Death Certificate in Madhya Pradesh (Mrityu Praman Patra)
Madhya Pradesh state government scheme
Death Certificate in Madhya Pradesh is registered and issued by the Registrar of Births and Deaths of the local body where the death occurred, the Panchayat Secretary in villages and the Chief Municipal Officer or Health Officer in towns. Madhya Pradesh notifies it under the Lok Seva Guarantee Adhiniyam with a 7 working day limit, applied through mpedistrict.gov.in or a Lok Seva Kendra, with urban certificates downloadable from mpenagarpalika.gov.in.
What is a death certificate in Madhya Pradesh?
A death certificate in Madhya Pradesh, the mrityu praman patra, is the certified extract of the death register kept by the Registrar of Births and Deaths of the local body in whose area the death occurred. Madhya Pradesh issues it under the Registration of Births and Deaths Act 1969 read with the state's own registration rules, and the Directorate of Planning, Economics and Statistics, the Yojana, Arthik evam Sankhyiki department: holds the Chief Registrar function for the state.
The registers themselves are local. In rural Madhya Pradesh the registrar is the Gram Panchayat, where the Panchayat Secretary or Panchayat Karmi receives the report and makes the entry. In urban areas the registrar is the Health Officer of the Municipal Corporation or the Chief Municipal Officer of the Nagar Palika or Nagar Parishad. District administration portals such as dhar.nic.in set this out explicitly: applications go to the Health Officer or CMO in urban areas and to the Panchayat Secretary or Panchayat Karmi in rural areas.
Madhya Pradesh is one of the states that has notified this service under a right-to-service law, the Madhya Pradesh Lok Seva Guarantee Adhiniyam. District portals publish a time limit of 7 working days for the certificate in both urban and rural areas, with a defined appeal ladder above it. That statutory clock is what distinguishes the Madhya Pradesh process from a purely departmental one, the applicant has a remedy if the officer sits on the file.
Three portals matter in Madhya Pradesh, and confusing them is the commonest reason a family loses time:
- mpedistrict.gov.in is the MP Lok Seva Guarantee portal, where the death certificate service sits under the certificates group and the application is filed. District pages such as Vidisha's point citizens to the e-KYC citizen services page on this portal.
- mpenagarpalika.gov.in is the state's urban local body portal, which carries a district-wise death certificate download for Nagar Nigam, Nagar Palika and Nagar Parishad records. Jabalpur district administration links its death certificate service straight to this portal.
- dc.crsorgi.gov.in is the national Civil Registration System, where the death can be reported and the certificate delivered electronically.
The Tehsildar and the Revenue Department do not sign death certificates in Madhya Pradesh. Revenue handles income, caste, domicile and namantaran work. The death register belongs to the panchayat or the municipal body.
Since 1 October 2023, the Registration of Births and Deaths (Amendment) Act 2023 has applied in Madhya Pradesh. It makes the Medical Certificate of Cause of Death compulsory from every medical institution, with a copy to the nearest relative, and it opens the national CRS portal for reporting, with the certificate delivered to the informant's registered email and mobile number.
Who can and who cannot apply?
You can apply for a Madhya Pradesh death certificate if the death occurred inside Madhya Pradesh and you are one of the persons the Registration of Births and Deaths Act 1969 recognises as an informant or interested party:
- The nearest relative, spouse, son, daughter, father, mother, brother or sister of the deceased.
- The head of the household in which the death occurred.
- The medical officer in charge of the hospital where the death took place, a government medical college hospital at Bhopal, Indore, Gwalior, Jabalpur, Rewa or Sagar, a district hospital, a community health centre or a private hospital, who reports it to the registrar directly.
- The jail superintendent for a death in prison, and the officer in charge for a death in a hostel, dharamshala, hotel or public institution.
- The gram kotwar or the police officer in charge, for an unclaimed or abandoned body.
- Any person who later needs a certified copy for a bank, insurer, pension office, property or succession matter.
You cannot apply in Madhya Pradesh for a death that occurred outside Madhya Pradesh. A family whose relative died in Maharashtra, Chhattisgarh, Rajasthan, Delhi or the Gulf must obtain the certificate from the authority there; for a death abroad, the Indian mission in that country handles the registration route, not a Madhya Pradesh panchayat.
