What is a birth certificate in Madhya Pradesh?

A birth certificate in Madhya Pradesh is the certified extract of the birth register kept by the Registrar of Births and Deaths of the local body where the child was born. Madhya Pradesh issues it under the Registration of Births and Deaths Act 1969 together with the M.P. Birth and Death Registration rules, and the state's district administrations state the rule plainly: every birth is to be reported and registered within 21 days at the place of its occurrence, live births and still births alike.

Two networks hold the register in Madhya Pradesh:

  • Rural areas. The Gram Panchayat secretary, supported by the patwari's birth notification, registers births in the panchayat's register.
  • Urban areas. The Nagar Panchayat, Nagar Palika Parishad or Nagar Nigam registers births through its health branch, and the certificate is served through the state's MP e-Nagarpalika urban local body portal.

The counter that citizens actually deal with is the Lok Sewa Kendra. The Sagar district administration lists the Lok Sewa Kendra near the Tehsil office as the place for a birth certificate, and Jabalpur lists its Lok Sewa Kendra and links citizens to the MP e-Nagarpalika portal. MP Online kiosks and Common Service Centres act as additional filing counters.

Where the register has a gap; no entry, or a birth over a year old: Madhya Pradesh does not leave families to negotiate with a clerk. It has notified two named services under the Madhya Pradesh Lok Seva Guarantee framework, both belonging to the Planning, Economics and Statistics Department:

Lok Seva Guarantee service Time limit Fee at LSK / MP Online / CSC Fee online
Birth non-availability certificate (जन्म का अप्राप्यता प्रमाण पत्र) 7 working days, urban and rural Rs 20 + legal charges Free
Permission for registration after 1 year of birth 15 working days, urban and rural Rs 20 + legal charges Free

Since 1 October 2023 the Registration of Births and Deaths (Amendment) Act 2023 has applied in Madhya Pradesh, making the birth certificate the single document to prove date and place of birth for admissions, Aadhaar, driving licences, passports and government appointments, and opening the national Civil Registration System portal at dc.crsorgi.gov.in for reporting a birth.

Who can and who cannot apply?

You can apply for a Madhya Pradesh birth certificate if the birth took place inside Madhya Pradesh and you are a person the Act treats as an informant or interested party:

  • The father or mother of the child.
  • The head of the household in which the birth occurred, for a home delivery.
  • The medical officer in charge of the hospital, CHC, PHC or nursing home where the delivery took place, who reports it directly and issues the birth notification certificate.
  • The jail superintendent for a birth in prison, and the officer in charge for a birth in a hostel or public institution.
  • The village headman or officer in charge for an abandoned newborn.
  • Any adult born in Madhya Pradesh who needs a certified copy of their own entry.

You cannot apply in Madhya Pradesh for a birth that took place outside the state. The register is territorial, so a family from Indore whose child was born in Maharashtra or Chhattisgarh must obtain the certificate from the local body there, and no MP registrar can convert that entry. Conversely, a child born to visiting parents in a Bhopal, Indore, Gwalior or Jabalpur hospital is registered in Madhya Pradesh.

You are also not eligible for a fresh registration where the birth is already entered, the correct request is an additional certified copy or a correction under Section 15. And a Madhya Pradesh registrar cannot register a birth more than one year old until the Planning, Economics and Statistics Department has granted the notified permission for registration after one year.

Madhya Pradesh applies no income limit, no caste condition and no domicile test to a birth certificate. It is a vital registration available to every child born in the state.

What documents are required?

For an ordinary registration within 21 days, the hospital's report and the parents' identity proof are enough. For the delayed-registration route Madhya Pradesh names the documents explicitly:

  • Hospital birth notification certificate, or an affidavit, in urban areas.
  • Birth notification certificate from the patwari or the Gram Panchayat secretary, in rural areas.
  • Birth non-availability certificate, the separate 7-working-day Lok Seva Guarantee service.

