 13(3) magistrate's order is attached.
- The father's name is being inserted without a marriage certificate or joint affidavit.
- The application number entered on the download screen does not match the mobile number or year of birth on record.
- Uploaded scans are unreadable or attached to the wrong slot.
What does it cost, and what about agents?
Gujarat issues the birth certificate at the government fee shown on the e-Olakh payment screen or quoted at the Registrar's counter, plus the notified operator charge if you file through a Jan Seva Kendra or Common Service Centre. Reporting a birth within 21 days carries no penalty at all. No talati, panchayat clerk, ward officer or middleman is entitled to a rupee beyond the listed fee.
Agents outside maternity hospitals in Ahmedabad, Surat and Rajkot sell a "same-day birth certificate" for a few thousand rupees. They file the same form on the same portal, and the file still queues before the same Registrar. Keep the receipt the portal generates, and report any demand for money beyond the listed fee to the District Collector's grievance cell.
What do you need the Gujarat birth certificate for?
A Gujarat birth certificate is the base document for school and college admission, Aadhaar enrolment for a child, a passport, a learner's licence, PAN, the electoral roll entry at 18, caste and domicile certificates, marriage registration, EWS and scholarship applications, and any government appointment where date of birth has to be proved. Because each of those offices keeps a copy, most Gujarat families order three or four certified copies in one application rather than returning to the panchayat or nagarpalika each time, and, just as usefully, save the e-Olakh application number so the same certificate can be redownloaded years later.

Frequently asked questions
Who issues a birth certificate in Gujarat?
The Registrar of Births and Deaths of the local body where the birth took place issues a birth certificate in Gujarat — the Gram Panchayat office in a village, the Taluka Panchayat for notified areas, the Nagarpalika office in a town and the Municipal Corporation in Ahmedabad, Surat, Vadodara, Rajkot, Bhavnagar, Jamnagar, Junagadh and Gandhinagar. The online system behind all of them is e-Olakh, run for the Health and Family Welfare Department.
What is e-Olakh?
e-Olakh is Gujarat's online birth and death registration and certificate system at eolakh.gujarat.gov.in, developed by the National Informatics Centre, Gujarat State, for the Health and Family Welfare Department, Government of Gujarat. District administration pages across Gujarat — Patan, Kachchh, Anand, Junagadh and others — point citizens to e-Olakh as the birth certificate route.
How do I download a Gujarat birth certificate from e-Olakh?
Open eolakh.gujarat.gov.in, click Download Certificate and choose Birth Certificate, then enter your application number followed by your mobile number and the year of birth. After verification the certificate is released as a PDF you can save and print.
Where must a birth in Gujarat be registered?
A birth in Gujarat must be registered at the place where it occurred, not where the family lives — the Patan district administration states plainly that births and deaths are registered only at the place of their occurrence. Registration is done at Gram Panchayat, Taluka Panchayat and Nagarpalika offices, and at corporation offices in the eight municipal corporation cities.
Within how many days must a birth be registered in Gujarat?
A birth must be reported to the Registrar within 21 days of its occurrence in Gujarat, as everywhere in India, under the Registration of Births and Deaths Act 1969. Reporting inside 21 days carries no penalty; after that the delayed registration route under Section 13 applies, with a late fee and, past one year, a magistrate's order.
What if the birth in Gujarat is more than 1 year old?
A birth more than one year old can be registered in Gujarat only on the written order of an Executive Magistrate or a First Class Magistrate under Section 13(3) of the Registration of Births and Deaths Act 1969, after the applicant proves the birth with hospital, school or panchayat evidence. Between 30 days and one year, the written permission of the prescribed authority plus an affidavit and the late fee is enough.
Can I add or correct the child's name on a Gujarat birth certificate?
Yes — the child's name can be entered in the birth register after registration by applying to the same Registrar who made the entry, and a correction of a spelling, date or parent's name is made under Section 15 of the Registration of Births and Deaths Act 1969 on documentary proof. The Registrar amends the original register entry and reissues the certificate through e-Olakh; a certificate altered anywhere else is not valid.
Does a Gujarat birth certificate expire?
A birth certificate in Gujarat never expires, because it certifies a permanent entry in the register of the panchayat, nagarpalika or corporation. An office asking for a "recent" copy is asking for a fresh certified extract of the same entry, which the Registrar issues at the per-copy fee.