You are also not eligible for a fresh registration where the death is already entered, the correct request there is a certified copy or a correction to the existing entry. A registrar will hold the file where the death is under police investigation and the post-mortem cause of death has not been released, and where the applicant can show no relationship to or interest in the deceased.
Madhya Pradesh applies no income limit, no caste condition and no domicile test to a death certificate. Residence only decides which registrar holds the register.
What documents are required?
- Report of Death in Form 2, signed by the informant.
- Medical Certificate of Cause of Death from the hospital or attending doctor.
- Cremation or burial ground receipt where the death happened at home, from the mukti dham, crematorium or kabristan.
- Identity proof of the deceased: Aadhaar, PAN, voter ID, samagra ID, ration card or passport.
- Identity proof of the applicant, with proof of relationship where the applicant is not the household head.
- Proof of the deceased's residence in the panchayat or ward — property tax receipt, electricity bill, samagra family ID or ration card.
- Affidavit stating the date, time and place of death, for a home death with no doctor and for delayed registration.
- Order under Section 13(3), only where the death is more than one year old.
How to apply for a death certificate in Madhya Pradesh: step by step
- Collect the Medical Certificate of Cause of Death from the hospital, or the cremation or burial ground receipt for a death at home. The hospital also reports the death to the registrar directly.
- Identify the correct registrar, the Gram Panchayat Secretary in a village, or the Health Officer or Chief Municipal Officer of the Nagar Nigam, Nagar Palika or Nagar Parishad, for the place where the death occurred, not where the family lives.
- File the Report of Death in Form 2 within 21 days, with the medical certificate or cremation receipt, the identity proof of the deceased and your own identity proof, and keep the acknowledgement.
- Or apply online on mpedistrict.gov.in, the MP Lok Seva Guarantee portal, where the death certificate sits under the certificates group and the application is logged against the Lok Seva time limit; a Lok Seva Kendra or a Common Service Centre in the tehsil premises will file it for you if you prefer an assisted counter.
- Track the 7 working day limit. Madhya Pradesh district portals publish 7 working days as the service limit for both urban and rural areas under the Lok Seva Guarantee Adhiniyam.
- Download or collect the certificate. For urban records, mpenagarpalika.gov.in offers a district-wise death certificate download; for rural records, collect the certified copies from the Gram Panchayat. Ask for five or six copies at once.
How do you check the application status?
Track the Lok Seva application on mpedistrict.gov.in using the acknowledgement number the portal or the Lok Seva Kendra issued. For an urban record, search the death certificate download on mpenagarpalika.gov.in by district and the deceased's details. A record that appears there is confirmation that the registrar has completed the entry. Where the report went through dc.crsorgi.gov.in, the national portal tracks the registration number and emails the certificate to the informant.
The MP e-District service publishes a WhatsApp helpline on +91 7552775227 and a Telegram bot for citizen queries on the Lok Seva portal. If the 7 working day limit passes, file a first appeal within 30 days; to the Chief Executive Officer of the Janpad Panchayat in rural areas, or the District Registrar in urban areas, which must be decided in 21 working days. A second appeal lies within 60 days to the District Registrar in rural cases and to the Additional Chief Registrar or the Collector in urban cases.
Why are death certificate applications rejected in Madhya Pradesh?
- The death was never reported in Form 2, so no register entry exists to certify.
- The application went to the panchayat or municipality of the family's residence rather than of the place of death. A frequent problem where a patient was moved to a medical college hospital in Bhopal, Indore, Jabalpur or Gwalior.
- The name, age or father's or husband's name does not match between the hospital record, the Aadhaar and the samagra ID.
- No medical certificate and no cremation or burial ground receipt was attached for a death at home.
- The death is more than one year old and no order under Section 13(3) accompanies the file.
- The death is under police investigation and the cause of death is not yet finalised.
- The search on mpenagarpalika.gov.in returns nothing because the death was registered in a Gram Panchayat, that portal covers urban local bodies, and a rural record has to come from the panchayat.
What does it cost, and what about agents?