Alongside these, registrars in Madhya Pradesh ask for:

  • Report of Birth in Form 1, signed by the informant.
  • Hospital discharge summary showing date, time, sex of the child and the mother's name.
  • Aadhaar of both parents, now part of the birth reporting information under the 2023 amendment.
  • Proof of the family's address at the time of birth, ration card, samagra ID record, electricity bill or property tax receipt.

Notably, Madhya Pradesh lists no documents at all for the birth non-availability certificate itself.

How to apply for a birth certificate in Madhya Pradesh

  1. Check that the birth was reported within 21 days. Ask the hospital for the birth notification certificate and the registration number, or ask the Gram Panchayat secretary or the ward office of the Nagar Palika. Reporting inside 21 days carries no penalty.
  2. For an urban birth, use the MP e-Nagarpalika portal. Jabalpur and other district administrations link citizens there for birth certificate services from the urban local body's own register; select the ULB, search the entry and download or request the certificate.
  3. For a rural birth, apply through the Gram Panchayat secretary or a Lok Sewa Kendra. Sagar district names the Lok Sewa Kendra near the Tehsil office as the point of contact.
  4. If no entry exists, first apply for the birth non-availability certificate on the Madhya Pradesh Lok Seva Guarantee portal — 7 working days, free online, Rs 20 plus legal charges at a Lok Seva Kendra, MP Online kiosk or CSC, with no documents required.
  5. If the birth is more than one year old, apply for permission for registration after 1 year on the same Lok Seva Guarantee portal to 15 working days, free online, Rs 20 plus legal charges at a kiosk: attaching the hospital birth notification certificate or affidavit (urban) or the patwari or panchayat secretary's notification (rural), plus the non-availability certificate.
  6. Take the permission back to the registrar, who then creates the entry and issues the certificate. Order all the copies you need in one visit.

How to apply offline at a Lok Sewa Kendra

  1. Carry the hospital birth notification certificate or the panchayat notification, the parents' Aadhaar and the address proof to the Lok Sewa Kendra, MP Online kiosk or Common Service Centre nearest the place of birth.
  2. Ask the operator to file the correct service; plain certificate issue, birth non-availability certificate, or permission for registration after one year.
  3. Pay Rs 20 plus the legal charges fixed by tender and take the receipt with the application number; the receipt carries the notified delivery date.
  4. Collect the certificate from the same counter or download it once the registrar approves the entry.

How do you check the application status?

Track the application number on the Madhya Pradesh Lok Seva Guarantee portal, or on the MP e-Nagarpalika portal for an urban local body certificate. Because both the non-availability certificate and the late-registration permission are notified services with fixed limits of 7 and 15 working days, a file that overshoots its date is an appealable delay under the Madhya Pradesh Lok Seva Guarantee Adhiniyam, not merely a slow file: raise it with the designated officer at the Lok Sewa Kendra and then with the district authority.

Why are birth certificate applications rejected in Madhya Pradesh?

  • The birth was never reported in Form 1, so there is no register entry to certify.
  • The application was filed with the wrong local body: jurisdiction follows the place of birth, not the family's home village or town.
  • The date of birth, sex or the mother's name does not match the hospital birth notification certificate.
  • A rural home delivery was filed with no patwari or Gram Panchayat secretary notification and no affidavit.
  • The birth is over a year old and the late-registration permission was never obtained.
  • The birth non-availability certificate was not attached to the delayed registration file.
  • Uploaded scans are unreadable or attached to the wrong slot.

What does it cost, and what about agents?

Madhya Pradesh publishes its charges for the two vital-registration Lok Seva Guarantee services as Rs 20 plus legal charges at a Lok Seva Kendra, MP Online kiosk or CSC, and free when the citizen applies online. Registration of a birth reported inside 21 days carries no penalty. Nothing beyond the listed fee is payable to any panchayat secretary, ward clerk, kiosk operator or middleman.

