Financial Assistance to Persons Suffering from Rare Diseases
Haryana state government scheme
Financial Assistance to Persons Suffering from Rare Diseases pays Rs 3,000 per month, with effect from 1 January 2024, to Haryana domiciles of any age diagnosed with a rare disease listed in the National Policy for Rare Diseases 2021, where verified family income is under Rs 3,00,000 a year. Apply free on saralharyana.gov.in.
What is the Haryana rare disease financial assistance scheme?
Financial Assistance to Persons Suffering from Rare Diseases is a state-funded monthly allowance run by the Directorate of Social Justice and Empowerment, Government of Haryana. The scheme recognises that a rare disease imposes a lifelong, high-cost burden on a household; recurring transfusions, enzyme replacement, specialist consultations and travel to a handful of centres that can treat the condition at all, and that ordinary disability or old age pension categories do not capture that burden.
According to the Directorate of Social Justice and Empowerment, the assistance is Rs 3,000 per month with effect from 1 January 2024. Payment is made by Direct Benefit Transfer into the beneficiary's own bank account. Crucially, the department states that this assistance is paid in addition to any benefit the applicant already receives under other social security pension schemes, so a person drawing the Haryana Divyang Pension of Rs 3,200 a month can also draw this Rs 3,000 if separately eligible.
What the scheme is designed to do
- Give households with a rare disease diagnosis a predictable monthly income to set against recurring treatment costs.
- Cover the conditions listed in the National Policy for Rare Diseases, 2021, a list of 55 conditions that includes thalassaemia and haemophilia.
- Use the Centre of Excellence network created under that national policy as the single diagnostic authority, so that eligibility rests on a specialist diagnosis rather than a district-level assessment.
- Reach beneficiaries through the Parivar Pehchan Patra database and DBT.
The state scheme is separate from, and additional to, the central government's one-time treatment support of up to Rs 50 lakh available at Centres of Excellence under the National Policy for Rare Diseases. This Haryana scheme is a recurring monthly allowance, not a treatment grant.
Who is eligible for the Haryana rare disease assistance?
You can receive the assistance if all of the following are true:
- You are a domicile of Haryana.
- You have been diagnosed with a rare disease listed in the National Policy for Rare Diseases, 2021, and the diagnosis was made at a Centre of Excellence notified under that policy.
- Your verified family income is less than Rs 3,00,000 per year.
- You hold a Parivar Pehchan Patra and the family record is verified.
- You hold valid identification, normally Aadhaar, and a bank account that can receive DBT.
There is no age limit in either direction. A newborn diagnosed with a notified condition and an adult in their sixties are equally eligible, and for a minor the guardian's account receives the credit.
You cannot apply if:
- You are not a domicile of Haryana, even if you are currently receiving treatment at a hospital inside the state.
- Your verified family income is Rs 3,00,000 a year or more.
- Your condition is not on the National Policy for Rare Diseases, 2021 list. A chronic or expensive illness is not automatically a rare disease for the purposes of this scheme.
- The diagnosis was made only at a private hospital and has not been confirmed at a notified Centre of Excellence.
- You have no Parivar Pehchan Patra, or the family record is unverified.
The income test is the point on which applications most often fail. The department reads the income figure from the verified Parivar Pehchan Patra, not from an affidavit or a self-declaration. A family whose PPP still carries an old or wrongly captured income above Rs 3 lakh will be rejected even if the real income is well below the limit.
What documents are required?
| Document | Mandatory | Notes |
|---|---|---|
| Centre of Excellence diagnosis certificate | Yes | Rare disease must be from the National Policy for Rare Diseases, 2021 list |
| Parivar Pehchan Patra (Family ID) | Yes | Verified family income must be under Rs 3,00,000 a year |
| Aadhaar card | Yes | Identity plus bank seeding for DBT |
| Haryana domicile or residence proof | Yes | Applicant must be a domicile of the state |
| Bank passbook | Yes | Aadhaar-seeded account; guardian's account for a minor |
| Age proof | No | No age bar, but date of birth is captured in the form |
| Treatment records and prescriptions | No | Useful supporting evidence for a recent diagnosis |
Two practical steps save weeks. First, check the current disease list and the list of notified Centres of Excellence on the Health Department portal at haryanahealth.gov.in before applying, because a diagnosis from a hospital that is not on the notified list will be sent back. Second, update the Parivar Pehchan Patra income field at meraparivar.haryana.gov.in and wait for verification to show complete before filing.
How to apply for the Haryana rare disease assistance online
- Confirm your condition appears on the National Policy for Rare Diseases, 2021 list published by the Health Department, Haryana at haryanahealth.gov.in.
- Obtain a diagnosis certificate from a notified Centre of Excellence. If your treatment so far has been at a private hospital, ask for a referral so that the diagnosis can be formally confirmed.
- Update the Parivar Pehchan Patra at meraparivar.haryana.gov.in so that the family income is recorded correctly and verified as below Rs 3,00,000.
- Open the Antyodaya-SARAL portal at saralharyana.gov.in and register with your mobile number and email, or sign in to an existing account.
- Select Apply for Services, search for the Social Justice and Empowerment Department service for financial assistance to persons suffering from rare diseases, and open the form.
- Fill in the Family ID, Aadhaar number, disease name, Centre of Excellence details, domicile particulars and bank account details.
