Odisha state government scheme

Quick answer

Death Certificate in Odisha is issued by the Registrar of Births and Deaths of the urban local body or Gram Panchayat where the death occurred, and Odisha runs a dedicated state portal at birthdeath.odisha.gov.in, the link Bhubaneswar Municipal Corporation itself points citizens to. Odisha's e-District portal does not carry the death certificate. Every death must be reported within 21 days.

Benefit
The official state record of a death in Odisha, required to close bank accounts, claim insurance and pension, mutate land in the RoR and obtain a legal heir certificate
How to apply
Both — online on birthdeath.odisha.gov.in or the national CRS portal, and offline at the Municipal Corporation, Municipality, NAC or Gram Panchayat registrar, or through a Common Service Centre

What is a death certificate in Odisha?

A death certificate in Odisha is the certified extract of the death register maintained by the Registrar of Births and Deaths of the local body in whose area the death occurred. Odisha issues it under the Registration of Births and Deaths Act 1969, a central law administered by the state, and it is the founding document for every claim a family makes afterwards, insurance, pension, bank balances, land mutation and the legal heir certificate.

Odisha keeps this on a portal of its own. The state Birth and Death portal at birthdeath.odisha.gov.in is the online route, and Bhubaneswar Municipal Corporation links citizens to it directly from its own site rather than to the e-District system. This matters because Odisha's e-District portal at edistrict.odisha.gov.in, the one most people already know for resident, income, caste, SEBC, solvency and legal heir certificates from the Revenue and Disaster Management Department, does not carry the death certificate at all. e-District mentions the death certificate only as a document you must attach to a Legal Heir Certificate application.

Odisha's registration units follow local-body boundaries:

  • Municipal Corporations: Bhubaneswar, Cuttack, Rourkela, Berhampur and Sambalpur register deaths through the corporation's health or registration wing. Bhubaneswar Municipal Corporation publishes a 24x7 toll-free helpline on 1929 and a general line on 8280282000 for citizen queries.
  • Municipalities and Notified Area Councils, the registrar sits under the Executive Officer of the ULB.
  • Rural areas. The Gram Panchayat registration unit, working with the CHC or PHC medical officer who reports institutional deaths.

Since 1 October 2023 the Registration of Births and Deaths (Amendment) Act 2023 has applied in Odisha. Medical institutions must now hand the Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, and the registered certificate can be delivered electronically to the informant's email and mobile number.

Who can and who cannot apply?

You can apply for an Odisha death certificate if the death occurred inside Odisha and you are a person the Registration of Births and Deaths Act 1969 treats as an informant or interested party:

  • The nearest relative of the deceased; spouse, son, daughter, father, mother, brother or sister.
  • The head of the household in which the death occurred.
  • The medical officer in charge of the hospital, nursing home, CHC or PHC where the death took place, who reports it directly.
  • The jail superintendent for a death in prison, and the officer in charge for a death in a hostel, dharamshala or public institution.
  • The Grama Rakhi, village headman or officer in charge, for a body found abandoned or unclaimed.
  • Anyone who later needs a certified copy for a bank, insurance, pension, property or succession purpose, on payment of the per-copy fee.

You cannot apply in Odisha for a death that occurred outside the state. The register is territorial: an Odia resident who died in West Bengal, Andhra Pradesh or Chhattisgarh is registered by the local body there, and no Odisha registrar can convert or re-issue that entry. You are also not eligible to have a fresh registration made where the death is already registered. The correct route is a certified copy of the existing entry, not a new one. A registrar will refuse where the death is under police investigation and the post-mortem cause of death has not been released, and where the applicant can show no relationship to or interest in the deceased.

There is no income limit, no caste criterion and no domicile test for a death certificate in Odisha. Residence matters only in deciding which registrar holds the record.

What documents are required?

In the ordinary case the hospital or the household has already reported the death in Form 2 and the registrar is simply certifying an existing entry. Odisha registrars ask for:

  • Report of Death in Form 2, signed by the informant.
  • Medical Certificate of Cause of Death from the treating hospital or doctor, compulsory from medical institutions since 1 October 2023.
  • Cremation or burial receipt where the death occurred at home with no hospital record.
  • Identity proof of the deceased: Aadhaar, voter ID, PAN, ration card or passport.
  • Identity proof of the informant and proof of relationship where the applicant is not the household head.
  • Proof of the deceased's address — ration card, electricity bill or holding tax receipt, to confirm the ward or panchayat.
  • Affidavit stating the date, time and place of death, for a home death with no attending doctor and for delayed registration.
  • Order of the Executive Magistrate, only where the death is more than one year old.

How to apply for a death certificate in Odisha online

  1. Confirm the death was reported within 21 days. Ask the hospital for the Form 2 acknowledgement, or check with the ULB registrar or Gram Panchayat. If the death was never reported, registration must happen before any certificate can be issued.
  2. Open birthdeath.odisha.gov.in, the state Birth and Death portal that Bhubaneswar Municipal Corporation links to, and create a citizen login with your mobile number and email. Verify the OTP.
  3. Choose the death service: registration of a death that has not yet been reported, or search and download of a certificate where the registration number already exists.
  4. Enter the death details. Fill in the full name of the deceased, date and place of death, age at death, father's or husband's name, permanent address, and the informant's name and relationship. Spellings must match the hospital record, because the registrar compares them against the register entry.
  5. Upload the supporting papers, the Medical Certificate of Cause of Death or the cremation receipt, identity proof of the deceased and your own identity proof: within the file size limits the upload screen states.
  6. Pay the fee shown on the payment screen and save the acknowledgement number. Order all the copies you need in a single application; each additional copy is charged at the per-copy fee.