Agents outside maternity hospitals in Bhopal, Indore, Gwalior and Jabalpur sell "urgent" birth certificates for a few thousand rupees. They file the same Lok Seva Guarantee application you can file free online, and the file still waits for the same registrar and the same notified time limit. Insist on the printed receipt with the application number, and take a demand for extra money to the Collector's Lok Seva Guarantee cell.

What do you need the birth certificate for in Madhya Pradesh?

A Madhya Pradesh birth certificate is the base document for school and college admission, Aadhaar enrolment for a child, a passport, a learner's licence, PAN, the electoral roll entry at 18, caste and domicile certificates, Ladli Laxmi and other child-linked state schemes, marriage registration, and any government appointment where date of birth must be proved. Each of those offices keeps a copy permanently, which is why families order several certified copies at once rather than returning to the Lok Sewa Kendra each time.

Frequently asked questions

Who issues a birth certificate in Madhya Pradesh?

The Registrar of Births and Deaths of the local body where the birth took place issues it in Madhya Pradesh — the Gram Panchayat secretary in rural areas and the Nagar Panchayat, Nagar Palika or Nagar Nigam in towns and cities — under the Registration of Births and Deaths Act 1969. District administrations such as Sagar and Jabalpur route citizens to the Lok Sewa Kendra and the MP e-Nagarpalika portal for the certificate.

Within how many days must a birth be registered in Madhya Pradesh?

Every birth must be reported and registered within 21 days at the place of its occurrence, as the Madhya Pradesh district administrations state quoting the Registration of Births and Deaths Act 1969. Live births and still births both fall inside that 21-day window, and reporting within it carries no penalty.

What if a birth in Madhya Pradesh is more than 1 year old?

Madhya Pradesh notifies a separate Lok Seva Guarantee service — "जन्म के 1 वर्ष के पश्चात पंजीयन के लिए अनुमति", permission for registration after one year of birth — under the Planning, Economics and Statistics Department, with a time limit of 15 working days in both urban and rural areas. It costs Rs 20 plus legal charges at a Lok Seva Kendra, MP Online kiosk or CSC, and is free if you apply online yourself.

What is a birth non-availability certificate in Madhya Pradesh?

The birth non-availability certificate, "जन्म का अप्राप्यता प्रमाण पत्र", is a Madhya Pradesh Lok Seva Guarantee service that certifies no entry for the birth exists in the register, notified at 7 working days in both urban and rural areas. Madhya Pradesh lists no documents for it and charges Rs 20 plus legal charges at a Lok Seva Kendra, or nothing at all if you apply online.

Can I apply for a Madhya Pradesh birth certificate online?

Yes — Madhya Pradesh district administrations direct citizens to the MP e-Nagarpalika portal for urban birth certificate services, and the state's Lok Seva Guarantee portal handles the birth non-availability certificate and the permission for late registration. The national Civil Registration System portal at dc.crsorgi.gov.in also accepts a birth report, with the certificate delivered electronically to the informant.

Does applying online cost less in Madhya Pradesh?

Yes — Madhya Pradesh charges Rs 20 plus legal charges when the birth non-availability certificate or the delayed registration permission is filed through a Lok Seva Kendra, MP Online kiosk or Common Service Centre, and shows the online application by the citizen as free. That gap is the main reason to file yourself rather than through a kiosk.

Can I add or correct a name on a Madhya Pradesh birth certificate?

Yes — the child's name can be entered in the birth register after registration, and errors of spelling, date or parent's name are corrected under Section 15 of the Registration of Births and Deaths Act 1969 by the same registrar who made the entry, on documentary proof. No other office in Madhya Pradesh can alter the certificate, and a certificate altered outside the register is invalid.

Does a Madhya Pradesh birth certificate expire?

A birth certificate in Madhya Pradesh never expires, because it certifies a permanent entry in the register of the Gram Panchayat or urban local body. An office asking for a "recent" copy is asking for a fresh certified extract of the same entry, which the registrar issues at the per-copy fee.