Madhya Pradesh issues the death certificate at the fee published on the Lok Seva Guarantee service page or at the panchayat or municipal counter. The MP Lok Seva portal's pattern for related births-and-deaths services is a small Lok Seva Kendra charge of the order of ₹20 plus tender-set service charges, with the online application itself free. Reporting the death inside the 21-day window carries no penalty at all.
No panchayat secretary, ward clerk, Lok Seva Kendra operator or middleman is entitled to anything beyond the published amount. Anyone offering an "urgent" Madhya Pradesh death certificate for a few thousand rupees is filing the same Form 2 you can file yourself, and the file still runs on the same 7 working day clock. Keep the receipt, and if money is demanded, use the appeal route under the Lok Seva Guarantee Adhiniyam and complain to the Collector, the statutory appeal is the cheaper and faster remedy.
What do you need the death certificate for in Madhya Pradesh?
A Madhya Pradesh death certificate is the document every downstream office starts from: closing bank and post office accounts, claiming life insurance and EPFO or state pension dues, transferring the LPG connection and the electricity meter, removing the name from the ration card, samagra family record and electoral roll, and moving an inheritance forward. It is also the base document for the legal heir or succession certificate and for the namantaran of agricultural land in the khasra and khatauni record, which the Tehsildar's court will not take up without it. Because every one of those offices keeps a copy permanently, families order five or six certified copies together.

Frequently asked questions
Who issues a death certificate in Madhya Pradesh?
The Registrar of Births and Deaths of the local body where the death took place issues the death certificate in Madhya Pradesh — the Panchayat Secretary or Panchayat Karmi in a village, and the Health Officer or Chief Municipal Officer of the Nagar Nigam, Nagar Palika or Nagar Parishad in a town. The Directorate of Planning, Economics and Statistics holds the state Chief Registrar function; the Tehsildar does not issue it.
How long does a death certificate take in Madhya Pradesh?
Madhya Pradesh notifies the death certificate under the Lok Seva Guarantee Adhiniyam with a time limit of 7 working days in both urban and rural areas, as published on district portals such as dhar.nic.in. If the designated officer misses that limit, a first appeal lies within 30 days and must be decided in 21 working days.
Which portal do I use for a Madhya Pradesh death certificate?
mpedistrict.gov.in is the Madhya Pradesh Lok Seva Guarantee portal through which the death certificate service is applied for, and it links the registration itself to the Civil Registration System. For urban records, mpenagarpalika.gov.in carries a district-wise death certificate download for Nagar Nigam, Nagar Palika and Nagar Parishad areas.
Within how many days must a death be registered in Madhya Pradesh?
A death must be reported to the registrar within 21 days of its occurrence in Madhya Pradesh, as everywhere in India, under the Registration of Births and Deaths Act 1969. Delayed registration remains possible under Section 13 with a late fee, and a registration more than one year after the death needs an order under Section 13(3).
What does a Madhya Pradesh death certificate cost?
Madhya Pradesh charges only the fee published on the Lok Seva Guarantee service page or at the local body counter, and the MP Lok Seva portal's pattern for related births-and-deaths services is a small Lok Seva Kendra charge of the order of ₹20 plus tender-set service charges, with the online application free. Confirm the exact amount on the service page or at the counter before paying anyone.
Where do I appeal if the death certificate is not issued?
A first appeal lies within 30 days — to the Chief Executive Officer of the Janpad Panchayat in rural areas and to the District Registrar in urban areas — and must be decided within 21 working days, according to Madhya Pradesh district portals. A second appeal lies within 60 days to the District Registrar in rural cases and to the Additional Chief Registrar or Collector in urban cases.
Can I get a Madhya Pradesh death certificate at a Lok Seva Kendra?
Yes, the Lok Seva Kendra is the state's assisted counter for this service and district administrations list it as the place to apply, alongside the Common Service Centres located in tehsil premises. The operator files the application for you on mpedistrict.gov.in; the registrar of the place of death still approves the register entry.
How many copies of the death certificate should I order in Madhya Pradesh?
Order at least five certified copies, because the bank, the insurer, the pension or EPFO office, the LPG and electricity transfer, and the namantaran of khasra and khatauni land records each keep a copy permanently. Where the record is on mpenagarpalika.gov.in, further copies can be printed from the download, which saves repeat trips to the ward office.
