MEDISEP (Medical Insurance Scheme for State Employees and Pensioners)
Kerala state government scheme
MEDISEP is Kerala's compulsory health insurance scheme for serving state employees and pensioners, giving Rs 1.5 lakh floater cover per three-year block plus Rs 3 lakh to Rs 20 lakh for organ transplants and 12 critical illnesses. Premium is Rs 500 a month from salary or pension. Enrolment is automatic through the DDO, not by individual application.
What is MEDISEP?
MEDISEP, short for Medical Insurance Scheme for State Employees and Pensioners, is a compulsory group health insurance scheme run by the Finance (Health Insurance) Department, Government of Kerala, from the Government Secretariat in Thiruvananthapuram. MEDISEP became operational on 1 July 2022 and is now in its Phase II policy cycle.
According to the MEDISEP portal, the scheme is intended to provide comprehensive health insurance coverage to all serving employees and pensioners of the state government. It covers roughly 11 lakh employees and pensioners, and counting their dependent family members, close to 30 lakh beneficiaries in total.
MEDISEP is delivered in partnership with The Oriental Insurance Company Limited, a public sector insurer selected through an e-tender process. The government pays the annual premium to the insurer in advance in four instalments and then recovers it from beneficiaries at Rs 500 a month.
What makes MEDISEP different from a retail health policy
- Pre-existing diseases are covered from day one, with no medical check-up before enrolment.
- There is no upper age limit, an 85-year-old pensioner is covered on the same terms as a 25-year-old employee.
- Every category of employee and pensioner gets an identical package; there are no tiers by pay scale or designation.
- The sum insured is a floater across the family, not a per-person cap.
Who is covered under MEDISEP?
Covered under MEDISEP:
- Serving employees of the Government of Kerala, including newly recruited staff.
- Pensioners of the state government, including family pensioners.
- Part-time employees of the state government.
- Employees of aided educational institutions brought under the scheme.
- Dependants and family members of all of the above, under the floater cover.
- Employees of universities, local self-government institutions (municipality, panchayat, corporation) and departments covered by the MEDISEP logins on the portal.
Not eligible for MEDISEP:
- Central government employees and central PSU staff posted in Kerala, who are covered by their own schemes.
- Private sector employees and self-employed residents of Kerala. They cannot apply for MEDISEP at all — the scheme is confined to state government service.
- Employees of institutions not notified as coming under the MEDISEP order are not eligible.
MEDISEP is not a voluntary scheme. A serving employee or pensioner in a covered category cannot opt out of the Rs 500 monthly recovery, and equally cannot buy into the scheme from outside government service.
How much does MEDISEP pay and what does it cover?
The Basic Benefit Package carries a sum insured of Rs 1.5 lakh on a floater basis, usable within a three-year block period. This is an important distinction from ordinary annual policies: the amount is not reset every 1 April, it is a pool that the family draws down over the block.
The Basic Benefit Package contains 1,920 treatment packages spanning medical treatment, surgical procedures and day-care procedures such as dialysis, chemotherapy and cataract surgery.
The Additional Benefit Package sits on top and is financed from a separate Rs 35 crore corpus fund. It gives an individual Rs 3 lakh to Rs 20 lakh for various organ transplants and for 12 specified critical illnesses. This is the reason the scheme's headline maximum benefit is Rs 20 lakh rather than Rs 1.5 lakh.
What the cover includes:
- Cost of treatment procedures, medicines, doctor and attendant fees, room rent, diagnostic charges and disease-related food costs at empanelled hospitals.
- A private ward at a tariff up to Rs 2,000 a day.
- Pre-hospitalisation expenses for 15 days before admission and post-hospitalisation expenses for 15 days after discharge, including medical and laboratory charges.
- All treatments for congenital conditions in newborns and children.
- COVID-19 treatment.
- Emergency treatment at non-empanelled hospitals in accident and life-threatening situations.
What the cover does not include: ordinary outpatient consultation and medicines. Existing medical reimbursement rules continue to operate for OP treatment, so MEDISEP should be read as a hospitalisation scheme.