- Upload the diagnosis certificate, domicile proof and bank passbook page within the file size limits shown, then submit.
- Save the Application Reference ID displayed on screen and sent by SMS, then wait for verification by the District Social Welfare Officer.
How to apply offline
- Download the application form for financial assistance to persons suffering from rare diseases from socialjusticehry.gov.in, or collect it at the District Social Welfare Office.
- Complete the form and attach self-attested copies of the Centre of Excellence diagnosis certificate, Family ID, Aadhaar, domicile proof and bank passbook.
- Submit it at a SARAL Kendra, a Common Service Centre or the District Social Welfare Office, and ask the operator to file it on the Antyodaya-SARAL portal.
- Collect the printed acknowledgement carrying the Application Reference ID before leaving the counter.
How much is paid and when?
The scheme pays Rs 3,000 per month, effective 1 January 2024, credited by Direct Benefit Transfer to the Aadhaar-seeded bank account of the beneficiary once sanction is issued. There is no lump sum component and no annual re-application requirement.
Assistance continues as long as the beneficiary remains a Haryana domicile, the family income stays below the Rs 3,00,000 threshold, and the diagnosis on record remains valid. Because this allowance is explicitly additional to other social security pensions, an eligible person may draw both this and, for example, the Rs 3,200 monthly Divyang pension, subject to meeting each scheme's own criteria separately.
How to check application and payment status
- Open saralharyana.gov.in and enter the Application Reference ID in the Track Application Status box on the home page.
- Or sign in to the SARAL account used for the application and open Track Application Status for the full case history and any query raised by the verifying officer.
- After sanction, check the bank passbook or statement first if the monthly credit does not arrive. A sanctioned case with no credit is usually a bank-seeding failure, not a rejection.
Why applications get rejected, and how to fix them
- Family income in the PPP above Rs 3 lakh. The most common cause. Correct the income entry at meraparivar.haryana.gov.in and apply again after verification completes.
- Diagnosis not from a notified Centre of Excellence — obtain a referral and a fresh certificate from a notified centre.
- Condition not on the National Policy for Rare Diseases, 2021 list. This cannot be cured by paperwork; check the list at haryanahealth.gov.in before applying.
- Domicile proof missing or inconsistent with the Family ID address.
- Bank account not Aadhaar-seeded, so DBT fails after sanction.
- Name mismatch across the Aadhaar, the diagnosis certificate and the bank record; reconcile all three spellings before reapplying.
Grievance and contact route
- District Social Welfare Officer of your district is the first authority for verification disputes and sanction delays.
- Antyodaya-SARAL helpline: 0172-3968400, for portal and tracking problems.
- Directorate of Social Justice and Empowerment, Haryana, at socialjusticehry.gov.in, for scheme policy and notification queries.
- Health Department, Haryana, at haryanahealth.gov.in, for the rare disease list and Centre of Excellence questions.
- Parivar Pehchan Patra grievances are raised separately at meraparivar.haryana.gov.in and must be resolved before the case can move.
Applying is free. No agent can charge a fee to add a name to the beneficiary list, and no government official will ask for your OTP or bank password.

Frequently asked questions
How much does Haryana pay for rare diseases?
Rs 3,000 per month is paid, with effect from 1 January 2024, according to the Directorate of Social Justice and Empowerment. The amount is paid by Direct Benefit Transfer and is given in addition to any other social security pension the applicant already draws.
What is the income limit for the Haryana rare disease assistance?
Verified family income must be less than Rs 3,00,000 per year. The figure the department uses is the income recorded and verified in the Parivar Pehchan Patra, not a self-declared figure, so correct the Family ID first if the income shown there is out of date.
Which diseases are covered under the Haryana rare disease scheme?
The diseases listed in the National Policy for Rare Diseases, 2021 are covered — a list of 55 conditions including thalassaemia and haemophilia. The Health Department, Haryana publishes the current list and the notified Centres of Excellence at haryanahealth.gov.in.
Is there an age limit for the rare disease assistance in Haryana?
No, the scheme applies to persons of any age. A child diagnosed with a notified rare disease is eligible on the same terms as an adult, with the guardian's account receiving the Direct Benefit Transfer.
Can I get this assistance along with a disability or old age pension?
Yes. The Directorate states that this financial assistance is provided in addition to any benefit the applicant receives under other social security pension schemes, so it does not replace an old age, widow or Divyang pension already being drawn.
Where do I apply for the Haryana rare disease financial assistance?
Apply free on the Antyodaya-SARAL portal at saralharyana.gov.in. The Directorate also publishes a downloadable application form on socialjusticehry.gov.in, which can be submitted through a SARAL Kendra, a Common Service Centre or the District Social Welfare Office.
Does a private hospital diagnosis qualify for the scheme?
No. The diagnosis must come from a Centre of Excellence notified under the National Policy for Rare Diseases, 2021. A private hospital report on its own will not clear verification, though it can support a referral to a Centre of Excellence for formal diagnosis.
How do I check my rare disease assistance application or payment status?
Enter the Application Reference ID in the Track Application Status box on saralharyana.gov.in, or sign in to the SARAL account used for the application and open Track Application Status for the full case history. If the monthly credit does not arrive after sanction, check the bank passbook first — a sanctioned case with no credit is usually a bank-seeding failure, not a rejection.
