Death Certificate in Sikkim
Sikkim state government scheme
Death Certificate in Sikkim is issued by the Registrar of Births and Deaths of the local body where the death occurred, and in districts such as Mangan the district administration names the Chief Medical Officer at the District Hospital as the issuing office. Deaths are reported within 21 days under the Registration of Births and Deaths Act 1969, online through dc.crsorgi.gov.in.
What is a death certificate in Sikkim?
A death certificate in Sikkim is the certified extract of the death register maintained by the Registrar of Births and Deaths of the local body in whose area the death occurred. Sikkim issues it under the Registration of Births and Deaths Act 1969, a central law administered by the state, and the entry is made where the death happened: not where the deceased lived.
Sikkim's registrar network runs through two channels at once, and knowing which one applies saves a family days:
- Urban areas, the Municipal Corporation in Gangtok, and the Municipal Councils and Nagar Panchayats in Namchi, Gyalshing, Mangan, Rangpo, Singtam, Jorethang and the other notified towns, hold the register for their wards.
- Rural areas, the Gram Panchayat Unit holds the register for its villages.
- Health institutions, because Sikkim's civil registration units are attached to the health network, hospitals, CHCs and PHCs report deaths directly. Mangan district administration, for example, names the Chief Medical Officer at the District Hospital, Mangan as the office for vital records, at PIN 737116.
For most families the practical answer is simple. If the death happened in STNM Hospital, a district hospital, a CHC or a PHC, the institution reports it and the certificate is collected from that hospital's registration counter. If the death happened at home, the Gram Panchayat Unit or the Municipal Council of that ward is the registrar.
Since 1 October 2023, the Registration of Births and Deaths (Amendment) Act 2023 applies in Sikkim. Every medical institution must now give a Medical Certificate of Cause of Death to the registrar and a copy to the nearest relative, and the death can be reported online on the national Civil Registration System portal at dc.crsorgi.gov.in, with the certificate delivered electronically to the informant's registered email and mobile number.
Who can and who cannot apply?
You can apply for a Sikkim death certificate if the death occurred inside Sikkim and you are among the persons the 1969 Act treats as informants or interested parties:
- The nearest relative of the deceased, spouse, son, daughter, father, mother, brother or sister.
- The head of the household in which the death occurred.
- The medical officer in charge of the hospital, CHC, PHC or nursing home where the death took place, who reports it directly.
- The jail superintendent for a death in custody, and the officer in charge for a death in a hostel, monastery, dharamshala or other institution.
- The Panchayat President or ward officer for an unclaimed or abandoned body found in that area.
- Any person who later needs a certified copy for bank, insurance, pension, PF, property or succession purposes, on payment of the per-copy fee.
You cannot apply in Sikkim for a death that occurred outside the state, the register is territorial, and a Sikkimese who died in Siliguri, Kalimpong or Delhi is registered by the local body there. You are also not eligible to have a fresh registration made where the death is already registered; the correct route then is a certified copy of the existing entry. A registrar will further refuse where the death is under police investigation and the post-mortem result and cause of death have not been released, and where the applicant can show no relationship to, or interest in, the deceased.
There is no income limit, no community criterion and no Certificate of Identification requirement for a death certificate in Sikkim. Residence matters only in deciding which registrar holds the record.
What documents are required?
- Report of Death in Form 2, signed by the informant.
- Medical Certificate of Cause of Death from the treating hospital, CHC, PHC or nursing home, compulsory from medical institutions since 1 October 2023.
- Cremation or burial record where the death happened at home with no hospital record, issued by the Gram Panchayat Unit, the Municipal Council or the managing committee of the ground.
- Identity proof of the deceased, Aadhaar, EPIC voter ID, PAN, ration card or passport.
- Identity proof of the informant, with proof of relationship where the applicant is not the household head.
- Certificate of Identification, ration card or Panchayat residence certificate of the deceased, used by registrars to confirm jurisdiction.
- Affidavit stating the date, time and place of death, for a home death with no attending doctor and for any delayed registration.
- Magistrate's order under Section 13(3), only where the death is being registered more than one year after it occurred.
How to apply for a death certificate in Sikkim
- Collect the Medical Certificate of Cause of Death. Where the death occurred at STNM Hospital, a district hospital, a CHC, a PHC or a private nursing home, ask the institution for your copy before leaving, the 2023 amendment entitles the nearest relative to one, and the institution simultaneously sends its copy to the registrar.
- For a death at home, obtain the cremation or burial record from the Gram Panchayat Unit, the Municipal Council or the managing committee of the ground. That record substitutes for a hospital record on date and place of death.
- File the Report of Death in Form 2 with the registrar of the area where the death occurred — the hospital registration counter, the Gram Panchayat Unit, or the Municipal Corporation or Municipal Council ward office. Attach the medical certificate or burial record, the identity proof of the deceased and your own identity proof, and proof of relationship.
- Do this within 21 days. Inside 21 days registration is the ordinary process; beyond it, delayed registration under Section 13 needs the permission of the prescribed authority, and beyond one year it needs a magistrate's order.
- Pay the fee quoted at the counter and take the receipt. Order every copy you need in the same application; copies are charged per copy.
- Collect the signed certificate from the same registrar, or download it where the death was reported through dc.crsorgi.gov.in.
How do you report a death online in Sikkim?
Sikkim uses the national Civil Registration System portal at dc.crsorgi.gov.in. Register as a citizen user with a mobile number and email address, choose death reporting, select Sikkim and the district, enter the details of the deceased and of the informant exactly as they appear on the hospital record, and upload the Medical Certificate of Cause of Death or the burial record. The portal issues a registration number on the acknowledgement and routes the report to the registrar holding the register for that area.
