What is a death certificate in West Bengal?

A death certificate in West Bengal is the State's permanent legal record that a named person died on a named date at a named place. West Bengal issues it under the Registration of Births and Deaths Act 1969, which makes registration of every death compulsory anywhere in India, and delivers the service through the Department of Health & Family Welfare and its online portal janma-mrityutathya.wb.gov.in. That portal describes its own scope as new issue, delayed issue, correction and digitisation of birth and death certificates, which covers every situation a West Bengal family is likely to face.

The certificate is not a formality. Without it a bank will not release a fixed deposit to a nominee, the Life Insurance Corporation and private insurers will not process a claim, the Land & Land Reforms Department will not record mutation of a plot into the heirs' names, a family pension cannot start, and a probate or succession certificate petition in the district court cannot be filed. Every one of those offices asks for the registered death certificate and not for a hospital letter or a cremation receipt.

West Bengal registers a death by the place where it occurred, not by where the family lives. A death inside the Kolkata Municipal Corporation area is registered by KMC. A death in a municipality such as Howrah, Siliguri, Asansol or Barasat is registered by that municipality's health department. A death in a village is registered by the gram panchayat's registration unit under the block. If a relative from Malda dies in a Kolkata hospital, the Kolkata registration unit issues the certificate, and the family's Malda address is recorded only as the usual residence.

Who can and cannot apply in West Bengal?

You can apply if:

  • You are the head of the household where the death took place. The Act places the reporting duty on you first, ahead of everyone else.
  • You are a near relative of the deceased, spouse, son, daughter, parent, brother or sister, and you are reporting or collecting a certificate for a death in your family.
  • You are the medical officer in charge of the hospital, nursing home or health centre where the death occurred. Institutional deaths are reported by the institution, not by the family.
  • You are the jail superintendent for a death in prison, or the officer in charge for a death in a hostel, dharamshala or similar institution.
  • You are a person who found the body, or the police officer handling an unclaimed or unidentified body, where there is no household to report the death.
  • You need a copy of an already registered death and can show a lawful interest, such as a bank claim, an insurance claim or a property matter.

You cannot apply if:

  • The death did not occur in West Bengal. Registration follows the place of death, so a death in Bihar or Jharkhand must be registered there even if the family lives in West Bengal.
  • You have no relationship to the deceased and no lawful interest in the record. The register is not open for casual inspection of a stranger's entry.
  • You are trying to register a death that is already registered. In that case you apply for a copy or a correction, not a fresh registration; a duplicate entry has to be cancelled later and is far harder to fix.
  • You are trying to register a death more than one year old without a magistrate's order. The Registrar has no power to register it until an Executive Magistrate or First Class Magistrate orders it.

What documents are required?

Document Mandatory What it does
Form 2 death report Yes The statutory report of the death
MCCD in Form 4 or Form 4A Where available Doctor's certified cause of death
ID proof of the deceased Yes Fixes the identity in the register
ID proof and relationship proof of the applicant Yes Shows you may apply
Address proof of the deceased Yes Fixes the registration unit and usual residence
Cremation or burial receipt Often asked Corroborates a home death
Affidavit plus magistrate's order Only if over one year late Authorises delayed registration

Scan every page cleanly before you begin. A cut-off MCCD or a half-visible hospital seal is the commonest reason a West Bengal application is sent back for resubmission rather than rejected outright.

