Death Certificate (Mrityu Praman Patra) in Uttar Pradesh
Uttar Pradesh state government scheme
Death Certificate in Uttar Pradesh is applied for on two different portals depending on where the death occurred: e-Nagar Sewa at e-nagarsewaup.gov.in for deaths in a municipal area, and the e-District e-Sathi portal reached from edistrict.up.gov.in for rural areas. The death must be reported within 21 days, and the e-District helpline is 0522-2304706.
Death Certificate in Uttar Pradesh, the Mrityu Praman Patra, is the legal record of a person's death, its date and its place, made under the Registration of Births and Deaths Act 1969 by the local body with jurisdiction over the place of death. No life insurance claim, family pension transfer, mutation of land into the heirs' names, bank account closure or succession proceeding will move in UP without it.
Which portal issues the death certificate in Uttar Pradesh?
Uttar Pradesh is the one large state where the first question is not how to apply but where. UP runs two separate channels for death registration and they do not overlap.
For a death that occurred inside a municipal area, any Nagar Nigam, Nagar Palika Parishad or Nagar Panchayat, the channel is e-Nagar Sewa at e-nagarsewaup.gov.in, the Urban Development Department's portal for urban local body services, which carries birth and death certificates alongside property tax, water and sewerage connections, trade licences and building plan approvals.
For a death that occurred outside a municipal area, the channel is the e-District programme, reached from edistrict.up.gov.in with its citizen front end e-Sathi. UP's e-District portal describes its coverage as certificates, complaints, the public distribution system, pensions, khatauni, revenue cases and employment registration, run by the IT and Electronics Department with NIC.
Getting this wrong is the commonest reason a UP application stalls. A family in a Lucknow or Kanpur ward that files through the rural channel, or a village family that files on e-Nagar Sewa, will find their application sitting with a registrar who has no jurisdiction over the place of death. Establish first whether the address where the death occurred falls inside a municipal boundary, and only then choose the portal.
Who can and cannot apply for a death certificate in Uttar Pradesh?
The informant is normally the nearest relative present at the death, the spouse, a son or daughter, a parent, or the head of the household. Legal heirs and insurance nominees can obtain certified copies afterwards, as can anyone with a genuine legal interest in the record.
For a death in a hospital or nursing home, the institution is the reporting authority and issues the Medical Certificate of Cause of Death. For a death in a jail, the officer in charge reports it. For an unclaimed body or a death in a public place, the officer in charge of the local police station is the informant.
You cannot apply in Uttar Pradesh for a death that occurred outside the state, registration follows the place of occurrence, so a Ghaziabad family whose parent died in a Delhi hospital registers that death in Delhi, not in UP. You cannot register the same death twice; where the entry already exists, ask for a certified copy rather than a new registration. A person with no relationship to the deceased and no legal interest cannot demand a copy. And a death more than a year old cannot be registered through the counter route at all, it requires the order of a first-class magistrate before the registrar can act.
What documents are required for a death certificate in Uttar Pradesh?
The core set is the death report in the prescribed statutory form, the Medical Certificate of Cause of Death where the death occurred in a medical institution, identity proof of the deceased, identity proof and relationship proof of the applicant, and proof of the address where the death occurred.
That last one does double duty in UP. Besides fixing the registrar's jurisdiction, it is what tells you which of the two portals to use, so have a document showing the address where the death actually happened, not the family's permanent address if the two differ.
Where the death happened at home with no doctor in attendance there is no medical certificate to produce. UP registrars in that case record the cause as stated by the informant and in practice ask for a letter from the ward parshad in a town or the gram pradhan in a village confirming the death, together with the cremation or burial receipt from the shamshan ghat or kabristan. Carry both.
Delayed cases need more. Beyond 30 days, add an affidavit and the written permission of the District Registrar. Beyond one year, the magistrate's order is the document that unlocks the registration and nothing substitutes for it.
How to apply for a death certificate in Uttar Pradesh online
- Establish whether the place of death falls inside a Nagar Nigam, Nagar Palika Parishad or Nagar Panchayat area. If it does, use e-nagarsewaup.gov.in; if it does not, use the e-District e-Sathi portal reached from edistrict.up.gov.in.
- Register a citizen account on the chosen portal with your mobile number and email, or sign in if you already have one, and open the death certificate or death registration service.
- Select the district and the local body covering the place where the death occurred. On e-Nagar Sewa this means picking the correct urban local body from the list.
- Fill the death details exactly as they appear on the hospital record — full name of the deceased, father's or husband's name, date and time of death, place of death, age and sex, and the certified cause where a medical certificate exists.
- Enter the applicant's details and the relationship to the deceased. Banks and insurers check this field against their own nominee records, so state the relationship precisely.
- Upload the documents in the formats and sizes the portal specifies, the medical certificate or the affidavit with the parshad's or pradhan's letter, the cremation or burial receipt, and the identity proofs. Blurred scans are the most common avoidable rejection.
- Pay at the payment step. Registration inside 21 days attracts no fee; beyond that the late fee applies, and the certificate copy carries its own small notified charge.
- Submit, save the application or registration number from the acknowledgement, then track it and download the certificate once it is issued.
How to apply offline at the local body or a Jan Seva Kendra
- Go to the office of the Registrar for the place where the death occurred. The Nagar Nigam, Nagar Palika Parishad or Nagar Panchayat office in a town, or the block or gram panchayat office in a village.
- Submit the death report form with the medical certificate of cause of death, or the affidavit and parshad's or pradhan's letter for a home death, along with the cremation or burial receipt and the identity documents.