How to apply offline at the registrar's office

  1. Go to the office that holds the register for the place of death. The Municipal Corporation's registration counter in Bhubaneswar, Cuttack, Rourkela, Berhampur or Sambalpur, the Municipality or NAC office in a smaller town, or the Gram Panchayat office in a rural area.
  2. Submit the Report of Death in Form 2 with the Medical Certificate of Cause of Death or the cremation or burial receipt, along with identity proof of the deceased and of yourself.
  3. Pay the counter fee and keep the receipt. Registration inside 21 days carries no penalty; beyond that the registrar quotes the late fee fixed in the Odisha rules before accepting the file.
  4. Collect the signed certificate from the same counter, or file the whole application through a Common Service Centre if travelling to the ULB office is difficult.

How do you check the application status?

Track the application on birthdeath.odisha.gov.in using the acknowledgement number printed on your receipt. For a counter application, the registrar's diary number is what the office searches on, so photograph the receipt before leaving. In Bhubaneswar, the BMC's 24x7 toll-free helpline on 1929 and its office line on 8280282000 handle citizen queries about pending files. Where the death was reported through the national Civil Registration System, the certificate is emailed to the informant's registered address against the same registration number.

Why are death certificate applications rejected in Odisha?

  • The death was never reported in Form 2, so there is no register entry to certify.
  • The application was filed with the wrong local body; jurisdiction follows the place of death, not the place of residence.
  • The name, age or father's name does not match the hospital record or the identity proof of the deceased.
  • No Medical Certificate of Cause of Death and no cremation or burial receipt was attached for a home death.
  • The death is more than one year old and no Executive Magistrate's order under Section 13(3) accompanies the file.
  • The death is under police investigation and the cause of death has not been finalised.
  • Scans are unreadable, incomplete or uploaded to the wrong slot.

What does it cost, and what about agents?

Odisha issues the death certificate free or at the government fee published on the portal payment screen or quoted at the registrar's counter, plus the notified operator charge at a Common Service Centre. Registration inside the 21-day window carries no penalty at all. Nobody, clerk, ward officer, panchayat secretary or middleman: is entitled to a rupee beyond that listed fee.

Agents outside hospitals and crematoria in Bhubaneswar and Cuttack sell a "same-day death certificate" for a few thousand rupees. They file the same form you can file yourself, and it queues before the same registrar. Insist on a receipt for every payment, and report a demand for money to the BMC helpline on 1929 in Bhubaneswar, or to the Collector's grievance cell in your district.

What do you need the death certificate for?

An Odisha death certificate is the starting document for closing bank and post office accounts, claiming life insurance and EPFO or state pension dues, transferring the electricity connection and gas connection, mutating land in the Record of Rights through the Tahsildar, surrendering the ration card and voter roll entry, and applying for the Legal Heir Certificate on the Odisha e-District portal, which expressly requires a copy of the death certificate. Each of these offices keeps its own copy, which is why families in Odisha order several certified copies in one application instead of returning to the registrar again and again.

Frequently asked questions

Who issues a death certificate in Odisha?

The Registrar of Births and Deaths of the local body where the death took place issues the death certificate in Odisha — the Municipal Corporation in Bhubaneswar, Cuttack, Rourkela, Berhampur and Sambalpur, the Municipality or Notified Area Council in smaller towns, and the Gram Panchayat registration unit in rural areas. The certificate is issued under the Registration of Births and Deaths Act 1969, not by the Tahsildar or the Revenue Department.

Which Odisha portal handles death certificates?

Odisha runs a dedicated Birth and Death portal at birthdeath.odisha.gov.in, which Bhubaneswar Municipal Corporation links to directly from its own citizen services page. The Odisha e-District portal at edistrict.odisha.gov.in does not carry the death certificate — it lists Revenue and Disaster Management certificates such as resident, income, caste, solvency and legal heir, and only asks for the death certificate as a supporting document for the legal heir certificate.

Within how many days must a death be registered in Odisha?

A death must be reported to the registrar within 21 days of its occurrence in Odisha, as everywhere in India, under the Registration of Births and Deaths Act 1969. Registration after 21 days is still possible under the delayed registration provisions of Section 13, with a late fee, and after one year only on the written order of an Executive Magistrate.

What does a death certificate cost in Odisha?

Odisha charges only the government fee shown on the portal payment screen or at the registrar's counter, and registration inside the 21-day window carries no penalty. Delayed registration attracts the late fee fixed in the Odisha rules, which the registrar quotes before accepting the file. Ask for the receipt for every rupee, and treat any larger "service charge" as an agent's markup, not a fee.

Do I need the death certificate for a legal heir certificate in Odisha?

Yes, the Odisha e-District portal lists a copy of the death certificate as a required document for the Legal Heir Certificate application, which is handled by the Revenue and Disaster Management Department. Get the death registered and the certificate issued first, because the legal heir file cannot move without it.

Is a medical certificate of cause of death compulsory in Odisha?

Yes, a medical institution in Odisha must issue a Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, under the Registration of Births and Deaths (Amendment) Act 2023 in force from 1 October 2023. For a home death with no attending doctor, the registrar records the cause as unclassified and relies on the cremation or burial receipt with an affidavit.

How do I get extra copies of an Odisha death certificate?

Ask the same registrar or the same portal login that issued the first copy, and pay the per-copy government fee. Banks, insurers, the EPFO, the electricity utility and the Tahsildar handling mutation each retain a copy, so most families order five or six certified copies at once.

Does an Odisha death certificate expire?

A death certificate in Odisha never expires, because it certifies a fixed entry in the permanent register of the local body. An office asking for a "recent" copy wants a fresh certified extract of the same entry, issued by the registrar at the per-copy fee.
