What is a death certificate in Telangana?

A death certificate in Telangana is the certified extract of the death register maintained by the Registrar of Births and Deaths of the local body in whose area the death occurred. Telangana issues it under the Registration of Births and Deaths Act 1969, a central law administered by the state, and delivers it to citizens through MeeSeva, the state's single-window citizen service network.

That split: register with the local body, counter at MeeSeva, is the single thing most families get wrong. The Registrar of Births and Deaths is:

  • Greater Hyderabad Municipal Corporation (GHMC) for deaths inside GHMC limits;
  • the Municipal Corporation or Municipality for Warangal, Nizamabad, Karimnagar, Khammam, Ramagundam and the other urban local bodies;
  • the Gram Panchayat for rural Telangana.

You do not visit those offices to collect the certificate. GHMC states plainly that citizens "can take such Certificates from Mee-Seva center only". The corporation approves the entry; MeeSeva prints it.

GHMC also runs the fastest institutional pipeline in the state. Every hospital must submit Form 1 (birth legal information) and Form 2 (death legal information) within 7 days of the event. For a death in a hospital, the hospital enters the event online itself, the Registrar approves it after verification, and the family simply collects the certificate at a MeeSeva counter. GHMC publishes the death certificate fee as Rs 45.

Since 1 October 2023, the Registration of Births and Deaths (Amendment) Act 2023 applies across Telangana. Every medical institution must hand a Medical Certificate of Cause of Death to the registrar and give a copy to the nearest relative, and the death can be reported directly on the national Civil Registration System portal at dc.crsorgi.gov.in.

Who can and who cannot apply?

You can apply for a Telangana death certificate if the death occurred inside Telangana and you are one of the persons the 1969 Act treats as an informant or an interested party:

  • The nearest relative of the deceased: spouse, son, daughter, father, mother, brother or sister.
  • The head of the household in which the death occurred.
  • The medical officer in charge of the hospital or nursing home where the death took place, who reports it directly under the 7-day rule.
  • The jail superintendent for a death in custody, and the officer in charge for a death in a hostel or other institution.
  • The Panchayat Secretary or ward officer for an unclaimed or abandoned body.
  • Any person who later needs a certified copy for bank, insurance, pension, PF, property or succession purposes, on payment of the Rs 45 fee per copy.

You cannot apply in Telangana for a death that occurred outside the state. The register is territorial: a Telangana resident who died in Andhra Pradesh, Karnataka or Maharashtra is registered by the local body there, and no Telangana registrar or MeeSeva operator can convert or re-issue that entry. You are also not eligible to have a fresh registration made where the death is already registered, the route then is a certified copy, not a new entry. A registrar will refuse where the death is under police investigation and the post-mortem report and cause of death have not been released, and where the applicant can show no relationship to, or interest in, the deceased.

There is no income limit, no caste criterion and no domicile test for a death certificate in Telangana. Residence matters only in deciding which local body holds the record.

What documents are required?

  • Report of Death in Form 2: filed by the hospital within 7 days for an institutional death, or by the family on the GHMC Death Home Form-2 for a death at home.
  • Medical Certificate of Cause of Death from the treating hospital or nursing home, compulsory from medical institutions since 1 October 2023.
  • Burial ground or cremation receipt, with the deceased's details entered in the burial ground app for a home death in GHMC limits.
  • Identity proof of the deceased, Aadhaar, voter ID, PAN, ration card or passport.
  • Identity proof of the informant, with proof of relationship where the applicant is not the household head.
  • Proof of address of the deceased at the time of death, ration card, electricity bill or property tax receipt.
  • Affidavit stating the date, time and place of death, for a home death with no attending doctor and for delayed registration.
  • Executive Magistrate's or RDO's order, only for the separate Late Registration of Death service where the death is more than one year old.