- The counter files the same electronic application, collects the notified fee and issues an acknowledgement with the application number.
- Collect the certificate after approval, or download it from the portal once the status shows issued.
Jan Seva Kendras and Common Service Centres run the same workflow for families without internet access. They may charge only their notified service charge on top of the government fee.
How do I check the status of my application?
Track on the same portal you applied on, using the application or registration number. For e-District and e-Sathi problems, the UP e-District helpline in Lucknow is 0522-2304706, open Monday to Saturday between 10 AM and 5 PM excluding government holidays, with email support at ceghelpdesk@gmail.com. For an urban application, the Nagar Nigam or Nagar Palika birth and death registration section is the office that actually holds the file.
Do not file a second application while the first is pending. Duplicate applications for the same death slow the original and can produce conflicting entries that then need a correction proceeding.
Does a UP death certificate expire, and what about old records?
A death certificate never expires and never needs renewal. The register entry is permanent and certified copies can be drawn from it decades later. Two practical caveats apply in Uttar Pradesh. First, entries that predate digitisation may not appear on either portal, and in that case the certified copy has to be obtained from the local body or block office holding the physical register. Second, families consistently under-order copies. Life insurance, the bank, the pension section, the mutation file at the tehsil, and each electricity, water and LPG connection transfer will each keep an original, so ask for four or five at the outset.
Why do UP death certificate applications get rejected?
The dominant cause is the portal and jurisdiction mismatch described above. An urban death filed through the rural channel or the reverse. After that come a name that does not match the deceased's Aadhaar or voter ID, a missing medical certificate for a death that occurred in a hospital, a home death filed without the parshad's or pradhan's letter and the cremation receipt, unreadable uploads, and a report filed after 30 days without the District Registrar's written permission. An application for a death older than a year without a magistrate's order does not proceed at all.
Where do I complain if the certificate is delayed?
Start at the Registrar's office. The Nagar Nigam or Nagar Palika birth and death section for an urban death, the block office for a rural one, with the application number. Escalate to the District Registrar of Births and Deaths, who in most UP districts sits in the office of the Chief Medical Officer or the District Magistrate, and then to the District Magistrate's public grievance route and the Integrated Grievance Redressal System of the state. For portal faults specifically, use the e-District helpline on 0522-2304706. Keep the acknowledgement, the application number and the fee receipt; every escalation asks for all three.
One last thing, said plainly for a family in the middle of a bereavement. Registering a death within 21 days in Uttar Pradesh carries no fee at all, and the certificate itself costs only the small amount the portal displays. Nobody has to be paid to have it approved, and an agent quoting several hundred rupees for "fast processing" is charging you for a queue he does not control.

Frequently asked questions
Which portal do I use for a death certificate in Uttar Pradesh?
It depends on where the death occurred. A death inside a municipal area — a Nagar Nigam, Nagar Palika Parishad or Nagar Panchayat — is registered through e-Nagar Sewa at e-nagarsewaup.gov.in, the Urban Development Department's portal for urban local body services. A death outside a municipal area goes through the e-District programme reached from edistrict.up.gov.in and its citizen front end e-Sathi. Applying on the wrong one is the single most common reason a UP application stalls.
How many days do I have to register a death in Uttar Pradesh?
A death must be reported within 21 days of its occurrence, the statutory window under the Registration of Births and Deaths Act 1969. No fee is charged for registration inside that window. A late fee applies between 21 and 30 days, a higher one between 30 days and one year with the District Registrar's written permission and an affidavit, and beyond one year the death can only be registered on the order of a first-class magistrate.
Is registration free if I apply within 21 days in Uttar Pradesh?
Yes, no fee is charged for registering a death within 21 days of its occurrence. A small fee applies for registration between 21 and 30 days and a slightly higher fee after 30 days. The certificate copy itself carries a small notified charge that the portal displays at the payment step.
Who issues the death certificate in Uttar Pradesh?
The Registrar of Births and Deaths for the area issues it — the Nagar Nigam, Nagar Palika Parishad or Nagar Panchayat in a town and the rural registrar through the block and gram panchayat structure in a village. This is a different authority from the Tehsildar, who signs UP income, caste and domicile certificates.
Is a medical certificate of cause of death compulsory in Uttar Pradesh?
Yes for any death that occurred in a hospital, nursing home or other medical institution — the institution issues the Medical Certificate of Cause of Death and reports the death itself. For a death at home with no doctor in attendance, the Registrar records the cause as stated by the informant, usually with a supporting letter from the ward parshad or the gram pradhan and the cremation or burial receipt.
How do I check my UP death certificate application status?
Track the application on the same portal you applied on, using the application or registration number from the acknowledgement — e-Nagar Sewa for an urban death, e-District or e-Sathi for a rural one. The e-District helpline in Lucknow is 0522-2304706, open Monday to Saturday 10 AM to 5 PM except government holidays, with support at ceghelpdesk@gmail.com.
Can I download a UP death certificate that was registered years ago?
Yes, provided the entry exists in the register of the local body where the death occurred. Older records that predate digitisation may not appear on the portal at all, and in that case the certified copy has to be obtained from the Nagar Nigam, Nagar Palika or block office that holds the physical register. A death certificate never expires, so an old entry remains fully valid.
How many copies of the death certificate should I order?
Order at least four or five. Life insurance, the bank, the pension section, the mutation file at the tehsil and each electricity, water and LPG connection transfer will each retain an original, and a fresh copy later means a fresh application and another wait every time.