| PEN or PPO number | Yes | Default password for the Insured Login |

How to use MEDISEP for cashless treatment
- Log in at medisep.kerala.gov.in using Insured Login with your MEDISEP ID, and download your MEDCARD from the Medcard Download link.
- Check the Hospitals section of the portal to confirm the hospital you plan to use is empanelled, and check the Packages section to see whether your procedure is in the list of 1,920 packages.
- Report to the MEDISEP help desk inside the hospital before admission with your MEDCARD and photo ID, and ask the desk to raise the pre-authorisation.
- Take the treatment on a cashless basis once pre-authorisation is approved; the hospital settles directly with Oriental Insurance and you do not pay from your pocket for covered items.
- In an accident or life-threatening emergency at a non-empanelled hospital, get admitted first, then inform the insurer on 1800-425-0237 and use the Reimbursement route on the portal afterwards.
- Track the balance in your three-year block using the PassBook and Status options after discharge.
How to apply for MEDISEP enrolment
- The DDO or nodal officer of the office adds the serving employee in the MEDISEP system and completes SPARK verification. Pensioners are handled through the treasury login.
- The employee or pensioner verifies their own and their dependants' details after logging in on the portal, and reports errors back to the DDO or treasury.
- The Rs 500 monthly deduction begins from salary or pension, and the MEDISEP ID and MEDCARD become available for download.
There is no counter, no form and no fee to be paid to any agent. If an employee moves office, the DDO uses the Transfer: Relieving/Joining option so the record follows the employee; a change of DDO is handled through the Change DDO Charge option on the login page.
Why do MEDISEP claims get rejected or delayed?
- SPARK verification not completed by the DDO or nodal officer, so the employee's family details do not reflect in the system. The MEDISEP portal has repeatedly issued notices to nodal officers and DDOs on exactly this point.
- Dependant not listed in the beneficiary record, so the hospital cannot raise a pre-authorisation in that person's name.
- Procedure outside the 1,920 packages, or a package amount already exhausted within the three-year block.
- Non-empanelled hospital used for a planned, non-emergency admission.
- Pre-authorisation not raised before admission at the hospital help desk.
- Room rent above the Rs 2,000 private ward limit, where the excess is borne by the beneficiary.
The fix for the first two is always the same: contact your DDO or treasury nodal officer and get the record corrected in the MEDISEP system, rather than arguing with the hospital.
What are the key dates and reconciliation stages?
MEDISEP runs on three-year block periods tied to the MoU with the insurer, which is why the Rs 1.5 lakh basic cover is expressed per block rather than per year. On the accounting side, the portal records that Phase I reconciliation ended in December 2025, and Phase II reconciliation started in February 2026, covering the salary of January 2026 and the pension of February 2026. A separate option under the Accounting menu handles Phase II reconciliation.
For an ordinary beneficiary these dates matter only in one way: they explain why a deduction or a passbook entry may take a month to appear correctly, and they are handled by the DDO, not by the employee.
Where do you get help or file a MEDISEP grievance?
- MEDISEP toll-free: 1800-425-1857, available 10:15 am to 5:15 pm
- Oriental Insurance helpline: 1800-425-0237, available 24x7 for treatment-related problems
- Email: info.medisep@kerala.gov.in
- Online grievance system on medisep.kerala.gov.in, operating at district, state and appellate levels
- Office: Finance (Health Insurance) Department, Government Secretariat, Thiruvananthapuram, Kerala 695001
The portal also carries Instructions to Beneficiaries, Instructions to Hospitals, FAQ sets written separately for employees and for pensioners, a Nodal Officers directory and video help. Reading the FAQ set that matches your category answers most questions faster than the helpline, because the employee and pensioner processes differ on enrolment even though the benefits are identical.

Frequently asked questions
How much does MEDISEP cost every month?
MEDISEP costs Rs 500 a month, recovered directly from the salary of a serving employee or the pension of a pensioner. The Kerala government pays the annual premium to the insurer in advance in four instalments and then recovers it from the beneficiary in 12 equal monthly deductions of Rs 500, so nobody has to pay a lump sum.
How much treatment cover does MEDISEP give?
MEDISEP gives Rs 1.5 lakh on a floater basis for the whole family, usable across a three-year block period rather than reset each year. On top of that, an Additional Benefit Package funded from a Rs 35 crore corpus gives Rs 3 lakh to Rs 20 lakh for various organ transplants and 12 specified critical illnesses.
Is MEDISEP optional for Kerala government employees?
No, MEDISEP is compulsory for serving Kerala state government employees and pensioners. Because it is compulsory and premium-linked to salary and pension, there is no separate public application form and no enrolment window; the DDO or treasury adds the beneficiary and the deduction begins.
Are pre-existing diseases covered under MEDISEP?
Yes, MEDISEP covers pre-existing diseases from day one with no waiting period and no medical check-up before enrolment. The scheme also applies no upper age limit, so an elderly pensioner gets exactly the same cover as a newly recruited employee.
What happens if I am admitted to a hospital that is not empanelled?
MEDISEP covers treatment at non-empanelled hospitals in accident and life-threatening emergency situations. In all other cases treatment must be taken at one of the empanelled hospitals in the network, which includes government super-specialty hospitals and a large number of private hospitals inside and outside Kerala.
Does MEDISEP cover OP treatment?
No, MEDISEP is a hospitalisation scheme and does not replace outpatient cover. The existing medical reimbursement arrangement continues for OP treatment. MEDISEP does cover medical and laboratory charges for the 15 days before admission and the 15 days after discharge.
How do I check my MEDISEP status or download my MEDCARD?
Log in at medisep.kerala.gov.in using the Insured Login option with your MEDISEP ID; the default password is your PEN if you are a serving employee and your PPO number if you are a pensioner. The portal has a Status option, a PassBook option, a Medcard Download link and a searchable list of empanelled hospitals and packages.
Where do I complain if a hospital refuses cashless treatment?
Use the online grievance system on medisep.kerala.gov.in, which works at district, state and appellate levels. For an immediate treatment problem call the Oriental Insurance 24x7 helpline on 1800-425-0237, or the MEDISEP toll-free number 1800-425-1857 during office hours, or write to info.medisep@kerala.gov.in.