How to apply for a death certificate in Telangana

  1. Collect the Medical Certificate of Cause of Death from the hospital. For a death in a hospital or nursing home in Telangana, the institution reports the death online itself and must file Form 2 within 7 days. Ask for the Form 2 acknowledgement and your copy of the medical certificate before leaving the hospital.
  2. For a death at home, get the burial ground receipt first. GHMC directs relatives of a person who died at home to obtain the burial ground receipt and register the deceased's details in the burial ground app. Until that entry is made, MeeSeva cannot issue the certificate.
  3. Fill the application form. Collect the death certificate application form at a MeeSeva centre for Rs 10, or download it free from your district portal — Nizamabad and other Telangana district sites publish both the "Application for Death Certificate" and the "GHMC Death Home Form-2".
  4. Submit it at the nearest MeeSeva centre with the medical certificate or burial receipt, the identity proof of the deceased, your own identity proof and proof of relationship. The operator scans and uploads the file against the local body's register.
  5. Pay the fee, GHMC publishes Rs 45 for the death certificate, and take the printed receipt with the application ID on it. Order every copy you need in the same transaction.
  6. Collect the certificate from the MeeSeva centre once the Registrar has approved the entry, using the same application ID.

How to apply where the death is more than a year old

Telangana runs Late Registration of Death as a distinct service listed alongside the ordinary death certificate on district portals. Under Section 13(3) of the Registration of Births and Deaths Act 1969, a death registered more than one year after it occurred requires an order from the Executive Magistrate or the Revenue Divisional Officer, supported by an affidavit stating the date, time and place of death and by whatever secondary evidence exists, the burial ground record, hospital papers, or a Panchayat or ward certificate. Take that order to the MeeSeva centre with the ordinary document set; the registrar then makes the entry and MeeSeva issues the certificate.

How do you check the status and verify the certificate?

Use the application ID on the MeeSeva receipt to track the file, either at the same MeeSeva centre or on the MeeSeva portal at ts.meeseva.telangana.gov.in. Where the death was reported on dc.crsorgi.gov.in, the national portal tracks the same registration number.

To confirm that a certificate is genuine, which banks, insurers, the EPFO and civil courts routinely do, use the GHMC verification page at bnd.ghmc.gov.in/Death_Certificate.aspx, which checks a death certificate against GHMC's own register.

Why are death certificate applications rejected in Telangana?

  • The death was never reported in Form 2, so no register entry exists to certify.
  • The hospital missed the 7-day Form 2 deadline and the event was never entered online.
  • For a home death in GHMC limits, the burial ground app entry was never made.
  • The application went to the wrong local body: jurisdiction follows the place of death, not the place of residence.
  • The name, age or father's or husband's name on the form does not match the hospital record or the identity proof of the deceased.
  • The death is more than one year old and was filed as an ordinary application instead of through Late Registration of Death with a magistrate's order.
  • The death is under police investigation and the cause of death has not been finalised.

What does it cost, and what about agents?

Telangana charges the Rs 45 GHMC death certificate fee at the MeeSeva counter, plus Rs 10 if you buy the printed application form there instead of downloading it free. Registration inside the 21-day window carries no penalty. Those are the whole lawful charges for an ordinary case, and the MeeSeva operator must give you a printed receipt for whatever you pay.

Agents outside hospitals and burial grounds in Hyderabad, Warangal and Karimnagar offer a "same-day death certificate" for hundreds or thousands of rupees. They are selling a form you can file yourself at any MeeSeva centre, and the file still waits for the same Registrar's approval. Insist on the MeeSeva receipt, pay only the counter fee, and report any demand for more to the GHMC Registrar of Births and Deaths or the district MeeSeva office, Nizamabad, for instance, publishes its MeeSeva centre number as 040-65656971.

What do you need the death certificate for?

A Telangana death certificate is the starting document for closing bank and post office accounts, claiming life insurance and EPFO or Aasara pension dues, transferring the LPG connection and the electricity meter, mutating agricultural land in Dharani and urban property in the municipal records, surrendering the ration card entry and the voter roll entry, and applying for a legal heir certificate from the Tahsildar or a succession certificate from the civil court. Every one of those offices retains its own copy permanently, which is why families should order five or six copies at Rs 45 each in one MeeSeva transaction rather than returning to the counter each time a new office asks.