Sikkim's own single sign-on citizen portal at sso.sikkim.gov.in carries 45 state services; income certificate, Scheduled Tribe, Scheduled Caste, OBC and Primitive Tribe certificates, marriage certificate, unmarried certificate, employment card, firm registration and more; delivered through the District Administrative Centres. The death certificate is not among the services listed there, which is why the local registrar and the CRS portal remain the route in Sikkim.
How do you check the application status?
Ask the registrar who took the file. Because Sikkim registers deaths at the hospital, Gram Panchayat Unit or Municipal Council that holds the register, the counter that accepted the Form 2 is the only office that can say where the entry stands. For a death reported on dc.crsorgi.gov.in, log in with the same credentials and track the registration number from the acknowledgement. For a hospital death, the Chief Medical Officer's office at the district hospital, the office Mangan district lists for vital records: is the escalation point inside the district.
Why are death certificate applications rejected in Sikkim?
- The death was never reported in Form 2, so there is no register entry to certify.
- The report went to the wrong body, jurisdiction follows the place of death, not the place of residence.
- The name, age or father's or husband's name on the form does not match the hospital record or the identity proof of the deceased.
- No Medical Certificate of Cause of Death and no cremation or burial record was produced for a death at home.
- The death is more than one year old and no magistrate's order under Section 13(3) accompanies the file.
- The death is under police investigation and the cause of death has not been finalised.
- The applicant cannot show any relationship to, or interest in, the deceased.
What does it cost, and what about agents?
Sikkim charges only the government fee quoted at the registrar's counter, per certified copy, plus any late fee prescribed for a delayed registration under Section 13. Registration inside the 21-day window is the ordinary, cheapest route. No registrar, hospital clerk, Panchayat secretary or municipal ward staff member is entitled to anything beyond the counter fee, and the receipt is your proof.
Sikkim's registration units are small and locally staffed, which makes a middleman entirely unnecessary. The same counter that would take an agent's file will take yours. Always insist on the printed receipt, and take any demand for money beyond the listed fee to the Chief Medical Officer at the district hospital or to the District Collectorate.
What do you need the death certificate for?
A Sikkim death certificate is the starting document for closing bank and post office accounts, claiming life insurance, EPFO and state pension dues, transferring the electricity and water connection and the LPG connection, surrendering the ration card entry and the voter roll entry, mutating land in the revenue records, and applying for a legal heir certificate or a succession certificate. Every one of those offices keeps its own copy permanently, which is why families in Sikkim should order several certified copies from the registrar in a single visit rather than returning repeatedly to a Gram Panchayat Unit or a district hospital counter.

Frequently asked questions
Who issues a death certificate in Sikkim?
The Registrar of Births and Deaths of the local body where the death occurred issues the death certificate in Sikkim — the Municipal Corporation, Municipal Council or Nagar Panchayat in urban areas such as Gangtok and Namchi, and the Gram Panchayat Unit in rural areas. Sikkim's registrar network is built around the health system, and district administrations such as Mangan name the Chief Medical Officer at the District Hospital as the issuing office for vital records.
Within how many days must a death be registered in Sikkim?
A death must be reported to the registrar within 21 days of its occurrence in Sikkim, as everywhere in India, under the Registration of Births and Deaths Act 1969. A report made after 21 days is a delayed registration under Section 13 and needs the permission of the prescribed authority; beyond one year it needs an order from a first-class or Executive Magistrate.
Can I apply for a Sikkim death certificate online?
A death in Sikkim is reported online through the national Civil Registration System portal at dc.crsorgi.gov.in, the platform every state uses for births and deaths. Sikkim's own single sign-on services portal at sso.sikkim.gov.in carries 45 state services including income, ST, SC, OBC, marriage and unmarried certificates, but the death certificate is not among the services listed there, so the local registrar or the CRS portal is the route.
Is a medical certificate of cause of death compulsory in Sikkim?
Yes, a medical institution in Sikkim must issue a Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, under the Registration of Births and Deaths (Amendment) Act 2023 in force from 1 October 2023. For a death at home with no attending doctor, the registrar records the cause as unclassified and relies on the cremation or burial record and an affidavit.
Where do I go if the death happened at a hospital in Sikkim?
Go to the records or registration counter of that hospital — STNM Hospital in Gangtok, the district hospital, or the CHC or PHC concerned — because the institution reports the death directly to its attached registrar. Sikkim's civil registration units sit inside the health network, which is why the Chief Medical Officer at a district hospital is the office district administrations list for vital records.
What if the death happened outside Sikkim?
No Sikkim registrar can register or re-issue a death that occurred outside Sikkim. The register is territorial: a Sikkim resident who died in West Bengal, Delhi or anywhere else is registered by the local body where the death occurred, and you must apply to that body. Sikkim offices will accept that certificate for pension, land and succession purposes.
How many copies of a Sikkim death certificate should I order?
Order at least four or five certified copies in one visit, because the bank, the insurer, the EPFO or pension office, the electricity utility and the land revenue office each retain a copy permanently. Each additional copy is charged at the per-copy fee quoted at the registrar's counter, and a second trip to a Gram Panchayat Unit or district hospital costs far more in time.
Does a Sikkim death certificate expire?
A death certificate in Sikkim never expires, because it records a fixed event in a permanent register. An office that asks for a "recent" copy is asking for a fresh certified extract of the same entry, which the registrar will issue at the per-copy fee.