How to apply for a West Bengal death certificate online

  1. Open the state portal at https://janma-mrityutathya.wb.gov.in/. This is the West Bengal Health & Family Welfare Department's birth and death portal and it is the entry point for new registration, delayed registration, correction and downloading a certificate.
  2. Choose the right service. Pick new issue if the death is recent and not yet registered, delayed issue if the 21-day window has passed, correction if a registered certificate carries a wrong spelling or date, and the certificate download or search option if the death is already registered and you only need a copy.
  3. Register or log in with your mobile number and complete the OTP verification. The same login is used later to track the application and to download the finished certificate, so use a number you will keep.
  4. Enter the death details exactly: full name of the deceased, sex, age at death, date of death, place of death with the hospital name or the full home address, the name of the father, mother or spouse, and the usual residence. Copy every spelling from the deceased person's Aadhaar or EPIC so that the register and the ID agree.
  5. Enter the informant's details: your name, relationship to the deceased, address and mobile number. The informant is a legal role under the Act, and the register records who reported the death.
  6. Upload the documents: the MCCD if a doctor issued one, the identity proofs, the address proof and, for a delayed case, the affidavit and the magistrate's order.
  7. Submit and save the acknowledgement number. Photograph the screen. That number is what the Registrar's office will ask for if you have to follow up in person.
  8. Wait for the Registrar's verification. The municipal health department, corporation ward office or panchayat registration unit checks the entry against the hospital record or conducts a local enquiry for a home death.
  9. Download the signed certificate from the same login once the status shows it as issued. A digitally signed PDF downloaded from the government portal is the valid document; you do not need it re-stamped anywhere.

How to register a death offline in West Bengal

An offline route genuinely exists, and for most home deaths in rural West Bengal it is still the normal one.

  1. For a hospital death, do nothing first β€” ask the hospital. The medical officer in charge is the person legally required to report the death to the Registrar, and most West Bengal hospitals and nursing homes hand the family the MCCD and forward the Form 2 report themselves.
  2. For a death at home, go to the Registrar of Births and Deaths for the place of death: the municipality or corporation health department in an urban area, or the gram panchayat registration unit in a rural area. Ask for Form 2.
  3. Fill Form 2 as head of the household, with the date, time and place of death and the deceased person's particulars, and sign it as informant.
  4. Attach the supporting papers, identity proof of the deceased, your own identity and relationship proof, address proof, and the cremation or burial receipt.
  5. Collect the receipt with the application or registration number before you leave the counter.
  6. Return for the certificate, or download it from the state portal once the entry appears there.

Bangla Sahayata Kendras and Common Service Centres will fill the online form for you if you cannot do it yourself. The operator may charge a small typing and scanning fee; that is a service charge for their labour, not a government fee for the certificate.

How do I check the status of my application?

Log in to janma-mrityutathya.wb.gov.in with the mobile number you registered and open your application list, where each entry shows its stage against the acknowledgement number. If the status has not moved for several weeks, the file is sitting with a human, and the fastest fix is to take the acknowledgement number to the Registrar of Births and Deaths for the place of death, the municipal health department, the corporation ward office or the block office for a panchayat unit. The portal publishes a technical support helpline, 1800 313 444 222, for faults with the website itself such as a failed OTP or a stuck upload.

Does a death certificate expire?

No. A death certificate in West Bengal never expires, because it records an event that does not change. Once the death is entered in the register, that entry is permanent and you can obtain a fresh certified copy years later through the same portal. What families do run into is the opposite problem: institutions such as banks, insurers and courts often want a recently issued certified copy rather than a photocopy of a twenty-year-old original, so order two or three copies at the time of registration and download fresh copies from the portal when needed.

Why do West Bengal death registrations get rejected or held up?

  • Name mismatch between the hospital record, the Aadhaar and the form. The register is checked against documents, and a differing spelling stalls the file.
  • No MCCD and no local corroboration for a home death, leaving the Registrar with nothing to verify against.
  • Wrong registration unit, applying to a municipality when the death happened in a hospital in a different corporation area, or the reverse.
  • Delayed reporting without the paperwork. A death over a year old submitted without the affidavit and the magistrate's order cannot be registered at all.
  • A duplicate entry, where the hospital already reported the death and the family applied again separately.
  • Illegible uploads, especially of a doctor's handwritten MCCD.

Where do I complain if the file is stuck?

Escalate in this order. First the Registrar of Births and Deaths for the place of death, which is the municipal or corporation health department or the gram panchayat unit holding the register. Next the District Registrar of Births and Deaths, a role held at the district health administration level. Above that sits the Chief Registrar of Births and Deaths for West Bengal, under the Department of Health & Family Welfare, which owns the state portal. For a purely technical fault on the website, call the portal helpline 1800 313 444 222 first, because a district office cannot fix a broken upload.