Documents required

Report of Death in Form 2
The statutory death report under the Registration of Births and Deaths Act 1969. GHMC requires each hospital to submit Form 2 within 7 days of the death event; for a death at home, Telangana district portals publish a "GHMC Death Home Form-2" for the family to file.
Medical Certificate of Cause of Death
Issued by the treating hospital or nursing home. The Registration of Births and Deaths (Amendment) Act 2023, in force from 1 October 2023, makes it compulsory for a medical institution to give this certificate to the registrar and a copy to the nearest relative.
Burial ground or cremation receipt
For a death at home in GHMC limits, the relatives must obtain the burial ground receipt and have the deceased's details entered in the burial ground app before the death certificate can be collected from MeeSeva.
Identity proof of the deceased
Aadhaar, voter ID, PAN, ration card or passport of the person who died, so the registrar can match the name, age and address against the register.
Identity proof of the informant or applicant
Aadhaar or another government photo ID of the relative applying, plus proof of the relationship where the applicant is not the household head.
Proof of address of the deceased at the time of deathoptional
Ration card, electricity bill or property tax receipt, used by the registrar to confirm the death falls within that local body's jurisdiction.
Affidavit stating date, time and place of deathoptional
Required for delayed registration and for a death at home where no doctor attended.
Order of the Executive Magistrate or Revenue Divisional Officeroptional
Needed only for the separate "Late Registration of Death" service, where the death is being registered more than one year after it occurred, under Section 13(3) of the Registration of Births and Deaths Act 1969.

Frequently asked questions

Who issues a death certificate in Telangana?

The Registrar of Births and Deaths of the local body where the death occurred issues the death certificate in Telangana — the Greater Hyderabad Municipal Corporation for Hyderabad, the Municipal Corporation or Municipality for other towns, and the Gram Panchayat in rural areas. The certificate is delivered to citizens through MeeSeva, so the register sits with the local body while the counter you visit is a MeeSeva centre.

What does a death certificate cost in Telangana?

GHMC publishes the death certificate fee as Rs 45, payable at the MeeSeva centre. The application form itself can be collected from a MeeSeva centre or downloaded free from the district portal. Registration of the death within 21 days carries no penalty; delayed registration is a separate MeeSeva service with its own prescribed fee.

Within how many days must a death be registered in Telangana?

A death must be reported to the registrar within 21 days of its occurrence in Telangana, as everywhere in India, under the Registration of Births and Deaths Act 1969. GHMC imposes a tighter internal deadline on institutions — every hospital must submit Form 2 for a death within 7 days of the event. Registrations after one year run through the separate Late Registration of Death service.

How do I get a death certificate if the death happened at home in Hyderabad?

Obtain the burial ground receipt and have the deceased's details entered in the burial ground app, as GHMC directs, and then collect the death certificate from a MeeSeva centre once the record is updated. GHMC also publishes a "GHMC Death Home Form-2" for home deaths, which Telangana district portals carry as a downloadable form.

How do I get a death certificate if the death happened in a hospital?

Nothing needs to be filed by the family in the ordinary case — GHMC states that where the death occurred in a hospital, the hospital enters the event online directly, the Registrar of Births and Deaths approves it after verification, and the citizen then collects the certificate from a MeeSeva centre only. Ask the hospital for the Medical Certificate of Cause of Death and the Form 2 acknowledgement before you leave.

How do I verify a Telangana death certificate?

Use the GHMC birth-and-death verification page at bnd.ghmc.gov.in/Death_Certificate.aspx, which lets you check a death certificate issued within GHMC limits against the corporation's own register. Banks, insurers and courts use the same page when they want to confirm a certificate is genuine.

Can I apply for a Telangana death certificate online?

Telangana delivers the death certificate through the MeeSeva portal at ts.meeseva.telangana.gov.in, and the death can also be reported on the national Civil Registration System portal at dc.crsorgi.gov.in. In practice most families in Telangana use a MeeSeva centre, because the operator scans the documents, pays the Rs 45 GHMC fee and issues the printed certificate across the counter.

Does a Telangana death certificate expire?

A death certificate in Telangana never expires, because it records a fixed event in the permanent register of the local body. An office that insists on a "recent" copy is asking for a fresh certified extract of the same entry, which MeeSeva will issue again at the same Rs 45 GHMC fee.