Registering a death in West Bengal is free within the 21-day window and otherwise costs only the small statutory late fee that the Registrar or the portal displays to you directly. Nobody needs to pay thousands of rupees to a middleman to "get the certificate made". If an agent quotes a large sum, they are charging you for a form you can file yourself, and no genuine Registrar or portal will ever ask you for the OTP sent to your phone.

Documents required

Death report in Form 2
The statutory death report under the Registration of Births and Deaths Act 1969, filled by the head of the household for a home death, or by the hospital's medical officer in charge for an institutional death.
Medical Certificate of Cause of Death (MCCD)optional
Form 4 where the death occurred in a hospital or nursing home, Form 4A where it happened elsewhere but was attended by a registered medical practitioner. Not available for many home deaths, which are then registered on the household declaration alone.
Identity proof of the deceased
Aadhaar, voter EPIC, PAN, passport or ration card of the person who died.
Identity proof of the applicant
Aadhaar or EPIC of the family member applying, plus proof of the relationship to the deceased.
Proof of address of the deceased
Establishes which West Bengal municipality, corporation ward or gram panchayat is the correct registration unit. Registration is done by place of death, and the usual residence is recorded separately.
Cremation or burial receiptoptional
The burning ghat or burial ground receipt is commonly asked for at municipal counters as corroboration, especially for a home death without an MCCD.
Affidavit or magistrate's order for delayed registrationoptional
Required only where the death is reported after the statutory window. A death reported more than one year after it occurred needs the order of a First Class Magistrate or Executive Magistrate before the Registrar can register it.

Frequently asked questions

Which portal issues the death certificate in West Bengal?

The West Bengal Janma-Mrityu Tathya portal at janma-mrityutathya.wb.gov.in handles death registration and death certificates for the state, and it lists new issue, delayed issue, correction and digitisation of certificates as its services. The certificate itself is signed by the Registrar of Births and Deaths of the municipality, corporation or gram panchayat where the death happened.

How many days do I have to report a death in West Bengal?

Twenty-one days from the date of death is the statutory window under the Registration of Births and Deaths Act 1969, which applies across West Bengal. Reporting later is still possible, but it moves the file into delayed registration, and a death reported more than a year late needs a magistrate's order.

Is there a fee for a West Bengal death certificate?

Registration itself is free when the death is reported within 21 days, because the Act makes registration a duty of the State and not a paid service. Late registration attracts the small statutory late fee, and a fee may apply for extra copies. Check the amount shown on the portal or at the Registrar's counter, and pay nothing to an agent.

Who is the Registrar for a death in Kolkata?

The Kolkata Municipal Corporation is the registration unit for deaths inside the KMC area, and it runs its own birth and death registration counters and online services alongside the state portal. A death in a KMC hospital or home is registered by KMC, not by the district office.

What is the Medical Certificate of Cause of Death and do I need it?

The Medical Certificate of Cause of Death is the doctor's statement of what caused the death, issued in Form 4 by a hospital or nursing home and in Form 4A by a doctor who attended a death outside an institution. A hospital death in West Bengal will always have one, and it is the strongest supporting document for registration; a home death without medical attendance is registered on the household's declaration instead.

Can I register a death that happened years ago?

Yes, West Bengal's portal explicitly offers a delayed issue route, but a death more than one year old can only be registered on the written order of a First Class Magistrate or an Executive Magistrate, along with an affidavit setting out the facts. Apply through the same portal and follow the delayed registration path.

My relative's name is spelt wrong on the certificate. What do I do?

Use the correction service on janma-mrityutathya.wb.gov.in, which is listed on the portal alongside new and delayed issue. Corrections are made by the same Registrar who issued the certificate, on documentary proof of the correct spelling such as the Aadhaar or the school record of the deceased.

Who do I call if the application does not move?

The technical support number published on the West Bengal Janma-Mrityu Tathya portal is 1800 313 444 222. For anything other than a technical fault, go to the Registrar of Births and Deaths at the municipality, corporation health department or gram panchayat that covers the place of death, since that office holds the register.
